AREA 6: EXTENSION AND COMMUNITY INVOLVEMENT RECOMMENDATIONS ACTION/S TAKEN PERCENTAGE OF COMPLIANCE 1. An evaluation tool may be developed for external evaluators for authentic feedback on the practices and programs of the department. The extension activities are monitored and evaluated thru participants feedback and Evaluation Form on Extension Services Program 100% 2. The program may consider involving all the faculty and students in their extension activities. There was collaboration among the students and faculty of TLE, Science and Mathematics programs that can be proven through project proposal, terminal report, progress report and Special Order issued by the EARIST President of the extension program/project conducted 100% 3. Proper dissemination may be considered to attract the participation of the community. This may be done through newsletters, radio, announcements, campaigns, pamphlets, etc. There was a proper dissemination of the extension activities thru communication, newsletter, brochures and other forms of social media. 100% 4. Inventory of supplies and materials for extension activities may be consistently conducted in preparation for future use and outsourcing if needed. Conducted an inventory of the supplies and materials for the future use in extension activities. 100% 5. Faculty and student extensionists may be encouraged to attend skills development training for more efficient and effective services. Faculty and students attended activities and training thru webinar and onsite. 100%

Recommendation 5 1. The Director together with Dr. Editha S. Magsino and Ms. Evelyn M. Polison and selected students attended the International Conference on Extension (IConEx) 2021 with the theme “Addressing Sustainable Development Goals towards Equity, Empowerment, and Gender Equality” held via Zoom last August 19-20, 2021.

Recommended the attendance of ten (10) Faculty Coordinators and three (3) Extension Staff in the 2021 TEIS Annual Conference on Extension entitled “SULO: Strengthening University Leadership in our Communities” last November 24, 2021 via Zoom, to wit: 1.
