1. DEFINITION OF TERMS

UNIVERSITY means Universiti Islam Antarabangsa Sultan Abdul
Halim Mu’adzam Shah (UniSHAMS).

KULLIYYAH means Kulliyyah of Medicine and Health Sciences.

DEAN means the administrative head of the Kulliyyah.

COURSE means a defined area or aspect of a subject, an organ
system, or a unit of the curriculum that comprises a number of inter-
related topics and is normally offered during a specified period of
time.

POSTING means a specific component of intensive clinical teaching-
learning experience the nature, objectives and duration of which are
precisely defined.

CONTACT HOURS means the duration of the actual face-to-face
instructional interaction between the academic staff and the students
measured in terms of hours.

NON-CONTACT HOURS means the duration of the non face-to-face
instructional interaction between the academic staff and the students
measured in terms of hours

CREDIT HOURS means the academic load of a Course or Posting
measured in units per semester.

PHASE 1 refers to the first two years of the Medical programme
when the principle teaching-learning emphasis is on the biomedical
sciences.

PHASE 2 refers to the Years 3 and year 4 of the Medical programme
when the principle teaching-learning emphasis is on the acquisition of
clinical knowledge, understanding and skills.

PHASE 3 refers to the year 5 of the Medical programme when the
principle teaching-learning emphasis is on the acquisition of clinical
knowledge, understanding and skills.

EXAMINATION refers to any test, assignment, project, or quiz given
by a lecturer in respect of the specified assessment requirement of a
Subject, Course or Posting.

CONTINUOUS ASSESSMENT refers to the process of periodically
assessing and evaluating student performance and learning
achievement throughout a Course or Posting. For the Phase 1
curriculum, it may include Mid Semester Test, projects, assignments,
practical work, PBL session and class participation, etc. For phase 2, it
includes case presentation, case write ups and supervisor’s report.

END OF SEMESTER EXAMINATION refers to an examination
conducted at the end of a Semester.

END OF POSTING EXAMINATION refers to an examination
conducted at the end of a Posting.

REMEDIAL EXAMINATION refers to an examination conducted for
students who fail End of Posting Examinations. Remedial classes are
conducted prior to students undertaking this examination.

RE-SIT EXAMINATION refers to an examination conducted for
students who fail End of semester examinations without sitting for
classes.

PROFESSIONAL EXAMINATION refers to a specific type of
examination conducted at the end of Year 2 and at the end of Year 5
of the Medical programme.

SUPPLEMENTARY EXAMINATION refers to an examination
conducted for students who fail Professional Examinations. Remedial
classes are conducted prior to students undertaking this examination.
The examination format is similar to the respective Professional
Examination.

EXTERNAL EXAMINER refers to a person, not being a member of
the university’s academic staff and not being involved in any teaching
of the particular group of students, who is appointed as an Examiner
by the University Senate upon recommendation by the Faculty.

ONE BEST ANSWER (OBA) is a special type of multiple choice
question in which only ONE answer required among all the five
options given. All the options may be right or all of them may be
wrong. The students have to give the best answer. The time
recommended for each One Best Answer type of question (OBA) is
1.5 minutes.

EXTENDED MATCHING ITEMS (EMIs) consist of lettered options
followed by a list of numbered problems/questions. For each
numbered problem/question the one lettered option should be
selected that most closely answers the question. The lettered options

can be used once, more than once, or not at all. The time
recommended for each Extended Matching Item (EMI) is 1.5 minutes.

MODIFIED ESSAY QUESTIONS (MEQs) refer to short answer
questions comprising of a series of open-ended sub-questions arising
from specific triggers. Each question is of 30 minutes duration and is
allocated 20 marks.

OBJECTIVE STRUCTURED PRACTICAL EXAMINATION (OSPE)
refers to an examination comprising a series of open-ended question
based on a specific trigger provided by an exhibit, model, slide or
photograph. The questions usually test more practically oriented skills
and knowledge. The time allocated for each station is 4 minutes and
each station carries 5 marks.

OBJECTIVE STRUCTURED CLINICAL EXAMINATION (OSCE) is
an examination similar to the OSPE but the content usually focuses on
clinical skills and associated knowledge. The time allocated for each
station is 4 minutes and 5 marks are allocated for each station.

SHORT ANSWER QUESTION (SAQ) refers to questions which
usually require short answers of about half a page in length. It
usually involves simple explanation to demonstrate understanding of
some information, procedure or process. Each question is of 10
minutes duration and is allocated 10 marks.

STRUCTURED ESSAY QUESTION (Essay) refers to an essay type
of Question which usually is about one to one and a half pages in
length. It is generally focused on a specific issue where ‘pros’ and
‘cons’ may need to be considered. Each question of 30 minutes
duration and is allocated 20 marks.

VIVA VOCE EXAMINATION refers to a clinically oriented,
structured oral examination. It is also includes demonstration of
understanding or examination of specific skills. It is conducted for
students attending Professional Examination who attain potential
distinction marks (> 75%) or borderline fail who obtain final marks of
> 45% < 49.5%. Time allocated for short viva voce is 20 – 30
minutes.

