Trident Area Agency on Aging is a not for profit, 501(c)3 organization that provides assistance, advocacy
and answers on aging. We are a private, non-governmental organization that receives grants and
donations to fund our services. The organization serves as the Aging and Disability Resource Center
creating a single, coordinated system for information and access for individuals seeking long-term care
by minimizing confusion, enhancing individual choice, and supporting informed decision-making.
It is the mission of Trident Area Agency on Aging is to promote and support the health and wellness of
current and future generations of older citizens in our communities. We focus on the overall well-being of
these older citizens so that they may enjoy lives of dignity, confidence, and personal and family fulfillment.
It is the vision of Trident Area Agency on Aging empower seniors, adults with disabilities and caregivers to
make informed decisions about their long-term care and to coordinate innovative, cost-effective and
quality home and community-based services for those with the greatest needs.

Eligibility Screening and Assessment Coordination Program provides a holistic approach for area
seniors in determining the level of need for Older Americans Act services.
Family Caregiver Support Program provides complete and current information about resources
available to assist caregivers. The program facilitates caregiver training, peer support and counseling
to help families cope with the emotional and physical stresses associated with caregiving. Support
services are available for caregivers to individuals 60 years of age and older, or individuals of any age
with Alzheimer’s disease or related disorders. Seniors who are raising children may be eligible for
support services through the Family Caregiver Support Program.
Information and Referral Assistance Program guides individuals through finding the services they
need, assisting them with long-term care planning and explaining eligibility for services.
Long Term Care Ombudsman Program advocates for residents in long term care facilities.
Ombudsmen investigate any complaint and advocate on the resident’s behalf to resolve the complaint to
the resident’s satisfaction. The Long Term Care Ombudsman Program trains and sponsors Volunteer
Ombudsmen who then visit facilities on a regular basis. They visit with residents and help educate the
residents and families on residents’ rights.
State Health Insurance Assistance Program provides health insurance counseling for Medicare,
Medicare Supplement, Medicare Savings Program, Medicare Advantage Plans and Senior Medicare
Fraud Awareness.
GetCareSC is an information and referral program for seniors and adults with disabilities living in South
Carolina. The web-based search tool can assist consumers, caregivers, family members and
professionals in identifying agencies and programs that offer the services needed in your community.

Adult Day Care Centers Adult Day Care Centers
A facility for the supervised care of older adults, providing activities such as meals and socialization,
one or more days a week during specified daytime hours. The participants, primarily persons with
physical and/or mental limitations who need socialization, physical assistance, and/or psychological
assistance, return to their homes each evening. The program is often used as respite by family
members caring for an older person who cannot be left alone safely in the home, and transportation
may be provided.
Adult Day Care Social provides programs and services focused on socialization for the senior.
The social program can reduce isolation and increase the self-esteem of the senior, while providing res-
pite to the caregiver.
Adult Day Care Health can provide therapeutic services. These health services may include;
counseling, health screenings, physical therapy, medication management and nursing care. These
programs are focused on seniors with either physical or mental disabilities.

Adult Protective Services Adult Protective Services
The Adult Protective Services (APS) Program is part of the Adult Advocacy Division of the South Carolina
Department of Social Services (DSS). The APS program has the responsibility to investigate and provide
protective services to a person eighteen (18) years of age or older who resides within the community
setting who has a physical or mental condition which prevents the person from providing for his or her own
care or protection. This includes adults who are impaired because of brain damage, advanced age, and
physical, mental or emotional dysfunction.
The mission of APS is to protect vulnerable adults from abuse, neglect, self-neglect and exploitation by
investigating and providing temporary assistance until risk is minimized and services are secured.
The intent of the APS program is to provide intervention and protection for vulnerable adults who cannot
provide care or protection for themselves in a manner that ensures attention will be given to the vulnerable
adult's right to self-determination, the client's lifestyle and culture, and the requirement that services be
provided in the least restrictive environment appropriate for the vulnerable adult as determined by the
vulnerable adult's needed level of care and safety.

Behavioral Health
Behavioral Health describes the connection between behaviors and the health and well-being of the body, Behavioral Health
mind and spirit. This would include how behaviors like eating habits, drinking or exercising impact physical
or mental health. Behavioral Health refers to mental health, psychiatric, marriage and family counseling
and addictions treatment, and it includes services provided by social workers, counselors, psychiatrists,
neurologists and physicians. Behavioral Health also includes both mental health and substance use,
encompassing a continuum of prevention, intervention, treatment and recovery support services.

Dialysis Clinics
Dialysis Clinics Dialysis is a procedure that is a substitute for many of the normal functions of the kidneys. Dialysis al-
lows people with kidney failure a chance to live productive lives. Dialysis helps the body by performing
the functions of failed kidneys. Doctors that specialize in diseases and conditions of the kidneys are
called nephrologists. There are two main types of dialysis.
Hemodialysis uses a machine and a filter to remove waste products and water from the blood.
Patients generally go to the dialysis unit three times weekly for treatment. On occasion, patients may
be able to perform dialysis treatment themselves at home in a process called home hemodialysis.
Peritoneal Dialysis uses a fluid that is placed into the patient’s abdominal cavity to remove waste
products and fluids from the body, this treatment requires the patient to have a more active role in their
dialysis treatment.

Make an emergency plan with neighbors, family, and caregivers to assist during an emergency,
review and update your plan on an annual basis.
Include your pet in your disaster plan, along with their medications, medical and ID records, food,
water, leash and crate. If you plan to evacuate, make plans to include your pets.
Be prepared to sustain yourself after an emergency for up to 72 hours.

