Separation can be painful for everyone involved – especially children. During this challenging time children need support, love
and contact with both of their parents, and other significant people, such as grandparents. Some certainty for the future is also
important for everyone.

What is a parenting plan?

A parenting plan is a voluntary
agreement that covers the day to day
responsibilities of each parent, the
practical considerations of a child’s daily
life, as well as how parents will agree and
consult on important long-term issues
about their children. It can be changed at
any time as long as both parents agree.
A parenting plan is not legally
enforceable and is different from a
parenting order, which is made by a
court. Parents who make a parenting
plan can ask the court to make an
order in the terms of that plan. Once
made, these orders are legally binding –
they have the same effect as any other
parenting order made by a court.
You can use this Parenting Plan to:

for yourself and your child(ren)

being prepared when you

into the Family Court system by

Natural Parents agree that Communication will never be inhibited between our children and Our Biological (Blood\Family) Relatives
Natural Parents agree that our Children will not be subjected to any Physical, Emotional or otherwise Excessive Form of Punishment.
Natural Parents forthwith will advise and discuss any form of Physical Sickness, Injury, Emergency, and Treatment of our Children.
Natural Parents consent to Relaxed Communication between All Biological Family by way of Mobile Phone and Electronic Services.
Natural Parents agree that All Mobile Phone Numbers for our Children will remain disclosed between All Biological Family at all times.

Natural Parents consent and agree to attend and liaise directly with the Childrens School, Sporting and Extra-Curricular Activities.
Mutual Written Consent is required where Parental Consent is required, or during any Extended Absence away from either Parent.
Natural Parents agree to disclose our Childrens Whereabouts upon request including, School, Sport, Social, Family and Otherwise.
I agree that the children will live with their Natural Mother on the condition that the Residential Address remain disclosed at all times.
Definitions
* Forthwith: Contact and Advisory Communication will be made between All Biological Family in a Prompt and Immediate Fashion.
* Disclose: Private Details will be Presented and Provided to All Biological Family Members, Forthwith.
* Electronic Services: Alternative Methods of Communication for Biological Family include Email, Skype, SMS and Facebook Messenger.
* Childrens Whereabouts: The Address of any such venue including those of Social and Family dwellings will be disclosed upon request.
* Extended Absence: An Extended Absence is defined as any duration longer than 48 hours outside of the care of either Natural Parent.

Both parents will receive copies of the following: Medical, Emergency, Dental and Health Records.
Recreational Enrollment Activity Records.
School Reports, Awards and Incident Reports.
Birth Certificates, Birth Records & Name Changes
Psychiatric, psychological, or other mental health records.

Changing the child's name(s):
Neither parent will change the last name of the child(ren) or have a different name on the child(ren)'s medical, school or other records
without the written consent of the other parent.

Restrictions

Child Care

Child care plan:
The children must not be left alone without age-appropriate supervision.
Each parent may select appropriate child care providers.
The parents must let each other know the name, address, and phone number of the children's regular child-care provider(s).
All child care providers must be agreed upon by both parents.

Right of first option of child care

In the event either parent requires child care for hours or more while the children are in his or her custody, the other parent
must be given first opportunity, with as much prior notice as possible, to care for the children before other arrangements are made.

Supervised Visitation

Definitions
For purpose of this order, supervised parenting time means that the non-custodial parent and the child(ren) shall be in the continued
presence of the designated supervisor.
It is the responsibility of the person designated to supervise, either by the parties' agreement or by order of the Court, to be present at
all times with the child(ren) and to exercise his/her reasonable judgement as to the advisability of the continuation of each parent
period, giving due consideration to the best interest of the child during the exercise of parenting time.

It is agreed that the supervised visitation provider is present to monitor only and not interfere with the parent's time during visitation
unless the child(ren) is under threat of being harmed or any other reasonable safety concern. This includes disallowing a parent to

Revisions

Failure To Reach Agreement
Should the parents be unable to reach a mutually agreeable decision regarding an issue relating to the child(ren)'s care or general
welfare, the parents shall seek professional assistance from a person licensed to provide family counseling.
Should the parents fail to reach an agreement through counseling, or should one of the parties refuse to engage in counseling,
either parent may petition the Court for further mediation/investigation.

Previous Court Order
All previous Court orders, not in conflict with this report, shall remain in full force and effect.

Petition For Mediation
The parties, after filing a Petition For Mediation, may return for further mediation/investigation if the above recommendations (i.e.
counseling, drug testing, parenting classes etc.) have been completed by the following:

Terms and Conditions of Order May Be Changed
The terms and conditions of this parenting plan may be supplemented or revised as the needs of the children and parents change.
Such changes will be in writing, dated and signed by both parents; each parent will retain a copy. If the parents want a change to be a
court order, it must be filed with the court in the form of a court document.
In the event controversy arises, the existing order of the Court shall remain in effect and the parents may seek mediation prior to
filing a motion with the Court for another hearing by filing a Petition for Mediation.