2. DURATION OF THE MEDICAL PROGRAMME

The duration of the Medical programme is five (5) to seven (7) years.
A student may repeat any year but is not allowed to repeat any particular
year twice.

The minimum residential requirement for students transferred from other
universities is three (3) years.

3. OBJECTIVES OF THE MEDICAL PROGRAMME

PEO 1: A competent practitioner who are knowledgeable, technically competent
PEO 2: in the practice of medicine and socially responsible and also include
PEO 3: Islamic ethical dimensions.
PEO 4: A competent practitioner who are to communicate well and work in a
team in the provision of primary, secondary and tertiary health care
services.
A competent practitioner who are able to function effectively as a
member of the nation’s health care delivery system.
A competent practitioner who practices holistic, ethical, professional and
Islamic values in providing service to the recipients and providers of the
healthcare industry.

4. TYPES OF ASSESSMENT

There are two main types of assessment, formative and summative.

4.1. Formative Assessment

Students are monitored on a continuous basis throughout the
programme and are given regular feedback on their performance and
progress. In each Course, Posting and Phase of the five-year
programme, satisfactory performance in the formative assessment
components is a prerequisite for eligibility to take the summative
assessment.

In addition to monitoring progress in the acquisition of knowledge and
psychomotor skills, the formative component of assessment includes
observation of students’ attitudes, personal qualities, behaviour and
involvement in co-curricular activities.

4.2. Summative Assessment

There are two Professional Examinations, one each at the end of the
year 2 and the year 5 of the medical programme. However, in order
to qualify to take the Professional Examinations, a student is required
to pass a series of summative assessments (i.e., End of Semester and
End of Posting Assessments).

Within the general context of summative assessment, provision is
made for periodic assessments within each Course in each semester
or Posting. The results of these assessments contribute to the Final
Grades achieved by students in each Semester or Posting.

5. METHODS OF ASSESSMENT

5.1. Phase 1 Assessment

5.1.1. The End of Semester Assessments and the First Professional
Examination each consist of two parts. The first part is the
Theory Examination; the second part is the Practical
Examination.

5.1.2. End of Semester Assessments

a) In Phase I, (Year 1 and Year 2), students will be assessed at the
end of each semester (semesters 1, 2, 3, and 4).

b) The assessment consists of:

Continuous Assessment 30%

End of Semester Examination 70%

c) The continuous assessment consists of mid semester tests,

practical reports, assignments, seminars, group

discussions,research presentation and problem-based learning

assessment for the respective courses. Other forms of

assessments carried out during each course may be included into

the continuous assessment for the course but the total weightage

will not exceed 30% of the total mark for that particular course.

5.1.3 Fail in the Semester Examination

a) The students who fail any course at end of the semester 1 and
semester 2 will be required to re-sit examination during the
semester break.

b) If the student fails not more than 4 core medical courses at the
end of Year 1, he will be allowed to re-sit the examinations.

c) If the student fails 5 or more core medical courses at the end of
Year 1, the student will have to repeat Year 1. Students are
allowed to repeat the year only ONCE.

d) The re-sit examinations will be conducted at the end of Year 1.

e) The maximum mark a student may achieve in the re-sit
examinations is 50% for each course.

f) Students who obtained a redeemable fail mark (maximum of 2
core medical courses) may be upgraded to the minimum passing
mark of 50% by the Board of Examiners provided
i. the overall performance (attendance, participation in the
formal and informal academic activities and
professionalism) is satisfactory and
ii. the overall CGPA is at least 2.33.

5.1.4 The First Professional Examination

a) The First Professional Examination is conducted at the end of Year
2.

b) The First Professional Examination consists of the following
components:

i. Continuous Assessments - 30%

c) The Continuous Assessment marks is the average marks of all core
medical courses in end of semester 3 and 4 examinations which
makes up 25% and the remaining 5% is derived from seminar
evaluation.

d) The First Professional Examination consists of:

e) The passing mark of the First Professional Examination is 50%, in
order to progress to Year 3.

In cases where the mark obtained is redeemable fail (45 – 49%), a
viva voce will be conducted to determine the final status of the
students.

g) Students who obtained redeemable fail mark and satisfactorily
pass the viva voce, their marks will be upgraded to 50%.

h) Distinction is automatically awarded to those who obtained 80%
and above, without having to sit for viva voce examination.

i) Students who pass the First Professional Examination will progress
to Phase II (Year 3) of the medical programme.

j) Students who fail the First Professional Examination will be given a
supplementary examination within one month. Candidates who fail
the supplementary examination will have to repeat Year 2. The
format of the supplementary First Professional Examination is
exactly the same as the First Professional Examination.

k) If the student fails the First Professional Examination in the repeat
year, the students will have to leave the Medical Programme.

l) The Board of Examiners for the First Professional Examination
consists of internal and external examiners. The external
examiners must be familiar with the integrated Medical curriculum
of UniSHAMS, but must not be involved, currently, in the teaching
of UniSHAMS medical students.