Home Health (Medical)
Home health services are items and services furnished to an individual on a visiting basis and in a place of Home Health (Medical)
temporary or permanent residence used as the individual’s home. Home health services consists of part-
time or intermittent skilled nursing care as ordered by a physician and provided under the supervision of a
registered nurse and at least one other of the following services:
Physical, occupational, or speech therapy;
Medical social services, home health aide services and other therapeutic services;
Medical supplies and the use of medical appliances (suppliers can be found on www.Medicare.gov);
Any of the foregoing items and services which are provided on an outpatient basis under arrangements
made by the home health agency with a hospital, nursing facility, or rehabilitation center and the
furnishing of which involves the use of equipment of such a nature that the items and services cannot be
readily available to the individual in his home, or which are furnished at such facility while the patient is
there to receive such items or services, but not including transportation of the individual in connection
with any such items or services.

Hospice
Hospice Hospice services are provided to individuals living with a life-threatening illness and focuses on comfort and
support rather than the cure of the illness. Hospice services include nursing and home health aide visits,
necessary medical equipment and medications related to the life threatening illness. Hospice nurses, home
health aides, social workers, clergy and trained volunteers work as a team under the direction of the patient’s
physician to provide physical, spiritual, and emotional support. Hospice may be provided in a patient’s home,
assisted living facilities and nursing homes. Anyone-friends, family, clergy, as well as the patient’s physician
may refer an individual to hospice care. Hospice services are reimbursed by most insurance including Medi-
care and Medicaid. Most hospice agencies accept individuals regardless of their ability to pay for services.

Probate Court Probate Court
The Probate Court has jurisdiction over Marriage Licenses, Estates of Deceased Persons, Guardianships
and Conservatorships of Incapacitated Persons, Settlements for Minors and Wrongful Deaths, and
Involuntary Commitments of Mentally Ill and/or Chemically Dependent Persons.
Marriage License Division - we issue licenses to couples who wish to marry anywhere in the State of
South Carolina, as can occur in any of our 46 counties.
Our Guardianship/Conservatorship Division is two fold: 1) we appoint legal guardians for an adult who has
been declared incapacitated. The term incapacitated is applied to an individual who has been evaluated
and identified as unable to effectively receive, evaluate, and make/communicate decisions pertinent o the
individual's physician health or safety; also, the individual has been identified as unable to manage his/her
property or financial affairs, or provide support for him/herself, or his/her legal dependents. This does not
include the appointment of a Guardian ad Litem (or GAL). By Court Order, a guardian has the authority to
make decisions regarding the incapacitated individual's health, education, maintenance, and support.
2) we appoint a conservator to continually manage the estate (i.e. financial matters) of an incapacitated
individual, which includes minors.
The Estate Division gives qualified applicants official authority to manage the final personal and business
matters of a decedent. This court appointee, titled the Personal Representative of the Estate, shall
ultimately transfer all estate assets to the proper heirs or devisee so, either per Law or the decedent's Will,
which must be filed in this court. With this authority, the Personal Representative has the responsibility to
provide the court a written account of all that occurred in the management of the estate with the continual
assistance of court personnel - or Estate Clerks, who also maintain the estate files' upkeep. All files are
public record, and range in year from the 1700s to the present.
Problem-Solving Courts (Charleston County):
Mental Health Court links offenders who would ordinarily be prison-bound to long-term community-
based treatment. They rely on mental health assessments, individualized treatment plans, and ongoing
judicial monitoring to address both the mental health needs of offenders and public safety concerns of
communities.
Veterans Court is designed to assist justice-involved defendants with the complex treatment needs as-
sociated with substance abuse, mental health, and other issues unique to the traumatic experience of war.
They are principal to the veterans court team and the participants.
Drug Court takes a public health approach using a specialized model in which the judiciary,
prosecution, defense bar, probation, law enforcement, mental health, social service, and treatment
communities work together to help addicted offenders into long-term recovery.

Residential Care Facilities (Community Residential Care) Residential Care Facilities (Community Residential Care)
Community Residential Care Facilities (CRCF) are licensed and monitored by the South Carolina
Department of Health and Environmental Control’s Division of Health Licensing. CRCFs offer room and
board and which, unlike boarding homes, provide a degree of personal care for a period of time in
excess of 24 consecutive hours for two or more persons, 18 years old or older. CRCFs should be
designed to accommodate residents’ changing needs and preferences, maximize residents’ dignity,
autonomy, privacy, independence, and safety and encourage family and community involvement. All
facilities are licensed as community residential care facilities, but may be referred to as "assisted living
facilities." Every CRCF is routinely inspected and is required to post their Department of Health and
Environmental Control (DHEC) license in a conspicuous location in the facility.
The Optional State Supplemental Program (OSS) is for individuals residing in approved, licensed
Community Residential Care Facilities (CRCF) who meet Supplemental Security Income (SSI)
eligibility requirements, except for income.
A person who may qualify for the OSS program must:
Be at least 65 years old or blind or disabled
Be a resident of a facility that is enrolled with South Carolina Department of Health and Human
Services to participate in the OSS program
Be a resident of South Carolina
Be a U.S. citizen or Lawful Permanent Resident Alien
Monthly Net Income Limit may not exceed $1,456 per month. The individual’s resources must not
exceed $2,000. Individuals receive full Medicaid benefits. In addition, state-funded cash assistance
payment is provided to the facility.
Application can be made online at www/scdhhs.gov or at the local county office.

Every day senior citizens in South Carolina fall prey to unscrupulous individuals who seek to defraud and
scam the elderly. Consumer fraud is defined as any deception, pretense, false statement, false promise or
misrepresentation made by a seller or advertiser of merchandise. If you answer the phone and it sounds
too good to be true, just hang up. If a stranger appears on your doorstep, do not open the door. If
someone pretends to be a relative that is in trouble, simply hang up and check with your relative first. You
can protect yourself and others by reporting suspicious activities involving scams and frauds.
