This is the Seventh Edition of Perioperative Mortality ( POMR )
Conference.
This annual program was organized by Kementerian Kesihatan
Malaysia (KKM ) and The National POMR Committe of Malaysia
since its inception in 2014 with Melaka being given honours of
hosting this grand event in 2022.
This year we are welcoming more than 250 Medical Doctor and
Paramedics from whole nation.
We would like once again thank to KKM and The National POMR
Committee of Malaysia for giving us the opportunity to host this
national event here in Melaka.

Selamat datang diucapkan kepada semua delegasi ke 7th National POMR
Conference 2022.
Pertama sekali, dengan sukacitanya saya mengalu-alukan kedatangan anda
semua ke 7th National POMR Conference 2022. Selepas lebih dua tahun kita berada di
dalam Pandemik Covid-19, dan setelah dua tahun perancangan, akhirnya persidangan
kali ini dapat diadakan secara hybrid. Kajian Kematian Perioperatif atau Perioperative
Mortality Review (POMR) adalah satu aktiviti yang meneliti penjagaan pra-pembedahan,
intra-pembedahan dan pasca pembedahan pesakit di hospital-hospital Kementerian
Kesihatan Malaysia (KKM) dengan melihat kepada kadar pelaporan dan juga kadar
kematian pesakit yang menjalani pembedahan. Ini adalah objektif utama POMR di mana
ianya adalah amat mustahak bagi membolehkan penilaian menyeluruh kualiti
perkhidmatan pembedahan dan anestesia di hospital KKM dan seterusnya syor
penambahbaikan dapat dimajukan ke pengurusan KKM.
Saya menyifatkan POMR sebagai penghubung dan alat yang amat penting dalam
peningkatan kualiti perkhidmatan pembedahan dan anestetik di Malaysia. Walaupun
begitu, tidak semua negara menjalankan inisitif ini. Malah, dilaporkan bahawa sehingga
tahun 2019, hanya Malaysia dan Myanmar merupakan negara di rantau Asia Tenggara
yang memantau POMR. Walaupun ini adalah sesuatu yang harus kita banggakan, kita
hendaklah sentiasa berusaha untuk melakukan yang terbaik. Sehubungan itu, saya
berharap anda mengambil peluang sepenuhnya daripada Persidangan POMR ke-7 yang
akan berlangsung selama tiga hari ini untuk berkongsi pengalaman anda dan belajar
daripada satu sama lain dalam usaha peningkatan kualiti perkhidmatan pembedahan
dan anestesia di hospital KKM secara keseluruhannya.
Saya juga mengambil kesempatan mengucapkan terima kasih dan tahniah
kepada Unit Audit Klinikal, Bahagian Perkembangan Perubatan selaku sekretariat induk
POMR Kebangsaan, Jawatankuasa Saintifik Persidangan kali ini dan Kelab Perubatan
Ortopedik Hospital Melaka, selaku Jawatankuasa Pengajuran Tempatan dalam
menjadikan Persidangan kali ini sebagai sebuah realiti. Terima kasih juga kepada anda
semua, terutamanya rakan Assessors kerana sudi hadir dalam menjayakan 7th National

Dengan berbesar hati saya mengalu-alukan kedatangan anda ke 7th
National POMR Conference 2022.
Selaku Pengerusi Persidangan pada kali ini, saya mengucapkan setinggi-
tinggi terima kasih dan penghargaan kepada penceramah-penceramah yang
dihormati kerana sudi meluangkan masa dalam kesibukan tugasan harian untuk
berkongsi pengetahuan dan pengalaman mereka di Persidangan pada kali ini serta
ahli Jawatankuasa Saintifik dan ahli Jawatankuasa Penganjuran Tempatan bagi
kerja keras memastikan kejayaan pengajuran Persidangan POMR yang ke-7 ini.
Tidak dilupakan juga, terima kasih para Assessors di atas kesudian untuk turut
serta menjayakan Persidangan ini dan para delegasi sekalian.
Sebagaimana maklum, Persidangan POMR adalah merupakan persidangan
yang dianjurkan secara dwitahunan. Persidangan POMR yang terakhir, iaitu
Persidangan POMR yang ke-6, telah diadakan pada bulan April 2019 di Sabah di
mana ianya mendapat sambutan yang amat menggalakkan. Persidangan POMR
tersebut adalah merupakan persidangan yang terakhir sebelum kita dilanda wabak
COVID-19. Persidangan POMR ke-7 ini yang pada mulanya dijadualkan pada tahun
2021 terpaksa ditangguh akibat pandemik COVID-19. Walaupon begitu, saya amat
bersyukur kerana dalam kita berada di fasa peralihan endemik ini, Persidangan
POMR ke-7 ini akhirnya menjadi kenyataan.
Kajian Kematian Perioperatif atau Perioperative Mortality Review (POMR)
bertujuan untuk menilai kualiti perkhidmatan pembedahan dan anestesia di
Kementerian Kesihatan Malaysia (KKM). Persidangan POMR ke-7 pada kali ini
adalah merupakan platform untuk perwakilan daripada pelbagai latar belakang
yang berbeza - samada perkhidmatan berasaskan pembedahan, perkhidmatan
anestesia, pentadbir klinikal, jururawat dan juga paramedik, untuk berkongsi
pandangan, pengetahuan dan yang paling penting, pengalaman berkaitan
pembedahan dan perkhidmatan anestesia di hospital KKM.
Walaupun pengetahuan adalah sesuatu yang boleh diperoleh daripada
menonton dan membaca, namun tidak bagi pengalaman. Sehubungan itu, saya
berharap agar setiap delegasi menggunakan sepenuhnya tiga hari persidangan ini,
untuk mengenal antara satu sama lain, berkongsi pengalaman dan belajar
daripada satu sama lain. Semoga usaha kita ini akan dapat meningkatkan kualiti
perkhidmatan pembedahan dan anestesia di hospital KKM. Semoga kita beroleh