5.2. Phase 2 Assessment (Year 3 and year 4)

5.2.1. Year 3

Phase 2 Year 3 of the programme consists of 1 week of Introduction to
Clinical Medicine (Elementary Clinic) followed by four 11-week rotation
postings in Internal Medicine I, General Surgery, General Paediatrics and
Women’s Health and Disease I. At the end of each posting, an End of Posting
Examination will be conducted. There are FOUR (4) End of Posting
Examinations in Year 3.

5.2.2. End of Posting Examination

The assessment consists of Part I and Part II:
Part 1:

(i) Continuous assessment 40%
(ii) Theory paper examination 60%

5.2.3. CRITERIA FOR PROCEEDING TO YEAR 4

a) A promotional exercise will be conducted at the end of Year 3 based on the
results of the End of Posting Examinations of the 4 rotational postings in Year
3.

b) Students who pass all the End of Posting Examinations in Year 3 will proceed
to Year 4.

c) Students who failed End of Posting Examination will have to sit for the resit
exam 1 week after the exam. Students who failed the resit examination will
have to sit for remedial examination at the end of the year 3. They are
required to pass the remedial examination in order to proceed to Year 4.

d) Students who failed one or more of the end of posting remedial examinations
are required to repeat the year.

e) In special circumstances in the repeating year end of posting examination
marks that have been obtained in the previous year can be used in
promotional exercise.

5.2.4. YEAR 4

Phase 2 Year 4 consist of 5 major postings i.e. Orthopaedic, Trauma &
Rehabilitation, Psychiatry, Public Health, Family Medicine and Elective Posting. In
addition, there will be two weeks each in Radiology, Opthalmology, ENT,
Anaesthesia, Dermatology and Forensic Medicine. There are FOUR (4) End of Posting
Examinations in Year 4.

5.2.4.1. The assessment consists of Part I and Part II:

PART I: 40%
(i) Continuous assessment 60%
(ii) Theory paper examination 100%

(i) and (ii) form the total theory mark

5.2.4.5. Elective Assessments

a) Each student’s Elective proposal must be submitted to the Faculty for
approval. A local supervisor is appointed to guide the students prior to
the Posting and to assist them to get a suitable placement. All students
must complete the Elective Posting before proceeding to Year 5, except
those students doing Remedial Postings.

b) Assessment of the Elective Posting has two components:

i) The “on-location” supervisor’s evaluation of the student during the
Elective Posting.
This takes into consideration:

- attendance
- enthusiasm
- clinical skills
- project work.

ii) The student’s Elective Report.

This Report, which is of not more than ten (10) pages in length,
follows a standard format provided by the Kulliyyah.

The local supervisor assesses both Reports and, using a standard
rating scale, determines whether the student’s performance during
the Elective Posting been satisfactory or unsatisfactory.

5.2.5. CRITERIA FOR PROCEEDING TO PHASE 3, YEAR 5

a) A promotional exercise will be conducted at the end of Year 4 based on
the results of the End of Posting Examinations

b) Students who pass all End of Posting Examinations in Year 4 will proceed
to Phase 3, Year 5.

c) Students who fail in two and less major end posting Examination in Year
4 are required to repeat the posting during the elective posting period as
a remedial posting and pass the reassessment. Students who fail the
remedial posting examination will have to repeat Year 4.

d) Students who fail in more than two major End of Posting Examinations
in Year 4 will have to repeat the year.

e) Students who fail more than three minor postings examinations must
repeat year 4.

f) Year 4 students who failed minor postings examinations must re-sit and
pass the examination as a prerequisite to sit for Final Professional
Examination.

5.2.6. YEAR 5

In Year 5, there are five rotation postings namely Internal Medicine II, Surgery &
Trauma, Paediatrics & Neonates, Women’s Health & Disease II and Psychiatry &
Palliative Care. At the end of each posting an End of Posting Examination will be
conducted. There are FIVE (5) End of Posting Examinations in Year 5.

5.2.6.1 END OF POSTING EXAMINATION OF PHASE 3 (YEAR 5)

In Phase 3 Year 5, students will be assessed at the end of each posting.
The assessment consists of Part I and Part II:

PART 1: Continuous assessment 40%
(i) Theory paper examination 60%
(i) and (ii) form the total theory mark 100%
(ii)

PART 2: 100%
Clinical examination

5.2.6.2. The components of continuous assessment include:

(i) Professionalism
(ii) Punctuality
(iii) Case presentation
(iv) Seminar presentation
(v) Case write-up
(vi) Log-book

5.2.6.3. Formal End of Posting theory paper examination consist of:

(i) One Best Answer
(ii) Patient Management Problem
(iii) OSCE

5.2.6.4. The clinical examination includes:

(i) One long case
(ii) Two short cases

5.2.6.6. Eligibility to sit for Final Professional Examination

The following criteria must be fulfilled in order to sit for the Final
Professional Examination.

a) Year 5 students who failed end of major postings examinations
must sit for resit. Students who failed the resits must sit for
remedial and pass the examinations as a prerequisite to sit for
Final Professional Examination.

b) Year 5 students who failed more than 2 major postings must repeat
year 5.

c) Year 5 students who failed remedial examination must repeat year
5.

