started working from home, whereas others had ceased working entirely to care for their dog. Owners were commonly frustrated and disappointed with lack of treatment success and ongoing medication side effects, and expressed feelings of helplessness. Sometimes this was again linked to the dog-owner relationship, for instance in the case of side effects where owners felt their dog’s demeanour or personality was affected. However, in other instances it was linked to lifestyle-related or financial issues rather than being a purely emotional impact. In a number of cases these feelings of helplessness were also associated with despair, where owners had lost hope of their dog’s condition improving. The majority of interviewees reported that other people external to the household struggled to understand the lifestyle, emotional and logistical impacts of owning a dog with epilepsy. This lack of understanding sometimes caused feelings of isolation, and was one of the most significant contributors to a reduced quality of life for some owners. Due to the episodic nature of idiopathic epilepsy, and many dogs appearing normal between seizures, owners often believed that other people with no prior experience of the condition could not appreciate the potential impact of the seizures, and were not understanding of the owner’s commitments. Feelings of isolation were exacerbated where owners cancelled or avoided social events due to fear of leaving their dog. This study provides new insights into the concerns and experiences of owners of dogs with idiopathic epilepsy, and the impact of caring for a dog with epilepsy on day-to-day routine and lifestyle. The use of a qualitative methodology allowed influencing factors and emotional aspects to be explored in depth. A number of direct impacts on owner quality of life were identified, including emotional and financial factors and the time associated with providing care for their dog, as well as revealing secondary impacts such as lack of sleep and social isolation. These findings illustrate for the first time the extent of the challenges that owners of dogs with idiopathic epilepsy might experience. The emotional impacts experienced by the participants may indicate a need for increased support for owners of dogs with epilepsy. Potential challenges and negative outcomes may not be adequately discussed by veterinary surgeons in an attempt to not upset or overwhelm owners, or due to lack of awareness of these challenges. However, avoiding the provision of data on negative outcomes in epilepsy may not necessarily be beneficial to carer quality of life. In human medicine, the provision of information by paediatric neurologists regarding sudden death in children with epilepsy has been discussed with parents, and there were concerns that it could be unnecessarily shocking or stressful for parents [32]. However, despite these concerns, 91% of parents in the same study wanted to be provided with the information [32]. Therefore, there may be some benefit in veterinary surgeons discussing more severe clinical signs or outcomes in canine epilepsy with dog owners, but this would need to be assessed on an individual basis, and further research is required to identify what information owners would find valuable, and at what stage of their dog’s condition. VALLEY FEVER IN DOGS By Krista Williams, BSc, DVM; Cheryl Yuill, DVM, MSc, CVH What is valley fever? Valley fever is a disease caused by a fungus known as Coccidiodes immitis. Although the proper name for this disease is coccidioidomycosis, it is most often called valley fever, California disease, desert rheumatism, or San Joaquin valley fever. Where is valley fever found? This soil-dwelling fungus has adapted to survive in desert climates and is prevalent in certain parts of Arizona, California, Nevada, New Mexico, Texas, Utah, northwestern Mexico, and parts of Central and South America. It has been also reported in south-central Washington State. Infections rates vary greatly by county, and not all the factors that contribute to the rate of infection in an area have been determined. Valley fever tends to occur during certain seasons. What species can be infected with valley fever? This disease is common in humans, and has been isolated in dogs, cattle, horses, deer, elk, mules, llamas, apes, monkeys, kangaroos, wallabies, tigers, bears, badgers, otters, fish, and marine mammals. Dogs appear to be very susceptible to infections with valley fever, probably because they sniff the ground and dig in the dirt, potentially inhaling large numbers of spores at a time. How is valley fever spread? Like many fungi, C. immitis has a complex life cycle. It has two completely different forms, depending on whether it is in the environment or has entered a host animal. When it is in the environment, it exists as a mold. During dry spells, the mold goes dormant in the soil, and can remain dormant for prolonged periods of time. Once the rains come, the fungus grows and produces long filaments of mold that contain infectious spores. The tiny spores readily become airborne when the soil is disturbed by winds or by construction, farming, or digging. If the spores are inhaled, they transform into a yeast-like organism that infects the lungs. “The disease cannot be directly transmitted from person to person, nor can it be transmitted from people to animals.” The disease is contracted by inhaling fungal spores. The disease cannot be directly transmitted from person to person, nor can it be transmitted from people to animals. In other words, an animal with valley fever is not contagious to other pets or your family members. What are the symptoms of valley fever in dogs? Once the spores are breathed into the lungs, they develop into larger structures called spherules. In a healthy adult dog, the dog’s immune system will wall off the organisms in the spherules and there will be no further problems. In these cases, the signs of disease are usually very mild, and often the dog does not even become visibly ill. “Once the spores are breathed into the lungs, they develop into larger structures called spherules.” However, in dogs that have a weak immune system because of age or underlying illness, serious illness can develop, therefore, both very young puppies and senior dogs are more susceptible 54 PAGE SHORTHAIR JOURNAL


