• Patient Story

o In the beginning of days for my husbands hospital stay he was in a coma.
For the first 15 days out of the total 52, I was all alone and scared and
Carlos was there. In that first week, I would ask the same questions over
and over and he would without any judgment answer them over and
over. I would apologize for asking the same things repeatedly and he
always said “no need to apologize, I will answer them over and over until
you no longer have any questions.” He would fight with his all to insure
my husband was taken care of to the best of his abilities. See my
husband had traumatic head trauma and his ICP had to be watched
closely. There were ups and downs of the pressure and his temperature
continually running slightly with fever. In that current condition it was
life threatening. Carlos kept filling the ice packs and packing them on as
well as applying the cooling blanket that would have to be on and off for
the first 2 weeks. The whole entire ICU team was amazing, but Carlos
was a one on one that helped me to be reassured things were going to
be ok.

DAISY Award Winner –
Desiree Sierra & Lisette Chavez

• Patient Story

o Desiree Sierra and Lizette Chavez - both of these nurses were amazing!
They notified me of what medications I was being given what they do and
why I’m taking them. This is very important to me and it shows a kind of
respect that I admire! Both notified me of my care plan and how they plan
to help me accomplish my goals! Lizette Chavez took the time to go over
each discharge goal with me and helped me understand everything I needed
to do after going home. Both nurses were incredibly busy but still took the
time to make sure I understood everything about my plan of care! This
means so much to me and I can’t tell you how very important this is. I’ve
never received this kind of wholistic care before and been this informed! It
really made the recovery process so much less strenuous and scary knowing
what all was going on and why.

Thank you all for making my recovery so easy and comfortable!!!

DAISY Award Winner – Chris Ebert

• Patient Story

o My husband and I were so excited to welcome our first baby into the world and had spent my entire pregnancy planning and
dreaming of how our birth experience would go. We took classes read lots of books and articles and researched many different items
to include in our birth plan to anticipate the birth we desired. When we were admitted to the L&D unit I was anxious hurting and so
uncertain of my ability to continue with my birth plan as written. Enter Chris who I’m convinced is more an angel walking on earth
than a nurse. She came into my room immediately recognized my anxiety and downright fear and sat on my bed with me held my
hand and calmly explained her role in my labor process and asked to explain all my medical options respecting my birth plan but
desiring to give me choices. More times than I can count or was aware of Chris was in my room that night checking on both my
husband and I and ensuring our needs were met and questions were answered Not only was Chris compassionate and kind to me she
offered support and compassion to my husband who was the only person allowed in to labor with me due to COVID restrictions. Not
having my own mother there Chris filled in that gap of comforting strong reassuring presence. At one point in the night we found
out that there was the possibility of her getting to go home early which anyone would jump at the chance for especially on night 4 of
4. However we also found out that Chris requested to stay with us because she felt like we needed her. This simple and

unnecessary act of kindness speaks volumes of the person that she is and the heart of gold that she has.

Palliative Care MYTHS ABOUT
PALLIATIVE CARE

#1: Palliative Care hastens death.

Palliative care does not hasten death,
but provides comfort and the best
quality of life based on diagnosis and
stage of illness.

Who can Benefit from Palliative Care #2: Palliative care is only for
people dying of cancer.
 Patients with serious, long-lasting disease or with a life-threatening illness
are good candidates for palliative care (e.g., cancer, heart disease, AIDS, Palliative care can benefit patients
dementia, ALS, Parkinson’s, kidney failure) with life limiting illnesses.

What can Palliative Care Offer the Patient #3: Pain is part of dying.

 Palliative care is performed alongside the patient’s current providers, Pain is not always a part of dying and
providing an additional layer of support (e.g., symptom relief, counseling, there are many ways it can be
spiritual comfort, pain relief) alleviated.

 Palliative care benefits and supports the patient as well as the
family/caregiver.

 Assists in establishing and matching treatment and care goals through illness.

SAFETY ALERT: Lessons Learned

ICU – Potassium Issue

Situation

S On 10/13/2021 3 patients in DRMC ICU required potassium replacement. The pharmacy sent 2
vials of IV K+ ( 20 Meqs) each. There should never be vials of potassium on the nursing floors. The
doses must be drawn up in oral syringes in the pharmacy before sending to the unit.

Background

B Due to an Effer-K shortage, an ICU practitioner ordered IV formulations of potassium chloride
and potassium acetate to be given via NG or feeding tube.

Assessment
Night nurse felt this order may not be correct and went to Charge RN about his concerns.

A Charge RN reviewed the order and noted this could potentially be a medication error ( if the
medication was given IV). Together both nurses “ stopped the line” and the medication was not
given.

Recommendation

R It is an ISMP and HCA Best Practice to send all oral medications that are not commercially
available in unit dosed packaging to the floor in an oral syringe. Oral syringes are not compliant
with IV lines, helping to prevent such mix-ups.