5.2.6.7. Final Professional Examination

5.2.6.7.1. Purpose of the Final Professional Examination

The purpose of the final examination is to certify that the students have achieved
the objectives of the Kulliyyah, namely

To produce doctors who, at the end of undergraduate training, will be able to:

• Have a unified value system who can integrate the physical, intellectual,
moral and spiritual realities resulting in a balanced development of the
total human personality

• Apply with competence, the knowledge and skills gained, with the
potential to specialized in any field of medicine

• Function effectively in any settings

• Identify and analyse health problems at the level of the individual, family
and community and solve them through health promotion, prevention,
cure and rehabilitation, utilizing resources economically and effectively

• Take into consideration and respect the religious and moral values, culture
and traditions of the communities they are working in and

• Understand and accept that medical education is a continuing life long
process.

In addition of testing of knowledge, other aspects assessed include:

• The student’s ability to perform history taking and proper physical
examination with care and compassion

• Interpretation of data from history taking and physical examination
• The use of problem solving skills to arrive at a professional

diagnosis/differential diagnoses
• Proposing investigation and management plan including interpretation of

the results.

Particular attention is paid to the ability of student’s to manage the patient
comprehensively (viz curative, preventive and promotive aspects of patient care)
taking into account the patient in the context of the family and community.

The final professional examination for the M.B.B.S. degree consists of two
parts:

c) Patient Management Problem (PMP)

There are two 2-hour papers consisting of two cases each. Three of the cases
are from the disciplines of Internal Medicine, Surgery and Obstetrics &
Gynaecology respectively. The fourth case is either from the Paediatrics,
Psychiatry and Palliative Medicine or Orthopaedics, Traumatology &
Rehabilitation discipline. Although the cases are from certain disciplines, the
patient management problems are not necessarily discipline-specific. They can
be designed to cover all aspects of patient care including medical ethics.

• Each case consists of three triggers (20 minutes/trigger).
• Each trigger of the PMP is answered on a separate answer book so that they

can be marked by the relevant examiners.
• An answer scheme is prepared for each case.
• The answer scripts are to be marked by the external and internal examiners.

d) OSCE

Students are expected to interpret the data and information provided placed at
10-15 stations (5/10 minutes/station). The stations consist of questions from
the following disciplines:

• There are 10-15 stations, each of five (5) or ten(10) minutes duration.
• The questions will emphasize on clinical skills and interactive sessions.
• The total mark for each station is 10 marks/20 marks.
• The minimum mark for each sub- question is 0.5
• The distribution of questions is as in table 4 below:-

d) Clinical Examination

Each student is examined in one long case and at least three short cases from the
following groups of cases:

A. Medicine and Ophthalmology
B. Surgery, ORL-HNS, Orthopaedics
C. Obstetrics & Gynaecology
D. Psychiatry and Paediatrics

Each group of examiners consists of three examiners from different disciplines.
Each student will be examined by two groups of examiners; one for long case
examination and the other for short cases examination. At least one of the
examiners groups is headed by an external examiner. Examiners are to explore a
student’s knowledge, not his/her ignorance.

Generally, what is expected from a student in the long case includes the ability to:
• Present relevant positive and negative findings from the history;
• Present and demonstrate physical findings;
• Formulate a diagnosis, identify problems and offer possible differential
diagnoses based on logical reasoning;
• Request and interpret relevant laboratory and other investigation and
• Outline appropriate management of the patient.

In the short clinical cases examination, the candidate is expected to recognize a
series of physical signs and subsequently arrive at possible diagnosis.

The Clinical examination consists of the following:

i) Long Cases

• Each candidate will have only one (1) case of 1½ hours’ duration.
• The cases can be selected from medicine or surgical disciplines.

ii) Short Cases

• Each candidate is examined in at least three (3) short cases.
• Each case will be examined for 15 minutes.
• A candidate who has been examined in Medicine-based disciplines in

long case is going to be examined in at least two (2) short cases in
surgery-based disciplines and one short case in Medicine-based
disciplines

e) The Board of Examiners for the Final Professional Examination consists of
internal and external examiners. The external examiners must be familiar
with the integrated Medical curriculum of UniSHAMS, but must not be
involved, currently, in the teaching of UniSHAMS Medical students.

6. ELIGIBILITY TO SIT THE EXAMINATION

6.1. End of Semester Examination

To be eligible for the End of Semester Examination, the student must
fulfill the following pre-requisite:

a) attendance (lectures, PBL, Clinical strand session and practical) at
least 80%.

b) Not involve in any serious disciplinary problem

6.2. First Professional Examination

6.2.1.A student who has completed Year 2 and passed all the End of
Semester Examinations and/or Resit Examinations is eligible to
take the First Professional Examination at the end of Year 2.

