For objective 14 To be able to impact of regularly reviewing my personal teaching practice
existing laws and regulations evidence for Objective 14. I have attached here figure (a) showing
that I normally start class with a prayer with my students showing Section 4, Article 11 of the
Code of Ethics for Professional Teachers, where a teacher should always recognize the Almighty
Goad as a guide of our own destiny and of the destinies of the nation. Figure (b) shows evidence
of using Rubrics as a generally accepted procedure of evaluation and assessment according to
Section 1, Article VIII of the Code of Ethics. Figure (c) which is a copy of my e-class record is
also an evidence of using generally accepted procedures in determining the academic marks and
the promotion of learners in the subject that I handle from the same Article of the Code of
Ethics for Professional Teachers.

Annotation
To prove that I was able to sustain engagement with the learners, parents/ guardians and
other stakeholders regarding school policies and procedures through school community
partnerships/ attached here are picture as evidence of my participation in a school- community
partnership. These pictures show our partnerships with the students, learners and other external
stakeholders. There were certificates showing our partnerships with our community, the
barangay Santolan where we distributed food packs to our less fortunate students last
Deceember and last March for those families whose houses got burned.

Mga Dapat Isaalang-alang Sa Pagbuo Ng Personal Na Pahayag ng
Misyon sa Buhay
Kahalagahan ng Pagkakaroon ng Direskyon ng Buhay ng Tao
ito ay sumasalamin sa kung ano ang iyong adhikain o layunin sa ating buhay
Ito ang nagsisilbinating gabay o batayan ukol sa ating mga kilos at pagpapasya sa buhay
binibigyang kabuluhan nito ang pamumuhay natin bilang isang indibidwal sa mundo sa
pamamagitan ng pagkakaroon natin ng magandang tunguhin sa buhay
Sa pagbuo mo ng PPMB, dapat na masasagot nito ang mga katanungang:
1. Ano ang layunin ko sa buhay?
2. Ano-ano ang aking mga pagpapahalaga?
3. Ano ang mga nais kong marating?
4. Sino ang mga tao na maaari kong makasama at maging kaagapay sa aking buhay?
Ayon kay Stephen Covey, nagkakaroon ng kapangyarihan ang misyon natin sa buhay kung ito
ay:
a. Mayroong kaugnayan sa kaloob-looban ng sarili upang mailabas ang kahulugan niya bilang isang
tao.
b. Nagagamit at naibabahagi nang tama at may kahusayan bilang pagpapahayag n gating pagka
bukod-tangi.
c. Nagagampanan nang may balance ang mga tungkulin sa pamilya, trabaho, pamayanan at iba pa.
d. Isinulat upang magsilbing inspirasyon, hindi upang ipagyabang sa iba.
MGA DAPAT TANDAAN SA PAGBUO NG PERSONAL NA PAHAYAG NG MISYON SA BUHAY
1. Huwag Sumuko. Maging determinado sa pag-abot ng mga layuning itnakda sa PPMB. Lahat ng
bagay ay may proseso at hindi madalian.
2. Huwag ikompara ang sarili sa iba. Ang lahat ng tao ay may iba’t-ibang kalakasan at kahinaan,
maging mga katangian, talento at kakayahan. Kumilos ng naaayon sa sariling kapasidad. Matutong
maging masaya sa maliit man o malaking tagumpay.
3. Humanap ng Solusyon kaysa magreklamo. Gamitin ang malikhain at malawak na pag-iisip upang
gawing mas mainam ang paggawa ng isang bagay. Kung nahihirapan, maaaring humingi ng tulong
sa iba, at ganun rin naman, kung nangangailangan ng tulong ang iba ay huwag magdalawang isip na
tumutlong sa kanila.
4. Huwag Kalimutan ang krayteryang SMART. Sa pagtatakda ng mga layunin sa PPMB ay laging isa-
alang-alang ang krayteryang SMART. Kailangan na ito ay tiyak (specific), nasusukat (measurable),
naaabot (attainable), angkop (relevant) at nasusukat ng panahon (time-bound).

Annotation
To show that I have applied a personal philosophy of teaching that is learner-
centered, I have attached here a copy of one of my lesson plans where I have indicated
annotations in green shades. I am a believer of the Humanistic Philosophy of Education
from Abraham Maslow and Carl Rogers. This philosophy focuses on both both cognitive
and affective aspects of students learning, which I also apply on my teaching.The lesson
plan example shows that I was able to apply this philosophy in all three areas of lesson
planning from the learning objective, instructional activities and assessments as
mentioned in the annotations
