Jump Rope for Heart - 2015 February 23 – 26
Matsunaga Elementary School will participate in “Jump Rope for Heart”. This event is an educational
program and fundraiser co-sponsored by the American Heart Association and the American Alliance for
Health, Physical Education, Recreation, and Dance. As an incentive for our participation in the program, 3-5%
of the money raised will be returned to the school for the purchase of physical education materials. We have
been listed in the Top 100 Schools in the United States several times in the past for this event!
AGAIN THIS YEAR - Website donations may be collected and submitted! Directions for the on-line
donation process are with the pledge envelope your child will bring home after the permission slip is signed
and returned to school. This makes donations easier, faster, and safer because less cash is carried to school.
Yes, cash or donations by check may be sent to school using the official Jump Rope for Heart Pledge
Collection Envelope. Children can also attend the event without making a donation as long as a signed
permission slip has been turned in on or before Wednesday, February 18th.
To ensure that every student in our program has an opportunity to participate, we have planned the event as a
“during the student day” activity. Each class is scheduled to attend Jump Rope for Heart during the week of
February 23 – February 26. Look on the back of this letter to see the event schedule for your child’s class.
Students with signed permission slips will come to the gym to participate. Students not having a signed
permission slip for Jump Rope for Heart will attend an alternative music or art class.
We have discussed the event with the students. We discussed the benefits of a heart healthy lifestyle,
participation as community service, the date, time, registration procedures, pledge envelope guidelines and on-
line donations, how everyone of every ability level can participate, special jumping contests, door prizes, and
the incentive prize structure offered by the American Heart Association. We have also been practicing rope
jumping skills in PE class. Kindergarten and first graders are learning many floor rope skills and limbo jumping.
First and second graders learn to jump over a rope they turn themselves. Third, fourth, and fifth grades create
jump rope routines, learn long-rope jumping skills, and use other equipment such as jump bands, skip-its, and
jump sticks. The Jump Rope for Heart event is a celebration of these skills!
Signed permission slips must be returned by Wednesday, February 18th for your child to be eligible for
door prize drawings! Jump Rope for Heart Pledge Collections Envelopes will be sent home once the
signed permission slip has been returned to school. Each student that turns in the signed permission slip by
February 18th and returns $5.00 or more will receive a “Splatter” Duck and Lanyard on the day of the
event. All other prizes are ordered and will arrive by mid-May. Remember, your child can participate without
making a contribution. Here is the incentive prize structure:

$250.00 or more – Portable Capsule Speaker and prizes listed above.
$500 or more – Smakaball and prizes listed above.
$1,000.00 or more – Zike Z150 and incentive prizes listed above.
Reminder: All incentive prizes will be ordered after the event. Your child should receive them by mid- May.

JUMP ROPE FOR HEART
RELEASE and INDEMNIFICATION (Permission Slip)
Jump Rope For Heart involves jumping rope – a strenuous activity that may include risks such as, but not limited to,
physical exertion, falls and contact with other participants. I hereby expressly assume all risks, including personal
injury and fatality, arising out of my child’s participation in Jump Rope For Heart and related activities.
It is my responsibility to ensure that my child’s clothing and equipment are properly fitted and appropriate for use in
this event. I represent and warrant that my child is physically fit and able to participate in Jump Rope For Heart. My
child agrees to stop and request assistance if he/she experiences any symptoms such as, but not limited to, dizziness,
excessive fatigue, shortness of breath, pain or any other conditions that would make it difficult or unsafe to continue.
I agree, for myself, my heirs, executors and administrators, to not sue and to release, indemnify and hold
harmless the American Heart Association, Inc., its affiliates, officers, directors, volunteers and employees
and all sponsoring businesses and organizations and their agents and employees, from any and all
liability, claims, demands, and causes of action whatsoever, arising out of my child’s participation in this
event and related activities – whether it results from the negligence of any of the above or from any other
cause. Furthermore, I authorize the use, copyright, or publication of my child’s name, image or voice as
may be captured by photograph or recording while participating in this event and related activities in any
medium for any purpose, including illustration, promotion or advertisement.
The foregoing release and indemnification agreement shall be as broad and inclusive as is permitted by the State in
which the event is conducted. If any portion of it is held invalid, the balance shall continue in full force and effect. I
have read, understand and agree to the terms of this Agreement.
I am the legal guardian of the participant, and I hereby consent to his/her participation. I have read and explained the
foregoing release and indemnification agreement to my child, and I hereby agree to its terms on behalf of myself and
the Participant.