6.3. End of Posting Examination

Pre-requisite for the End of Posting Examination includes:

a) Attendance 80%
b) Satisfactory completion of the log book
c) Not involve in any serious disciplinary problem

6.4. Final Professional Examination

To be eligible to take the Final Professional Examination, a student must
have:

a. Satisfactorily completed all Year 5 Postings;
b. Had no absence, without a valid reason, of more than two (2)
b. weeks continuously in any Posting;
c. Performed satisfactorily in all Year 5 End of

PostingExaminations.
d. Not involve in any serious disciplinary problem

7. THE MARKING AND GRADING SYSTEM

7.1. Marking of papers and all arithmetical manipulations will be in
percentages prior to conversions to grades.

7.2. The conversion of percentages into grades, where necessary, is as

8. EXAMINATION RESULTS AND CRITERIA FOR PROMOTION

8.1. Promotion to year 2

8.1.1. To be promoted to Year 2, a student must have:

a) Passed all the End of Semester Assessments
or

b) having failed one or more End of Semester Assessments,
passed the related Re-sit Examination(s).

8.2. Year 2 Assessments

8.2.1. As in Year 1, End of Semester Assessments will be held at the
end of semester 3 and semester 4.
Students who fail any End of Semester Examination must
take a Re-Sit Examination.

8.2.2. A student who fails five (5) or more courses is not eligible to
take the First Professional Examination and, as a consequence,
is required to repeat Year 2.

8.3. The First Professional Examination

8.3.1 The First Professional Examination is designed to assess the
student’s ability to apply and integrate basic science knowledge
into the solution of common clinical problems.

8.3.2 To be promoted to Year 3, a student must obtain at least
50% of the total marks.

8.3.3 Candidates who obtain total marks in the Borderline Failure
category (i.e., a final mark of > 45% < 49%) will be given a
Viva Voce Examination.

8.3.4 A candidate who achieves a satisfactory standard at this
Examination will be awarded a C (50%) pass.

8.3.5 A candidate who achieves 80% and above will be awarded
distinction.

8.3.6 Any candidate who fails the First Professional Examination:

a) Is required to take a Supplementary Examination to be
conducted not less than two weeks after the First Professional
Examination. This examination consists of a Theory
Examination and a Practical Examination similar to those
comprising the Professional Examination.

b) A student who passes the Supplementary Examination will be
promoted to Year 3.

c) A student who fails the Supplementary Examination will not be
promoted to Year 3 but may be permitted to repeat any of the
courses offered in Year 1 and/or Year 2 over a period of one
(1) year. The courses to be repeated will be decided upon by
the Board of Examiners of the Kulliyyah.

d) In the repeating year, no student is permitted to transfer any
credits, marks or grades from previous Endof Semester
Examinations the course being repeated.

e) A repeating student will be able to re-take the First
Professional Examination in the following year. If the student
fails the First Professional Examination again, he/she will have
his/her last opportunity to pass this Examination via the
Supplementary Examination. If a student fails to pass the

Supplementary Professional Examination taken during his/her
repeat year (i.e., after four (4) attempts) he/she will be asked

8.4. Phase 2 Assessments

8.4.1. To be promoted from Year 3 to Year 4 a student must:

a) Complete all Year 3 postings
and

b) pass all End of Posting Examinations.

8.4.2. To pass a Year 3 End of Posting Examination, a student must
obtain at least 50% of the total marks

8.4.3. In the repeating year, no student is permitted to transfer
credits, marks or grades from core subjects of the previous
year’s Postings.

8.4.4. To be promoted from Year 4 to Year 5 a student must have:

Completed, satisfactorily, all Year 4 Postings, including the
achieving at least a 80% attendance record at all formal
teaching-learning activities. A student who fails to complete
the required aspects of any Posting will not be eligible to take
the relevant End of Posting Examination.

8.4.5. To pass a Year 4 End of Posting Examination, a student must
obtain:

a)At least 50% of the total marks
and

b) not less than 40% marks (out of 100% marks) in the
Clinical component.

8.4.6 Students who fail in two and less major end posting
Examination in Year 4 are required to repeat the posting
during the elective posting period as a remedial posting and
pass the reassessment. Students who fail the remedial posting
examination will have to repeat Year 4.

8.4.7 Students who fail in more than two major End of Posting
Examinations in Year 4 will have to repeat the year.

8.4.8 Students who fail more than three minor postings
examinations must repeat year 4.

8.4.9 Year 4 students who failed minor postings examinations must
re-sit and pass the examination as a prerequisite to sit for
Final Professional Examination.

8.4.10 In the repeating year, no student is permitted to transfer
credits, marks or grades from the previous year’s Postings.

8.5. The Final Professional Examination

8.5.1. To pass the Final Professional Examination, a candidate must
obtain all of the following:

a) A pass in the Theory Examination ³ 50%
and

b) A pass in the Clinical Examination ³ 50%.

