OUTSTANDING YEAR END REVIEW RATINGS

2015
Mid-Year Comments – Final Comments for 2015 not yet posted: I cannot say
enough about Dana’s performance. First, after two consecutive years (2013, 2014)
finishing as a President Club Winner, Dana at the Mid-year point of 2015 turned
in a terrific sales performance. As of June, Dana is ranked 25 of 182 in the nation,
putting her at the Gold Award Level. She did this while serving as my Interim DM
from March 1st, covering both responsibilities with excellent results. Dana attended
a DM staff meeting, ran the district portions at two launch meetings, and delivered
exceptional sales performance in her territory, and the district. Additionally, she
was selected for a joint presentation at our Region POA in January, successfully
got Pradaxa added to the Ft. Wood formulary, and regained Spiriva formulary
status at Mercy Springfield. Dana continues to be the strongest member of an
exceptional district, and is completely prepared to make the move into a district
manager position. I’m excited to see her finish in the second half! Note: (I finished
2015 as a President’s Club winner for my local territory. The district I managed
finished #2/22, which if eligible, would have qualified me for a President’s Club win
for Great Plains).

2014
Dana has had 2 truly Outstanding years! I can only say that I believe this
performance is due entirely to the hard work she puts into her terrritory, the
tremendous level of collaborative relationships she has developed, and her
unwillingness to ever sit on her accomplishments. Dana will be serving as Interim
DM while I am on rotation in 2015, and this should be a great step towards her goal
of DM. I completely support her in that goal.

2013
Dana’s performance was Outstanding across the board this year. Her sales
performance placed her in the Presidents Club at #2 in the Region and #4 in
the NATION. Dana was an exceptional example for teamwork, collaboration and
communication. She took on additional duties as the district’s Computer Expert,
and provided tremendous support during an unusually difficult transition. She is
a team leader in her market with her retail counterparts, and within the district
through her participation at POAs and conference calls. I am extremely proud of
her perfomance, and excited for her potential in 2014.

2007

First of all I want to congratulate you on an excellent sales performance this year. You and your
team have worked throughout the year to ensure your physicians were informed about the
benefits our products provide to their practice and ultimately to their patients. Your efforts on
the team have resulted in placing 4th in the Zone President’s Club Rankings and thatplaces
you and your team at the Silver level of the Extreme Rewards contest. Thank you for all of your
contributions to the district this year. Your work in the hospitals has been instrumental in the
increased awareness and use of Aggrenox this year. You have worked with numerous hospital
accounts throughout your territory to ensure the physicians, pharmacists and administrators
were informed of the current Stroke Guidelines and when asked by your hospitals you were
able to provideadditional information on the development of stroke protocols. With the
significant changes in our speaker department as well as the contracting issues we had earlier
in the year, your work as a speaker liaison was challenging to say the least. I appreciate the
additional time and effort you placed on this responsibility to ensure our speakers were
informed of changes to company procedures. You assisted both our Specialty Teams as
well as our Primary Care teams as you coordinated speaker programs throughout the year. I
would like to thank you for your leadership on your Neurology team as well as in the District
this year. Your contributions to your team’s routing and call plan has helped the team exceed
expectations on this year’s call plan objectives. You also provided key insight to the team
during our district meetings. You demonstrated excellent selling messages and shared insight
into the use of our promotional material during our meetings which in turn helped everyone on
the team sell more effectively.

Description of Achievement
Dana uncovered an opportunity with one of her lead accounts when she heard about a BI intia-
tive called E Vouchers. Dana reached out to me, and subsequently to our Geo Team RD coun-
terpart, to learn more about the program and how to pursue it. Dana developed an internal
champion at the account, and enlisted the support of the RD to establish a BI portol with
McKesson, and established E Voucher availability for the account. Dana coordinated with her
pulmonary specialty counterpart to test the program in key offices. Following that, Dana intiated
a team wide conference call, which she led.
