Medicare Part B

In 2016, the standard Part B premium amount will be $121.80 (or higher depending on

income). However, most people who get Social Security benefits will continue to pay the same
Part B premium amount as they paid in 2015. This is because there wasn’t a cost-of-living
increase for 2016 Social Security benefits. Beneficiaries will pay a different premium amount in
2016 if they:

• Enroll in Part B for the first time in 2016. Previous beneficaries stay at $104.90.
• Don’t get Social Security benefits.
• Are directly billed for Part B premiums.
• Have Medicare and Medicaid, and Medicaid pays premiums. (The state will pay the
standard premium amount of $121.80.)
• Have a modified adjusted gross income as reported on IRS tax return from 2 years ago of $85,000
for an Individual or $170,000 for a Couple ($170.50 to $389.90/month)
• Changes in 2016:
• Part A deductible - $1288 (days 1-60)
• Skilled Nursing - $161.00 per day (days 21-100)

SEP Reminders (Special Enrollment Period)
Beneficiaries can make changes to Medicare Advantage and Medicare prescription drug
coverage when certain events happen in their lives. For example, they can enroll in an MAPD
plan if they move, want to end their expensive group coverage or retire. These chances to
make changes are called Special Enrollment Periods (SEPs). Rules about when they can make
changes and the type of changes you can make are different for each SEP.

MyHealth Online
What does MyHealth offer?
With MyHealth patients can view health information including allergies, medications, lab results and
immunizations. They can also see past and upcoming clinic appointments send messages to their clinics,
schedule appointments and more.

Who can sign up for MyHealth?

Any Stanford Health Care patient, at least 18 years old, with a physician who is in a MyHealth-active clinic,
can sign up for MyHealth.

How can I find out more about MyHealth?

If you would like more information about MyHealth , please have your members speak with their
physicians or a clinic staff member, or review the information on the MyHealth website at :
myhealth.stanfordhealthcare.org.

Member ID Cards

Stanford Health Care Advantage provides each member with an ID card, which contains
demographic information about the member as well as important coverage information,
such as the PCP’s name and phone number, copayment or coinsurance responsibilities and
important service phone numbers. It is the member’s responsibility to present his or her
member ID card at the time medical services are obtained.

Stanford Health Care Advantage encourages providers to make a copy of the member’s ID card
for their records and confirm with the member at each visit that the individual’s insurance
coverage and PCP have not changed.