8.5.2. Candidates who obtain 49.5% in any of the above components
will be given a passing mark.

9. THE ASSESSMENT OF PERSONAL QUALITIES, ATTITUDES,
BEHAVIOUR AND INVOLVEMENT IN CO-CURRICULAR ACTIVITIES

Personal qualities, attitudes, behaviour and involvement in co-curricular
Activities can be assessed from a qualitative perspective. Therefore, on-
going assessment of these attributes is part of the continuous assessment of
students and so contributes to their overall performance outcomes.

9.1. Methods of assessment

a) To assess students’ attitudes, personal qualities and behaviour, a
standard checklist will guide each assessor. To assess students’
involvement in co-curricular activities and community services
activities, a student activity form will be given to each student to be
filled out and returned to the Kulliyyah.

b) Evaluation, of an essentially informal nature, will be carried out
throughout the Medical programme but the recording of the
assessors’ impressions will be done only once every
Semester/Posting.

c) Students’ personal qualities, attitudes and behaviour will be observed
both directly and indirectly during learner-teacher encounters during,
for example, formative assessments, practical sessions, ward rounds,
problem solving sessions, etc.

9.2. Implications

a) The overall grading for personal qualities, attitudes, behaviour and
co-curricular activities will be used in the summative assessment at
the end of a Semester or Posting. A student who is consistently
graded as ‘unsatisfactory’ will be required to undergo a remedial
programme involving counseling and/or a community service activity
or project designed to improve the specific weakness in attitude,
behaviour or personal qualities.

b) A student given a consistently ‘unsatisfactory’ grade may be barred
from taking the relevant Professional Examination.

c) An overall grade for personal qualities, attitude, behaviour and co-
curricular activities will be given at the end of the Medical programme.
This grade will be taken into account when deciding on whether or
not to award honors to students who have excelled academically.

10. THE PREPARATION AND VETTING OF EXAMINATION QUESTIONS

10.1. Lecturers are required to prepare Objective questions (OBA) and
OSPE questions early – preferably during the preparation stage of
each lecture, seminar or practical.

10.2. All questions should be discussed at the discipline level before they
are submitted to the Semester/Posting Coordinators for use in
formative or summative assessments.

10.3. The Semester/Posting Coordinators should check all questions for
relevance, appropriateness and integration of strands before they are
vetted at the Semester or Posting level.

10.4. For End of Semester/Posting Examinations, all question vetting
sessions should be chaired by the Year Coordinator.

10.5. For MEQs, the Year Coordinator, with the assistance of the relevant
Subject/PBL/Posting Coordinator, should identify the main themes,
the aspects or strands to be integrated into the questions, and the
members of the team to be responsible for the writing up of each
question. The team should comprise the Subject/PBL//Posting
Coordinator or the senior lecturer for the main theme, and
representatives from the Biomedical, Clinical, Public Health and
Behavioural strands.

10.6. Each question should be vetted at the Year level before it is submitted
to the Academic Management Centre.

10.7. For the Professional Examinations, the vetting of questions should be
done at three levels: (1) Discipline level; (2) Faculty Vetting
Committee level, and; (3) at the Dean’s level.

10.8. All End of Semester, End of Posting, Professional and Supplementary
Examination questions must be submitted to the Academic
Management Centre to be typed in accordance with the University’s
required format. All questions must be treated strictly as confidential
and stored in locked filing cabinets of the strong room in the
University’s Examination Unit.

10.9. In all End of Semester Examinations, End of Posting Examinations,
Professional Examinations and Supplementary Examinations the use
of previously used questions from the Kulliyyah Question Bank should
be limited to not more than 30% of the total number of the questions.

10.10. In all summative assessments, a new set of questions should be used
if either the lecturers or the Coordinators find, or have reason to
suspect, that there has been a breach of confidentiality in relation to
all or any of the questions.

11. THE CONDUCT OF EXAMINATIONS

Examinations are conducted in accordance with the following rules and
procedures:

11.1. Candidates are required to obey any instruction given by an
Examination invigilator for proper conduct of the examination.

11.2. Candidates are required to be in their places in the examination room
not less than ten (10) minutes before the time scheduled for
commencement of the examination.

11.3. No item (e.g., bag, hand-phone, writing paper, blotting paper,
manuscript or book) other than a specified aid, is to be brought into
the examination room.

11.4. Candidates shall not be admitted to an examination later than thirty
minutes from the time of commencement of the examination.

11.5. Candidates shall not be permitted to leave the examination room
before the expiration of thirty minutes from the time of the
commencement of the examination.

11.6. Candidates shall not be re-admitted to the examination room after
they left it unless, during the full period of their absence, they have
been under approved supervision.

11.7. Candidates shall not by any improper means obtain, or endeavour to
obtain, assistance in their work, give, or endeavour to give, assistance
to any other candidate, or commit any breach of good order.

11.8. Smoking and eating are not permitted during the course of any
examination.

11.9. A candidate who commits any infringement of the rules governing
examinations is liable to immediate expulsion from the examination
room, disqualification from the particular examination, and to such
further penalty as may be determined in accordance with the
University’s Bye-laws.

