Secondly, we want to thanked our family and friends for giving us support and
encouragement throughout the process of completing this assignment. Without all of this we

1.0 CHAPTER 1 : INTRODUCTION
1.1 HISTORY
Blood Bank Services in Malaysia has been initiated by a group of women
volunteers of the Red Cross British in 1955. Initially the service is only open on
Wednesdays from 5.00 pm to 5.30 pm at the Kuala Lumpur General Hospital. On this
initiative, a total of 25 to 44 donors were collected every month. Donors at that time
consisted of members of the police, military and government personnel. After 15 years
of the start of the service, as much as 5,830 units of blood and no less than 13,860
successful compatibility test. Due to the increasing service, Blood on the move to
another building at Hospital Kuala Lumpur in 1971. The building is known as "National
Blood Service Centre" and was inaugurated in April 1972 by the Director General of
Health, Ministry of Health. The National Blood Service Centre placed under the Ministry
of Health hospital is headed by a Director. He was responsible for all the blood service
that includes technical and management issues. Starting in 1975, the Blood Services
Centre has played an important role in the standardization of work procedures,
methodology, reagent and blood bank equipment in all new buildings negara. In 2002
National Blood Centre was built in Jalan Tun Razak and it was inaugurated by the
Director Datuk Sri Hasmah Mohd Ali. The building is stately and magnificent to date in
ensuring the collection of blood and ensure a safe, clean and quality. National Blood
Centre (NBC) is an organization responsible for planning and developing blood
transfusion services at the regional and national levels.
1.2 LOCATION AND ADDRESS
National Blood Centre were located at Jalan Tun Razak, 50400 Titiwangsa, Kuala
Lumpur, Wilayah Persekutuan Kuala Lumpur. The location and environment of the
blood centre is strategic based on our observation. From here we found that National
Blood Centre is located at city area that full of facilities in its surrounding. The National
Blood Centre was build near the main road and highway, other than that the building
also surround with neighbourhood community and other health facilities/centre around
it. Throughout our observation, we found that this building located at a crowded, noisy
placement and near water source such as river which can be a negative aspect in
planning to build an archive near the main building.

2.0 CHAPTER 2: THE NATIONAL BLOOD CENTRE ARCHIVE
2.1 INTRODUCTION TO YOUR ARCHIVE
As a component of foundation, National Archive Blood Centre intended to be the best
public blood place in the nation of Malaysia and to be a model for other accommodation
chronicles in the country. Mindfulness in record keeping, profound interest in the
verifiable fields, and with a straightforward favourable luck has brought about the
suggestion of setting up Malaysia National Archives Blood Centre that situated in the
Selangor, locale region.
National Archives Blood Centre will safeguard the patient clinical record that identified
with National Blood Centre accommodation. Reports identified with the clinical or
patients record that have been keeping 49 years of age and out of organizations
utilization which are start with National Blood Centre set up will be brought to recently
settled structure. In excess of 746,485 patient and givers records will be saved in the
National Blood Centre Archives and the number will build each year with about over
100%. Those records informed by Dato Seri Dr Siti Hasmah bt Hj Mohd Ali in
conjunction with the initiation of National Blood Centre on 2003.National Archives
Blood Centre building will be situated in Bandar Teknologi Kajang, Selangor and will
be responsible for patients or donor records of importance to the Malaysia medical and
hospitality records. National Blood Centre Archives will also preserve materials from
privates medical institute such as corporations and organizations that linked to the
National Blood Centre.
The tasks of the National Archives Blood Centre are :
a) Assisting the preservation of materials in the archives.
b) To support the production, storage and issue of blood and blood products.

The important aspect of the National Blood Centre Archives is keeping all the medical
of records whether it is patients or donors and it will be made available to all. The
official National Blood Centre Archives will represent over the years the activities of
the National Blood Centre. These will include records such as blood donation
management, blood procurement, blood transfusion management, donor records,
patient sample and recipient management. This variety of information means scholar
can make use of it in other fields of medical studies. This archives will also have a
variety of internships, student assistants and volunteer opportunities. Whether you are
creator of records, researcher or a community member who needs assistance with the
materials that document your community, our wide range of programs and resources
are intended to serve National Blood Centre future as medical or hospitality that serve
community in blood donors of Malaysia.
2.2.1 VISION
a) The vision of the National Archive Blood Centre is to serve the National Blood
Centre community towards self -management in providing a system of records and
also help to become an excellent reference centre in the field of Medical
Transfusion.
2.2.2 MISSION
a) The mission of the National Blood Archives Centre is to strive to provide important
archives and records with quality services through a spirit of teamwork, prudence,
professionalism supported by respect, knowledge and innovation.
2.2.3 OBJECTIVES
a) Collect, store and preserve all record materials at the National Blood Centre.
b) Assist the National Blood Centre to establish a systematic, effective and
economical system of records.
c) Assist National Blood Centre to become an excellent reference centre in the
field of Medical Transfusion.
d) To boost the National Blood Centre image by assist with the good records
systems.

