INVITATION FOR EMPANELMENT OF VENDORS/VENDOR RENEWAL

The General Manager, Security Paper Mill, Hoshangabad (M.P.) INDIA – 461 005, (A unit of SPMCIL) invites
sealed Application are invited for Vendor Registration.

Sealed documents are invited separately for each item from eligible and qualified vendors for their empanelment
for the following goods or services:

The above requirement is on estimated basis and this may vary as per requirement of SPM, Hoshangabad.

1. Vendor who wants to register them self with Security Paper Mill, Hoshangabad will have to submit be
application in the prescribed format and should have to submit a Demand Draft of Rs.250/- along with
the application in favor of “The General Manager, Security Paper Mill” payable at Hoshangabad.

2. Vendors can select any items from any categories in which they would like to be registered to get
enquiry from us. For registration more than one item, firm has to submit the separate documents for
each particular items/services. Vendor has to submit different envelops for each category of
items/services. Multiple items under one application will be rejected.

3. Interested vendors can download the Application Form our website:
http://spmhoshangabad.spmcil.com or the detailed application format may be purchased from the
office at Security Paper Mill, Hoshangabad between 10:00 hrs and 17:00 hrs.

4. Non-refundable Empanelment fee of Rs. 250/- will be charged for each vendor in the form of account
payee demand draft/ Banker’s Cheque/ Fixed Deposit Receipt, drawn on a scheduled commercial bank
in India, in favour of Security Paper Mill, Hoshangabad (A unit of SPMCIL), payable at Hoshangabad.

5. Vendors will ensure that their registration form, duly sealed and signed, complete in all respects as per
instructions contained in the registration form. Bidder to furnish stipulated documents along with
tender in support of fulfillment of tender criteria. Further correspondence in this regard will not be
entertained for any reason. Non-submission or incomplete submission of documents may lead to
rejection of offer.

6. Documents are dropped in the tender box located at the address given above on or before the closing
time indicated above.

7. All empanelled vendors will be allocated unique registration number.

8. The registration form is not transferable.

9. The validity period of empanelment will be 5 years from the date of empanelment and their
performance will be reviewed annually or as per requirement.

10. Potential new vendors may be added to the empanelment list continuously based on the satisfaction
of the eligibility criteria.

11. Firms found not satisfactory will be eliminated.

12. The existing suppliers to SPM, Hoshangabad are also required to apply for their registration.

13. The Empanelment Fee should be furnished in any one of the following forms : -

(a) Account Payee Demand Draft or
(b) Bankers Cheque.
14.The firm should furnish the firm’s name, full postal address (Office), Phone Number and Fax Number;
Name of the authorized contact person and e-mail address.

15.The firm should not have been blacklisted/debarred for dealing by SPMCIL, BRBNMPL, Government of
India or any State Govt. in any manner as on date of submission of registration form and an
undertaking should be submitted along with the form, to this effect.

16. The vendor can submit only one application against one type of material/work/services or items.
Vendors can apply for multiple items, for that they have to submit separate application, but
empanelment fee shall be Rs. 250/- Only. Multiple items under one application will be rejected.

17.Participants are required to stamp and provide their authorized signature on every page of document
and all the supporting documents submitted as a token of acceptance to the mentioned terms and
conditions.

IMPORTANT NOTE:

1. This form should be signed by proprietor/partner/Director/Company Secretary.

2. Incomplete forms received without the required enclosures shall be rejected.

3. Enlistment of any firm is done entirely at the discretion of SECURITY PAPER MILL, HOSHANGABAD and
SECURITY PAPER MILL, HOSHANGABAD reserves the right to reject any application without assigning any
reason.

4. Registration form should be sent to us along with the Demand Draft of Rs. 250/- (Rupees Two Hundred Fifty
Only) in favor of “ Security Paper Mill” payable at Hoshangabad to enable us to process this application.

GENERAL INFORMATION

A. The application form duly filled in all respects, together with all required enclosures must be submitted within
prescribed due date, late applications shall not be entertained.