12. THE ROLES OF INVIGILATORS AND OTHER STAFF DURING THE
EXAMINATION PROCESS

12.1. Beside internal invigilators, external invigilator(s) may also be
appointed by the Academic Office to assist the running of the
examination.

12.2. The Chief Invigilator collects the question papers (in sealed
envelopes) and any other relevant documents from the University’s
Academic Office.

12.3. The Chief Invigilator, other invigilators and relevant members of the
academic staff should be present at the examination room not later
than twenty (20) minutes prior to the commencement of the
examination.

12.4. Invigilators and Academic Office staff assist the Chief Invigilator to
distribute the question papers and any other relevant documents to
the students.

12.5. The invigilators are required to check the question papers to ensure
that there are no missing pages, spelling errors, etc.

12.6. Support staff from the Academic Office must ensure that all the
necessary materials such as calculators, clock, pencils, erasers, blank
paper, white board pens, etc., are in place and available as required.

12.7. At least one member of the support staff from the Faculty’s
Academic Office must be present throughout the entire examination
session.

12.8. Students are permitted to enter the examination room once the
Chief Invigilator is satisfied with preparations. This should be about 15
Minutes prior to the commencement of the examination.

12.9. Before the commencement of the examination, the Chief Invigilator
briefs the students about the examination, the examination rules and
announces any correction/s to be made.

12.10. The Chief Invigilator advises the security staff or invigilators about
who will accompany any student who wants to leave the examination
room, temporarily, during the examination.

12.11. The Chief Invigilator announces the end of the examination.

12.12. On completion of the examination, the Chief Invigilator, with the

assistance of the other invigilators, collects and checks the question
papers, the answer papers and all other relevant documents. Once
satisfied that all the question papers and the answer papers are in
order, the Chief Invigilator signs and dates the relevant documents
and submits the documents to the Academic Office staff.

12.13. The Academic Office staff check, sign and date the receipt of all
Documents forwarded by the Chief Invigilator.

12.14. The Academic Office staff distribute the answer papers to the relevant
examiners.

12.15. On receipt of the students’ answer papers, each examiner is required
to check and sign, as having received in good order, the appropriate
answer papers.

13. MARKING AND ENTERING MARKS PRIOR TO THE EXAMINATION
COMMITTEE MEETING

13.1. During the marking process, the marks obtained are entered by the
examiners onto previously prepared Subject/Posting templates.

13.2. On completion of the marking process, the Subject or Posting
Coordinator checks, signs as accurate, and dates each of the
Subject/Posting marks sheets.

13.3. A Subject/Posting coordinators enters the mark/grades into Excel
spreadsheets.

13.4. On completion of the marks/grades entry process, the clerk
responsible for entering the marks, checks, signs as accurate, and
dates one print out of each spreadsheet.

13.5. The signed print outs are forwarded to the Subject/Posting
Coordinator.

13.6. The Subject/Posting Coordinator makes a final check of the
marks/grades, countersigns and dates the sheets. If any corrections
are necessary, these are indicated in a clear, legible manner.

13.7. The sheets are forwarded to the Academic Office for final preparation
for the Examination Board Meeting.

14. EXAMINATION COMMITTEE AND SPECIAL FACULTY MEETINGS

14.1. All End of Semester, End of Posting, Professional Examination and
Supplementary Examination results should presented and endorsed at

Examination Committee and Special Faculty Meetings before they are
submitted to the Academic Management Centre and to the Senate.

14.2. The Examination Committee comprises members of the Faculty’s
Curriculum and Examination Committee and all lecturers and
academic staff who have been involved in the preparation of the
questions, the invigilation of the relevant examination and the
marking of the answer scripts.

14.3. The Deputy Dean (Academic) chairs Examination Committee Meetings
for End of Semester and End of Posting Examinations.

14.4. The Dean chairs all Special Faculty Meetings to endorse assessment
results and all Examination Committee Meetings to endorse
Professional and Supplementary Examination results.

14.5. All examination results are strictly confidential and subject to the
approval of the Senate of the University.

15. IRREGULARITIES IN THE PREPARATIONS FOR AND CONDUCT OF
EXAMINATIONS

Where the Dean of the Faculty has reason to believe that irregularities have
been committed in the preparation and/or conduct of any examination
she/he, at her/his discretion, may suspend the examination process and
conduct an investigation, the result of which shall be forwarded to the
Deputy Rector (Academic) and Rector for appropriate action.

The Dean may, in consultation with the Deputy Rector and Rector arrange a
substitute examination under which condition the results of the original
examination become null and void.