The blue-colored building behind the National Blood Centre portrays show the symbol
of a building our archive that becoming an archives of National Blood Centre in
Malaysia and are intended to serve patients and community especially Malaysian
community. For the text symbol that written is describe the name of archive and we
also take the logo of National Blood Centre to represent our archives logo that only
keeping National Blood Centre records. For the yellow color is represent or describe
the records will always been protected in our archives.
2.3 LOCATION
2.3.1 FULL ADDRESS
The location that we choose is new site for planning to build an archive which located at :
Jln HP 3, Bandar Teknologi Kajang, 43500, Selangor

2.3.2 SHOULD BE AND SHOULD NOT BE
The site for an archival facility should be :
a) Located within a short response time of emergency services.
b) It is capable of allowing swift evacuation of people in the event of an emergency.
c) It should be located on a stand-alone or island site with free access to the entire
building perimeter and the new building should be completely protected from dangers
posed by neighbouring spaces and buildings.
d) The site should be accessible to potential users to cultural and educational institutions.
e) The site also must be accessible by main roads and by public transportation to facilitate
movement.
f) It must be easily accessible from the parent organization which is the record creator.
The site for an archival facility supposedly should not be:
a) Liable to subsidence or flooding such as river, underground water, dam or ocean.
b) Must not be at a risk from earthquakes, tsunamis, landslides, fire explosion, polluted
area and contaminated land.
c) It should not be near to strategic installation or symbolic site, source of
electromagnetic and place or building that attracts rodents or insects.
2.3.3 THE EXPLANATION OF SHOULD AND SHOULD NOT BE
Jalan HP 3, Bandar Teknologi Kajang will be our choice of the new site for the National
Blood Centre after carefully taking some measurements in terms of should be and
should not be.
As for the should not be, the National Blood Centre new visit is far from earthquakes,
tsunamis or landslide because it is clearly that the reason we choose new site is we
are not limited to any spaces or surrounding over there. Secondly, the should not be
in for locations is obviously that our new site far from a strategic installation or symbolic
site that could be a target in an armed conflict and polluted area. As for our
establishment for medical archive, our new site also not near with industrial or
agricultural facility or other installations that emitting to harmful gases, smoke, dust
and most importantly our new site also did not near to the risk of fire, explosions or
impacts form or related to nearby sites or in the same building.

As for should be, the surrounding of the new site itself is well-clear from any kind of
environmental issues. There are easily accessible from the parent organization known
as records creator. Due to the wide spaces of new site it also accessible by main roads
and by public transportation as well. The size of the new site land is pretty
convenience. National Blood Centre Archive will be no major construction problem that
will cause many negative effects as for our new site land. Our archives locations also
did not attract to any kind of rodents or insects.

2.4 THE BUILDING
The selection of the archive building is essential because the archive is a place of
inactive record storage that has value. We have found a vacant site in the Bandar
Teknologi Kajang area and have decided to build an archive. We made the archive
there due to several factors; we were able to design the archive itself. We were able
to determine the building criteria needed according to the importance of the National
Blood Centre (NBC). We can also ensure that our buildings have adequate security
features to keep records from being damaged. In addition, we can also design the
exterior and interior of the archive building with a modern concept. We can use modern
techniques and materials to construct the archive without making changes, such as
existing buildings. The construction of the new facility also makes it easier for us to
apply all the requirements that have been planned. Therefore, the new archive building
could give an advantage to the National Blood Centre.
2.5 THE SERVICES AND OPENING HOURS
2.5.1 SERVICES IN ARCHIVE
a) Online Service (NBC Finding Aid) – It is services to facilitate users in finding the specific
material in the archives online.
b) Research Hall – A large space equipped with tables and chairs can accommodate 20
to 60 people, so the researchers can use this space to view the requested material.
c) Library – The visitors and users can use the library in the archive while waiting the
archivist for their material to retrieve from the storage room.
d) Registration Counter – Archive visitors can make a walk-in registration at the
registration counter as a researcher or archive user to obtain permission to access the
material.
e) Special Reading Room – Space that has facilities to access materials such as
microfilm, video form, tape-recorded, etc.

2.7 THE RULES AND REGULATION
Rules and Regulation for visitors and researchers:
a) First-time archive users are required to register as a researcher before accessing the
material in the archive.
b) The researchers must always carry an Identification Card (I/C) or Passport and tag
researchers while in the archive area.
c) Archive users are prohibiting from entering restricted areas without permission.
d) Every user and archive researcher are prohibited from smoking and bringing food and
beverages into the archive area, research hall, library, special reading room and leisure
area.
e) Researchers are prohibited from carrying sharp tools such as pens, scissors or any
tools that can physically damage the archive material.
f) The use of mobile phones and cameras is not allowed in the research hall and special
reading area.
g) Researchers shall be responsible when handling documents or material issued to them
by not folding any pages in the paper.
h) Researchers must ensure that each document used is arranged in the same order
when receiving the record.
i) The visitors only allow to enter the leisure area and library.