to illness from valley fever. In these patients, the spherules continue to grow and eventually burst, releasing more infectious organisms to spread throughout the lungs or to other organs in the body where the cycle repeats itself over and over. Valley fever can take two main forms of disease in the dog, the primary disease and the disseminated disease. The primary disease is limited to the lungs. Signs of primary valley fever include a harsh dry cough, a fever, a lack of appetite, and lethargy or depression. These signs usually occur about three weeks after infection although sometimes, the organism can lay dormant in the body for up to three years before signs occur. In the disseminated disease the fungus has disseminated or spread to other parts of the body. The bones and joints are most commonly infected, and lameness is the most common sign. The joints may become swollen and painful. Other signs are non-specific and may include lack of appetite, lethargy, or depression, a persistent fever, and weight loss. Infection can occur in the eye causing inflammation and sometimes blindness. In rare cases, the fungus invades the brain, resulting in seizure activity. How is valley fever diagnosed? If you live in an area where this disease is prevalent and your dog is showing signs that are consistent with valley fever, your veterinarian will recommend diagnostic testing to determine whether your dog has this infection. This testing will most certainly include a titer test to determine whether your dog has valley fever antibodies (evidence of exposure to the fungus). Depending on your dog’s symptoms and the severity of illness, your veterinarian may also recommend additional blood tests and diagnostic X-rays of the chest and any affected legs. The fungus can also be detected by microscopic examination of samples of fluids or infected tissue. Veterinarians in other parts of North America seldom, if ever see cases of valley fever. Therefore, if your dog has spent any time in an area where this disease is prevalent, it is important to let your veterinarian know about this travel history should your dog develop signs that are consistent with valley fever. What is the treatment for valley fever? At the present time, dogs that develop valley fever require lengthy treatment with antifungal medications. The duration of treatment will depend on the severity of infection. In many cases treatment will be required for 6-12 months. If the fungus has invaded the nervous system, the dog may require antifungal medication for life. “At the present time, dogs that develop valley fever require lengthy treatment with antifungal medications.” There are a number of antifungal medications that are effective against the disease. The most commonly prescribed medications are ketoconazole (brand name Nizoral®), itraconazole (brand names Itrafungol® and Sporanox®), and fluconazole (brand name Diflucan®). Although the treatment is prolonged, dogs usually begin to feel better within 1-2 weeks after the treatment begins. The most common side effects of these medications include vomiting and loss of appetite. Since they can be toxic to the liver, your veterinarian will recommend periodic blood tests to monitor liver function. Other supportive care may be needed depending on the location of the infection. This may include hospitalization for intravenous fluids or oxygen therapy; management of congestive heart failure may require surgical intervention; and infected, painful eyes may require surgery to alleviate the pain. What is the prognosis for recovery? A small number of dogs, usually those that develop disseminated disease, will die from valley fever. However, the majority of dogs that are treated appropriately will recover from this disease. Your veterinarian will periodically repeat the antibody titer test to determine when the antifungal medication can be stopped. Shorthair Journal Policies and General Advertising Information SUPPORT THE SHORTHAIR JOURNAL Reprinting of Articles: All materials printed in any GSPCA publication becomes the sole property of the GSPCA. All reproduction rights reserved. Materials (except advertising) may be recopied without written permission in newsletters published by AKC-recognized German Shorthaired Pointer Clubs. All others must receive permission in writing from the Editor. Credit must be given to Shorthair Journal or the GSPCA. Editing: The GSPCA reserves the right to edit all material, including advertising. Ad Claims: Any ads claiming health clearances or ratings (OFA, Cerf, etc.) must be accompanied by proof of such with copies of the actual certifications or clearances. Pedigrees must be submitted in written form or as a copy of certified pedigree. GSPCA assumes no responsibility for errors in ad pedigrees. Photographs: Photos submitted must be originals (no photocopies or laser prints). Photos sent via email must be in the formats such as (tif or jpg) and at least 200 dpi or preferred 300 dpi resolution. Please identify photo with your name, mailing address and the name of the dog. Indicate where you would like the dog placed in the mock up. Indicate any special cropping requirements. Please do not send color copies or reproductions of original photo. NO WEB PHOTOS CAN BE USED. All digital photos must be of suitable resolution to transition to printed media 300 dpi. Ad Copy/Article Submission: Please submit ad copy in writing or via e-mail as an attachment. When submitting via e-mail be sure to include, special fonts, logos and photos with your attachments. (Embedded photographs and fonts will not print correctly). Larger files may be uploaded to file server, please call for login and passwords. Materials printed reflect the opinions of the authors and not of the GSPCA. Ad Size: Full bleed size should be 8.75 x 11.25 inches. Live area is 7.5 x 10 inches. Any information which exceeds the live area could be cropped off during binding/ trimming. Ad Payment: Payment for advertising must be received with the submission of your ad. Payment can be made with check, postal money order, or on-line using Paypal account or a credit card. For Credit Card or Paypal payment go to: www.gspca.org, click on the Shorthair Journal and follow the instructions. Checks or money orders should be made payable to GSPCA and mailed to PO Box 2084, Tarpon Springs, FL 34688. All checks or money orders must be in US funds. Ad Specifications: Ad specifications are 8.75 x 11.25 inches for full bleed. Please observe a margin of 0.5 inches on all sides - no text, logos, graphics, or relevant portions (ie; dog feet, nose) of photos should be beyond this margin. Final output size is 8.5 x 11 inches. Such letters that are acceptable must indicate the name of the author. (4) It is the responsibility of all advertisers in the GSPCA Shorthair Journal to insure the accuracy of all references made to official field/obedience/show wins and awards.