Selamat datang ke Melaka Bandaraya Bersejarah kepada semua peserta
Persidangan POMR Ke-7 Melaka 2022. Terlebih dahulu, saya ingin mengucapkan tahniah
dan syabas kepada semua Jawatankuasa Persidangan POMR Ke-7 Melaka 2022 yang
terlibat di atas usaha dan komitmen yang diberikan seterusnya berjaya melaksanakan
persidangan ini . Tidak lupa juga ucapan terima kasih kepada semua fasilitator dan
penceramah yang telah terlibat dalam menyampaikan ilmu kepada para peserta.
Seperti yang diketahui , pelaksanaan POMR yang berkesan memerlukan
komitmen daripada semua peringkat untuk meningkatkan kualiti
perkhidmatan pembedahan, anestetik, perkhidmatan sokongan dan logistik dalam

DATO’ DR. FITJERALD HENRY
Dato’ Dr. Fitjerald Henry did his MBBS in University Malaya in 1989
andsubsequently pursued Fellowship in Royal College of Surgeons in 1994 with
a special interest in Colorectal Surgery. He underwent training in Colorectal
Surgery at the Colorectal Unit, Singapore General Hospital in the year
2000.Dato’ Dr. Fitjerald is currently an active member of multiple colorectal
committees locally and internationally. He is a Senior Consultant Colorectal
Surgeon and the past Head of Department of Surgery, Hospital Selayang till 2019. Currently,
he is the Head of Colorectal Services Malaysia as well as the Chairman of the Colorectal
training Committee, Malaysia. He is also the Chairman of the National POMR Committee
Malaysia.
DR. ZALINA BINTI ABDUL RAZAK
Dr. Zalina received her Medical Degree from University Kebangsaan Malaysia in
1988 and than proceeded with her Masters in Anaesthesiology from the same
university in 1998. Dr. Zalina is part of many committees involved in the
development of the Anaesthesiology Training Programme in Malaysia. Dr
Zalinais also part of the Conjoint Board panel as well as an examiner for the
Anaesthesiology Programme. She was the Deputy Head of Service for 2 years
and subsequently became the Head of Service of Anaesthesiology since
September of 2021.
PROFESSOR DR. DAVID WATTERS
David Watters is the Past President of RACS (May 2015-May 2016) who since
2000 has been Professor of Surgery for Barwon Health in Geelong, initially with
the University of Melbourne (2000-2010), and then Deakin University (2011-).
He is a general surgeon with interests in general, colorectal and endocrine
surgery and the Chairman of the inaugural Victorian Perioperative Consultative Council (2019
-). He is actively engaged in advocating for global surgery, having spent almost 20 years in
developing countries including Papua New Guinea (Professor of Surgery, 1992-2000), Hong
Kong (1991), Zambia (1985-90) and South Africa (1982-84). He is an Edinburgh University
graduate, and in addition to the FRACS, he is a fellow of the Edinburgh, Hong Kong, and East
Central and Southern Africa Colleges of Surgeons, a Life Member of the Medical Society of
Papua New Guinea (2017) and Honorary Member of the Asian Surgical Association (2015).
Since leaving PNG he has returned on many occasions to teach, examine and support his
former trainees and colleagues in their leadership of the Department of Surgery. His research
interests include surgical audit and performance, surgical outcomes, perioperative mortality,
surgical history and global health.
He has over 180 peer reviewed publications, and he has published 6 books including
Stitches in Time - Two centuries of Surgery in Papua New Guinea (Xlibris, 2012) and Anzac
Surgeons of Gallipoli (RACS 2015). In recognition of his contribution to surgery and surgical
training in PNG he was awarded the OBE (2012). Within the Order of Australia he was
awarded the AM for significant service to Medicine and Medical Education and Leadership
Roles with Professional Organisations (2018 Queen’s Birthday Honours.) Deakin University has
appointed him the title of Alfred Deakin Professor (since August 2016). He is a Rotary Paul
Harris Fellow (Port Moresby, 2000), on the Editorial Board for the World Journal of Surgery,
and a Senior Editor for the Australian and New Zealand Journal of Surgery.