Rules and Regulation for staff archival:
a) The archival staff must ensure the archival storage areas are secured all the time.
b) The staff do not allow to smoke, eat and drink in the storage area to avoid fire and
rodents in the archive storage.
c) The archival staff must follow all procedures in handling all archive materials.
d) Always make sure each storage room is locked after entering it.
e) Ensure that no unauthorized people enter the archive storage area.

This is National Blood Centre Archive full floor plan. National Blood Centre Archive is a small
archive. There are four rooms or areas in the archive which are public and staff room,
processing room, storage room for paper-based materials and storage room for magnetic
tape. The bottom left of the archive is public area. The public area consists of investigation
room, finding aids, research hall, library, and washroom. There are special reading room and
temporary storage room in research hall. There is a library office in the library. Above the
public area, there are staff area. The staff area consists of registry, technician room, server
room, store, meeting room, head of department room, deputy room, clerk station, leisure area
and pantry. In pantry, there are washroom and prayer room. At the centre of this building is a
storage area. Storage area consists of paper-based record 1, paper-based record 2, magnetic
record 1 and magnetic record 2. In paper record 1, there is a STRONG ROOM. At the top left
and bottom right of this building there are future expansion 1 and future expansion 2. At the
top right of this building there is a processing area. The processing area consists of loading
bay, receiving room, triage room, dirty room, work area, humidifying room, store, reprographic
room, preservation and conservation lab, preservator room, archivist room, work station,
leisure area, and pantry. In pantry, there are washroom and prayer room. Then there is a
corridor that link for each area in the archive. The corridor width is sufficient for two carts to
pass each other.

The National Blood Centre Archive have one specific area for public area. This is an open
area that can be access and used by both public and staff of National Blodd Centre Archive.
Public area consists of investigation room, finding aids, research hall, library, and washroom.
There are special reading room and temporary storage room in research hall. There is a library
office in the library. This area must be conducive and have comfortable space to be used by
the public. For security purpose, there is also security cameras (CCTV) in staff and public
area.

The National Blood Centre Archive have one specific area for staff area. This is an open area
that can be access and used by both public and staff of National Blodd Centre Archive. The

washroom and prayer room. The staff area needs to be flexible, comfortable, and conducive
for staff to work in a good condition. The relationship in staff area consist of smart access and
open access. A room such as registry, server, deputy, head of department room, emergency
exit and staff entrance was a smart access which can be access by authorized person only.

The processing area of National Blood Centre of Archive consists of loading bay, receiving
room, triage room, dirty room, work area, humidifying room, store, reprographic room,
preservation, and conservation lab, preservator room, archivist room, workstation, leisure
area, and pantry. In pantry, there are washroom and prayer room. The relationship in
processing area consist of smart. A room such as receiving room, triage room, dirty room,
work area, humidifying room, reprographic room, preservation, and conservation lab,
preservatory room, archivist room and emergency exit and processing entrance were a smart
access which can be access by authorized person only. These area is being used by the
archivist and conservator to do the preservation and conservation, packing and labelling,
cataloguing, inventories and many more. Table and tools are provided in this area.
Sink use by the researcher to clear the contaminated at the materials also floor drains in the
processing area. Received or transferred materials must be cleaned before placed in the
storage area. The shelves use by the staff to put all the incoming archival materials for
temporary. All the materials receive should be register and cleaned before put in the storage
area. When received the materials, archivist are responsible to checked it properly and make
sure the item received is correct.

The National Blood Centre of Archive have 70% space for storage area. At the centre of this
building is a storage area. Storage area consists of paper-based record 1, paper-based record
2, magnetic record 1 and magnetic record 2. In paper record 1, there is a STRONG ROOM.
At the top left and bottom right of this building there are future expansion 1 and future
expansion 2. The relationship in storage area is a smart access paper-based record 1, paper-
based record 2, magnetic record 1 and magnetic record 2, STRONG ROOM and emergency
exit and storage entrance were a smart access which can be access by authorized person
only.
The function of a storage area are to store all the archival materials National Blood Centre
Archive, to protect archival materials in National Blood Centre of Archive from fire, water, and
threat and to preserve archival materials in National Blood Centre Archive which are paper-
based and magnetic tape materials. Storage area in archive is a facility that provide a proper
environment for the purpose of storing records and the materials that required permanent
protection for the lifetime storage, upkeep, and preservation, storage area of National Blood
Centre Archive is divided into 4 area such as paper-based record 1, paper-based record 2,
magnetic record 1 and magnetic record 2.

3.8 CONCLUSION
In conclusion, National Blood Centre Archive is an archival institution that act to protect
the heritage of the National Blood Centre records. National Blood Centre Archive is to strive
to provide important archives and records with quality services through a spirit of teamwork,
prudence, professionalism supported by respect, knowledge and innovation. Archival
institution for health organization is important role throughout history as an important legal
document for the exercise of individual’s rights. As for National Blood Centre Archive important
for the creator of records, researcher, scholar and even community who needs an information
regarding blood transfusion, donor records and etc. Throughout the studying in planning and
creating an archive for an institution, we gain a lot of information and experience in creating a
design for Homestyler and Sketchup floor plan based on the criteria given in order to create a
stable and good archive for health organization.
