1. Receptionist Role:

 He / She can Add new patient’s record, edit, and update it.
 Can Search a patient’s record based on patient’s UHID, Name, Surname and Mobile
Number.
 He/ She can collect consultation fee.
 Convert OPD patient to IPD patient in case of patient is getting admitted in hospital.
 He/ She can view a list of all OPD and IPD patients.
 Can enter patient’s discharge record
 Can generate Coronary Report of a patient.
 Can view Coronary report of patients.
 Can generate PTCA Report of a patient.
 Can View PTCA Report of a patient.
 Can generate Hospital Bill for an IPD Patient
 Can view hospital Bill generated.
 Can generate Discharge Summary for an IPD Patient
 Can view Discharge Summary generated.

1.1 Receptionist Dashboard

 Dashboard is a Home Screen for Receptionist login.
 He/ She can navigate to any link just by clicking on the tab on dashboard.
 User can return to the Dashboard by clicking HOME on left menu bar.

1.2 Add New Patient

 For Adding new patient’s record- just click on Add New Patient tab on dashboard. Also
can click on the option given on the left menu bar as Add New Patient.
 User can add any type of patient like IPD, OPD. Old patient or new patient with this
option.
 Select Patient type: OPD/ IPD and Old/ New/ Other
 Select Consultation and any other test if suggested by doctor.
 Fill patients First name, Middle name and Surname
 Select DOB by clicking on the Calendar icon and select the date
 Similarly fill all other details of a patient into provided textboxes
 Click Reset to clear all the field of the form
 Click Submit to insert patient’s record into database.

Remember:
 Always enter date in MM/DD/YYYY format (Ex: 08/15/2016)
 Always check all the field have been field before submitting the form
 Patients Unique Health ID- UHID of 9-digit will be generated automatically by the
application. NHCK/0000/01 (Ex: NHCK00001)
 Patient’s OPD-ID of 11-digit is generated automatically by the application as
NHCO/DD/MM/YY/COUNTER (Ex: NHCO0908161)
 Patient’s age is auto calculated by providing DOB
 Patient’s BMI is auto calculated by providing Height and Weight



Validations:
 Error is shown if date format is not correct.
 Error is shown if patient type is not select, or if Consultation/Diagnostic Fee is not
selected.

1.3 Search Patient

 To search patient, Click on Search tab on dashboard or from the left menu bar.
 User can search any OPD/IPD patient record with this option by typing patient’s UHID /
Name/ Surname/ Mobile number.

1.4 View Patient

 Receptionist can view OPD/IPD patient recorded list from this option.
 To view OPD patient’s list click on View OPD Patients tab on Dashboard or click the
option from left menu bar.
 To view IPD patients list click on View IPD Patients tab on Dashboard or click the option
from left menu bar.
 Search option is provided to search a particular patient from the list below.
 User can search patient’s record by typing in the textbox patient’s
UHID/Name/Surname/Mobile number

1.5 Generate Consultation Bill

 User can generate consultation bill for all OPD patients
 Click on Generate Bill tab on the Dashboard or select the option from left menu bar.
 To generate consultation bill- Type the patient’s UHID and click on Generate Bill button
 To print the Consultation bill - Now click on Confirm to Print button

Remember:
 Type the UHID correctly to generate bill
 Consultation bill can only be generated on the same day the patient record was
inserted.


Validation: Type Only valid UHID.

1.6 Convert OPD to IPD

 Click on Convert OPD to IPD tab on Dashboard or select this option form left menu bar.
 In case of OPD patient needs to be admitted then user can convert such patient to IPD
type
 Enter valid UHID of the patient
 Enter Date of Admission by clicking on the Calendar icon and select the date.
 Similarly fill all the field and submit the record
 Click Reset to clear all the form fields

Remember:


 Always follow the date format correctly as given next to Calendar icon (MM/DD/YYYY)
 Patient’s IPD- ID of 11-digit is generated automatically by the application in the format
NHCIP/MM/YY/1 (Ex: NHCIP08161)

Validation:

 Error is shown if user tries to convert the patient with same UHID and same admission
date more than on time.
 Inserting of Duplicate record are avoided.

