Site HSE Plan preparation guideline

The Site HSE Plan is not intended to replace the existing PETRONAS Technical Standard and HSE procedures, but to complement
them. It also serves as guideline for HSE requirements to contractors involved in PGB GPU activities. This guideline describes the
process and the key elements of developing and implementing Site HSE plan. The purpose of this plan is to:

a) Ensure the requirements and expectations of PGB GPU Policy and regulatory requirements are addressed
b) Identify key activities, role and responsibilities as to meet contract goal
c) Identify potential hazard, risk and its control
d) Enable contractor to estimate time and resources to implement HSE plan

3. The contractor shall develop the HSE Plan based on the contract award duration. The existing HSE Plan is invalid once the
contract end.

4. The HSE Plan’s element and element’s item is subjected to change. The changes may subject to revision of PTS, Statutory
requirement and incident’s lesson learnt and recommendations. The contractor shall adhere and amend the existing HSE
Plan accordingly.

5. The HSE Plan’s element is the minimum requirement that the contractor shall adhere to and the additional requirement
shall be considered based on the risk and hazard assessment conducted prior to the commencement of the contract.

6. The contractor shall make a declaration/statement if the element does not cover in their contract’s scope of work and shall
be discussed with the contract owner.

7. In the element’s confirmation column (YES, NO, N/A). The contractor shall tick (/) one of the box in the checklist.
a. YES: Contractor will comply with the element
b. NO: Contractor will not comply with the element (contractor shall produce specific reason not to comply with the
element and produce mitigation plan)
c. N/A: Element is not covered in contract’s scope of work and the scope will be covered by other contractor or PGB
GPU. (Contractor shall produce a statement based on the agreement with contract’s owner)

8. The HSE Plan shall be prepared by Contractor’s Project Manager and approved by Contractor’s MD/CEO.
9. Copy of approved HSE Plan shall be given to contract owner and HSE Department. The original copy shall be kept by