B. The registered vendor will be communicated directly by SECURITY PAPER MILL, HOSHANGABAD after scrutiny
of their application. All such vendors will be allotted a Unique Vendor Registration Number which should be
invariably quoted in all their future correspondence.

C. The Vendor registration will be valid for Five Years from the date of registration by SECURITY PAPER MILL,
HOSHANGABAD. Whereas performance of vendors will be evaluated every year. Registered Vendor interested
to renew the registration on expiry of validity should apply for Re- Registration in the same format given
below before the date of expiry of the registration.

D. Benefit of becoming Registered Vendor: Based on requirements, SECURITY PAPER MILL, HOSHANGABAD
shall send Enquiries/ Limited Tender related to item registered by Registered Vendors through courier/ Post.
In case of Open Tender intimation of the same will be communicated to Registered Vendors. Interested
Registered Vendors may respond to such enquiries for participation in the Tenders.

CYCLE TIME FOR SUPPLIER REGISTRATION
On receipt of application/form at SPM, Hoshangabad, all the application/forms are opened on last week of
Monday every month and registration shall be completed within three months. Intimation to supplier
regarding their registration/ rejection shall be given within Ten working days of such approval. Rejected
applications can be considered only after three months from the date of rejection upon receipt of fresh
applications from the supplier.

Applicants need to comply with the requirements of the registration process. The information/ documents
furnished will be treated in strict confidence.

Instructions for filling the Registration Form

 All columns in the Registration Form are to be duly filled up. Indicate, ͚NIL͛/ ͚Not Applicable͛, wherever
details are not available or not relevant respectively.

 All sheets of the Registration Form are to be signed & stamped/ signed by the authorized
signatory.

 Expeditious furnishing of any clarification/ information required during registration to be ensured,
failure of which may affect the registration process.

 PO copies/ Experience certificates/ Test certificates or any other required document will be sent for
verification to the concerned issuing authority. Registration will be liable to be cancelled if any
document is found to be fake/ false/ forged on verification.

 Submission of fake/ false/ forged documents will invite action by SPM as per extant Guidelines of
SPMCIL.

 Just submission of supplier registration form does not mean automatic registration. Registration shall
be done, after due evaluation and will be intimated accordingly.

 For a document in language other than Hindi/ English, a self-attested Hindi/ English translated document to
be attached.

Based on scores in Vendor Registration Form, following registration criteria is
recommended:

VENDOR PERFORMANCE RATING (VPR)

Vendor Performance rating (VPR) shall be calculated taking into account Quality,
Delivery and Service factors for every consignment/ purchase order for continuance of
business.

1. Suggested evaluation methodology for quality, delivery and service factors is as below:

a. Quality Rating (QR):

Quality rating is based on acceptable quantity of material offered for inspection or
delivered by supplier and shall be calculated as:

b. Delivery Rating (DR):

Supplier will be rated on delivery parameters wherein Delivery rating shall be 30 in
case of adherence to PO delivery. For non-adherence to PO delivery (i.e. delay in
supply), marks to be deducted in proportion of delay to PO delivery.

Deduction = 30*delay in days/ PO delivery in days

DR = 30 – Deduction

Note:
- Delay is to be calculated as per PO delivery terms.
- In cases where delay is attributed on account of SPM, the revised delay shall be

calculated as per delivery extended in the amended PO.
- In cases where delay is attributed on account of supplier due to rework/

rectification/ replacement etc., the delivery is to be reckoned with from the
original delivery date.

c. Service Rating (SR):

The Service rating shall be based on the response of suppliers to SPM during and after
execution of supplies.

2. Demerit Factor:

In case negative feedback (substantiated by documentary evidence) is received
at a later date from shop/ site on the supplies made, demerit factor should be
applied again accordingly on QR, DR and SR.

3. Vendor Performance Rating (VPR) = QR + DR + SR

4. Material category-wise average VPR shall be calculated for the period of
previous year plus elapsed period of current year or of last three executed
Purchase orders, whichever is more.

In case no PO has been executed by the supplier, VPR will be taken as Not Assessed .