1.7 Discharge Patient

 Click on Discharge Patient tab on Dashboard or select this option from left menu bar.
 Enter valid patient UHID
 Enter Date of Discharge patient by clicking on the Calendar icon and select the date
 Discharge time will be added automatically as current system time.
 Similarly fill all the field of the form and Click Submit button
 Click Go Back button to go to Dashboard


Remember:

 Always follow the date format correctly as given next to Calendar icon (MM/DD/YYYY)
 User can change discharge time.

Validation:

 Error is shown if Patient record does not exists in IPD table
 Error is shown if Date format is incorrect

1.8 View Discharged Patients

 Click on View Discharged Patients tab on Dashboard or select this option from left
menu bar.
 User can view records of all the discharged patients

1.9 Generate Coronary Angiography Report

 Click on the Coronary Report tab on the dashboard, or click on Existing Software in left
menu bar and select Coronary Angiography Report
 Enter valid patient’s UHID, name and rest of all field in the form and click Submit button
 To clear the field click on Reset Button

Remember:


 Always follow the date format correctly as given next to Calendar icon (MM/DD/YYYY)
 User can enter ’ N/A’ or ‘--’ if any fields is to be left blank.

Validation:

 Error is shown if Patient record does not exists in IPD table
 Error is shown if Date format is incorrect

 Click on Existing Software and Select View Coronary Angiography Report from left Menu

1.11 Generate PTCA Report

 Click on the PTCA Report tab on the dashboard, or click on Existing Software in left menu
bar and select PTCA Report
 Enter valid patient’s UHID, name and rest of all field in the form and click Submit button
 To clear the field click on Reset Button

Remember:


 Always follow the date format correctly as given next to Calendar icon (MM/DD/YYYY)
 User can enter ’ N/A’ or ‘--’ if any fields is to be left blank.

 Click on Existing Software and Select View PTCA Report from left Menu Bar.
 Click on the UHID of a Patient to view his/her PTCA Report

1.13 Generate Hospital Bill

 Click on the Hospital Report tab on the dashboard, or click on Existing Software in left
menu bar and select Hospital Report
 Enter valid patient’s UHID, name and rest of all fields in the form and click Calculate Total
button to generate Total and Click on Submit button to save.
 To clear the field click on Reset Button

Remember:


 Always follow the date format correctly as given next to Calendar icon (MM/DD/YYYY)
 User can enter ‘0’ (Zero) if any amount field is to be left blank.
 User can enter ’N/A’ if any fields is to be left blank.

Validation:

 Error is shown if Patient record does not exists in IPD table
 Error is shown if Date format is incorrect
 All fields in the form are “required”

bar and select Discharge Summary
 Enter valid patient’s UHID, name and rest of all fields in the form and Click on Submit
button to save.

 Click on Existing Software and Select View Discharge Summary from left Menu Bar.

2 Doctor’s role:

 Doctor can view day-to-day OPD appointments.
 He/ She can mark the patient as “Checked” as patient visit the doctor.
 Doctor can prescribe medicines/ diagnosis.
 Doctor can record patient’s complain as what the patient is suffering from and so on.
 Doctor can add diet plan for the patient
 Schedule next visit for the patient.
 Can view patient’s History also can edit it.
 Convert OPD patient to IPD.
 Can View IPD patient’s list.
 Can search the patient by patient’s name/surname/mobile number or UHID.

2.1 Doctor’s Dashboard

 Dashboard is a Home page
 User can navigate to other web pages by clicking on the tab or selecting the option from
the Left Menu Bar.

2.2 Search Patient Record

 Click on Search Patient tab or select the option form Left Menu bar
 Search OPD/IPD patient by patient’s name/ surname/ mobile number or UHID

2.3 View OPD Appointments

 Click on OPD Appointments tab or select the option from left menu bar
 Here Doctor can search patient’s record
 Can view all the OPD appointments
 There are three grid view list
 First on the top displays the list of patients waiting
 Second on the left hand side, display the patients Queued
 Third list on right hand side, displays the patients Visited the doctor.