16. BREACH OF DISCIPLINE IN EXAMINATIONS

Where the Rector is satisfied that a candidate has committed a breach of any
provision of these regulations or any rules or procedure made thereunder or
any instruction made by an authority under these regulations, such candidate
shall be deemed to have committed an offence and the Rector may:

a) Authorize the student discipline authority to take action against the
candidate according to the University rules. If the student is found
guilty of a serious offence, as defined by the student discipline
authority, he will receive an F grade for the implicated Semester or
Posting or other examination;

b) Report the matter to the Senate for such action, or;

c) Act in any other way he deems fit

17. SPECIAL EXAMINATIONS

17.1. Absence from any Examination will not be permitted except on
medical or psychological grounds that, on the day of examination,
have been duly certified by the University clinic or, in the case of a
serious emergency, by a University panel doctor.

17.2. Where a student whose absence from those examinations mentioned
above is excused, the Faculty’s Curriculum and Examination
Committee may authorize a Special Examination.

17.3. A student on Medical Leave for pregnancy or prolonged illness will not
be eligible to sit for a Special Examination unless she or he fulfills the
80% attendance requirement.

18. APPEALS FOR THE RE-EVALUATION OF EXAMINATION RESULTS

18.1. At Faculty level, there shall be a Board of Appeal which is responsible
to the Senate for the purpose of dealing with all appeals from
students of the Kulliyyah/University.

18.2. The Dean shall be the Chairman of Board of Appeal.

18.3. The members include the Deputy Deans (Academic and Student
Affair) and TWO other members of the academic staff who are not
the examiner or examiners of the subject or Posting
Assessment/Examination as a result of which the appeal is made. The
examiner or examiners may, however, be called by the Board to
present his/her/their report(s) or views if deemed necessary. The
representative from the University’s Academic Office shall be invited
to attend the meeting.

18.4. All appeals must reach the Dean of the Kulliyyah not later than three
weeks after the result is announced and must be accompanied by a
receipt showing that the specified fee has been paid to the University.

18.5. The re-evaluation result will be endorsed by the Kulliyyah’s
Examination Committee and by the Senate of the University before it
is notified to the student. The re-evaluation result is final.

Appendix: 4 Guidelines for Assessment in Clinical Examination

1. LONG CASE
There will be at least one external examiner and one internal examiner for each
candidate. Examiners are reminded to explore a candidate’s knowledge and not his
ignorance; cover as wide field as possible and move on to something new whenever
a candidate get stuck.

1.1. History taking

Assessment and weightage should be made in relation to relevant symptomatology,
sequence of the disease process and associated problems rather than to routine
questions. There must also be a due emphasis on relevant history including the
family and social background of the patient.

1.2. Physical examination

Assessment should be made in relation to correct technique of examination and the
ability to elicit physical signs. Emphasis is given on the thoroughness of examination
of the system involved.

1.3. Formulation of Diagnosis

Emphasis should be on the ability to arrive at differential diagnoses in relation to the
problems identified on the history and physical examination. Look for the ability to
logically deduce the most likely diagnosis.

1.4. Investigation

Look for the ability to request and justify the investigations relevant to the problems
identified. Emphasis should be on simple investigations rather than sophisticated
ones and the ability to request investigations in a logical sequence. Candidate should
be able to interpret common investigations e.g. X-ray, ECG, etc.

1.5. Patient Management

Look for ability to recommend and justify appropriate treatment based on the
diagnosis. Emphasis should be on the priorities of management rather than detailed
management protocols e.g. operative procedure.

2. SHORT CASES
Emphasis should be on correct techniques of examination and the ability to elicit
correct physical signs. Look for ability to interpret physical signs satisfactorily.

The guidelines for marking long and short cases are as in the appendix 5 and 6.
These guidelines are flexible. The dispersions of marks may vary with each case.

Each examiner will mark the student’s performance separately before arriving at a
consensus mark.

3. OUTSTANDING PERFORMANCE
History: Elicit problems, relates data, stresses important points, well-organized
approach.

Physical Examination: Elicits and interprets correctly all signs, good techniques,
good organizational approach.

Formulation of Diagnosis: Makes reasonable conclusions from available data, able to
give correct provisional and differential diagnoses.

Investigations: Plans/requests and interpret investigations appropriate to the
problem with attention to specificity, reliability, patient safety, comfort and cost. Able
to justify the request for investigation.

Patient Management: Suggests appropriate comprehensive management. Exhibits
awareness of the role and possible complications of the proposed intervention
(adverse drug reactions, morbidity)

4. SATISFACTORY
As in outstanding but misses few relevant information in history, omits or examines
less satisfactorily for some relevant physical signs, able to give correct provisional
diagnosis but not all differential diagnoses, misses some relevant investigations or
some investigations are not appropriate or misses few relevant points in
management.

5. INADEQUATE
History: Concentrates on data not relevant to the problem, misses important
information, not well organized.

Physical examination: Misses some important physical signs, poor examination
technique, poor organization, invents signs.

Formulation of Diagnosis: Makes improper and irrelevant conclusions from available
data, wrong provisional diagnosis, poor in differential diagnoses.

Investigations: Misses relevant investigations, asks for investigations not
appropriate to the problem, misinterpret data.

Patient management: Misses many important points on principles of management,
suggests inappropriate management, shows lack of awareness on role of proposed
interventions and their possible complications.
