communities in In fiscal year 2015 we brought cardiac rehabilitation to south
south Lake County. Lake County for the first time so that those recovering from
cardiac conditions could get stronger closer to home. We also
opened up a 30-bed short-term rehabilitation center to meet
the demand in the area for this service line. Loved ones no
longer have to travel out of south Lake County to visit their
family members who need to be in a rehabilitation facility.
These additions grow South Lake Hospital’s commitment to
providing quality healthcare throughout the full continuum of care for the residents we serve.

As always, we continue to ensure that we are focusing on educating our community on prevention
and obtaining optimal health. This report highlights our current outreach efforts and partnerships
that help to make south Lake County a healthy, vibrant community. It is our privilege to serve the

SOUTH LAKE HOSPITAL I 2
An identified gap in services was local cardiac
rehabilitation. Cardiac rehabilitation is an outpatient
service for patients who have suffered a heart attack
or other heart condition that benefit from cardiac
rehabilitation as part of the recovery process.
Cardiac rehabilitation patients attend sessions
two to three times per week for up to eight weeks.
This rigorous schedule is necessary for optimal
outcomes, and having to travel frequently outside
of one’s hometown proves difficult for the patient and loved ones who have to assist in
transportation.

South Lake Hospital identified this gap
and opened cardiac rehabilitation on the
hospital campus located inside the South
Lake Heart & Vascular Institute, providing
a local option for patients in south Lake
County. This multidisciplinary facility
allows patients to attend cardiac rehab and
have access to schedule appointments with
cardiologists, vascular specialists and an
endocrinologist if necessary.

SPORTS, HEALTH
& WELLNESS

South Lake Hospital provides sports, health and wellness programming to residents of south Lake County through its National Training
Center(NTC)/LiveWell Fitness Center, with an emphasis on prevention. This includes a special focus on youth in the community with the goal
of instilling healthy lifestyle habits at an early age so that they can carry these habits with them into adulthood. The fitness center and sports
staff have created programming and partnerships that provide fun, impactful opportunities for exercise and teaching proper nutrition.