Remember:

 Doctor should mark patient with Queued status to Checked
 To change the status Click on Make Checkout

2.4 Edit Patient’s Record

 Q : How to edit patient’s record?
 A : Click on View OPD Appointment tab. Click on the UHID of the patient that you want
to edit from the First Grid View list.
 Doctor can view patient’s personal details, Clinical information also Contact Information


Remember:

 Click on Edit Information button to Edit the patient’s record

 Select the tab to add prescription, diagnosis, complain, clinical information, patient’s
history, Diet Plan, Schedule Next Visit, Add Advice/ Remarks if any.

3 Nurse Role

 Nurse can view OPD/IPD patient’s list
 Can search the patient by patient’s name/surname/mobile number or UHID.
 Convert OPD patient to IPD.
 Can View OPD patient’s personal information, clinical information and contact information
 Nurse can view patient’s prescription,Diagnosis,Complains…etc provided by the Doctor


Remember:


 Nurse can only edit Clinical Vital information of a patient
3.1 Nurse Dashboard

 Dashboard is a Home page
 User can navigate to other web pages by clicking on the tab or selecting the option from the Left
Menu Bar.

3.3 View Patient’s Information

 Q : How to View patient’s record?
 A : Click on the patient’s UHID to view its Record.
 Click on View Prescription tab to view patient’s prescription and other clinical details.
 Nurse can view patient’s personal details, Clinical information also Contact Information

3.4. Edit Patient’s Vital Information

 Nurse can only edit patient’s Clinical Vital Information
 Click on Add Vital Parameters button to add new Clinical information like patient’s height,
weight, BP, pulse, temperature, etc
 Enter patient’s weight, temperature, pulse, blood pressure, sugar level.
 Click Save button to save the information entered
 Click Reset button to clear all the fields

3.5. Convert Patient from OPD to IPD

 Click on Convert OPD to IPD tab or select the option form left menu bar
 Enter valid patient’s UHID and rest of all fields in the form and Click on Submit button to save.

3.6 View IPD Patient list

 Click on IPD Patients tab or select the option form the left menu bar to view IPD Patient’s List

4. Pharmacist Role:

 Pharmacist can view OPD/IPD patient’s list
 Can search the patient by patient’s name/surname/mobile number or UHID.
 Can View OPD patient’s Personal Details like: name, UHID, Address
 Pharmacist can view patient’s Prescription, Diagnosis provided by the Doctor
 Generate Bill for the Medicines sold by him.

4.1 Pharmacy Dashboard

 Dashboard is a Home page
 User can navigate to other web pages by clicking on the tab or selecting the option from the Left
Menu Bar.

4.2 Search Patient
 Click on Search Patient tab or select the option form Left Menu bar
 Search OPD/IPD patient by patient’s name/ surname/ mobile number or UHID

4.3 View Patient Diagnosis & Prescription

 Click on Search Tab on the dashboard or select the option from left menu bar
 Click on the patient’s UHID to view his/her Diagnosis and Prescription
 Click on View Diagnosis button to view patient’s diagnosis.
 Click on View Prescription button to view patient’s Prescription

4.4 Sale Medicines

 Click on Sale button to sale the medicines prescribed shown in fig 4.4
 Enter the medicine name, Quantity, Rate and expiry date
 Click on Add button to add more medicines
 Click on Total to calculate Total
 Click on Save button to save the sale receipt.
 Click on Cancel to clear all the fields
 Click Delete to delete a row if not required


Remember:

 Always enter correct Date format MM/DD/YYYY (Ex: 07/22/2016)
 Enter only numbers under Quantity column

4.5. Generate Sale Bill

 Click on Generate Bill tab or select the option from the left menu bar.
 Enter the valid patient’s UHID

Remember:

 Always follow the date format correctly as given next to Calendar icon (MM/DD/YYYY)
 Sale Bill can only be generated on the same date medicines sold.


Validation:


 Error is shown if Patient record does not exists in OPD table
 Error is shown if Date format is incorrect

5. Pathologist Role

 Pathologist can view OPD/IPD patient’s list
 Can search the patient by patient’s name/surname/mobile number or UHID.
 Can View OPD patient’s Personal Details like: name, UHID, Address

 User can navigate to other web pages by clicking on the tab or selecting the option from the Left