DR. NOR ZAILA BINTI ZAIDAN
Dr. Nor Zaila binti Zaidan is an Infectious Diseases Physician who is currently
practicing at the Melaka General Hospital. Prior to her medical degree, she did her
Bachelor’s in Science (Medical Sciences) and went on to get her Degree in Medicine
from University Putra Malaysia. Dr Nor Zaila than proceeded with Masters in Internal
Medicine from University Malaya. She served in several hospitals including Tawau
Hospital and Kajang Hospital before continuing her interest in Fellowship in Infectious Diseases in
2018. Dr. Nor Zaila was than posted to Melaka General Hospital in 2020 as the Infectious Diseases
Physician. She was appointed as the National Antimicrobial Resistance Committee member the same
year. In 2021, she was appointed as the Melaka State Infection Control Coordinator. At present, Dr.
Nor Zaila is heading the Antimicrobial Stewardship Committee in the Melaka General Hospital.
DR. HASMIZY BIN MUHAMMAD
Dr. Hasmizy Muhammad is a Consultant Cardiothoracic Anaesthesiologist and Head of
Department, Department of Anaesthesiology and Intensive Care, Sarawak Heart
Centre, Malaysia. He received his medical degree in 1998 and Master of Medicine
(Anaesthesiology) in 2006 from University Science Malaysia. He was trained in cardiac
anaesthesia and perfusion at Sarawak General Hospital in 2008 and Konkuk
University Medical Centre, Seoul, South Korea in 2011. He obtained the Graduate Diploma in
Clinical Ultrasound from the University of Melbourne in 2016 and gained experience in ECMO from
the University Hospital Regensburg (Germany) and Paediatric Cardiac Anaesthesia at the Royal
Brompton Hospital (UK). He is the Vice President of the Malaysian of Cardiothoracic Anaesthesiology
and Perfusion Society (MASCAP), a co-opted council member College of Anaesthesiologists, Academy
of Medicine, Malaysia and Chairman of Sarawak Perfusion Society.Dr Hasmizy is actively involved in
teaching and training in the fields of anaesthesiology, perfusion, and echocardiography. He is an
examiner for the degree of Master of Medicine (Anaesthesiology) conducted by Board of Examination
of Anaesthesiology & Intensive Care in Malaysia. His main fields of interest are echocardiography,
hemodynamic monitoring, and extracorporeal membrane oxygenation.
DATO’ DR. JIFFRE BIN DIN
Dato’ Dr. Jiffre bin Din is a senior consultant surgeon who is currently practicing in
the Ministry of Health (MOH) hospital in Kuantan, Malaysia. He is a graduate from
University Kebangsaan Malaysia, specialised as a true-blue General Surgeon, a dying
breed among the many new surgical based specialties. Despite his busy schedule with
general surgery services in public hospital, he is passionate in postgraduate training
motivated by his trainees’ difficulties in passing their exit examination. He is the MOH coordinator for
Master of Surgery programme and member for General Surgery specialty conjoint committee and a
member of examiners for Master of Surgery. He even has completed Master (Msc) in Medical
Education from University of South Wales, United Kingdom in 2020. He believes every doctor should
possess the essential competencies to become a competent trainer. He is one of the core-writer for the
‘Training the Trainer’ (TtT) guide for National Postgraduate Medical Curriculum and is a member of
the TtT steering committee. Currently, he is the MOH Head of General Surgical Service. Apart from
saving lives he has a keen interest in Malaysian history and saving many local arts
DR. FADZLINDA BINTI SHAHARUDDIN
Dr. Fadzlinda received her MBBS from the International Islamic University Malaysia in
2003. She was initially posted to Hospital Kajang and subsequently to Hospital
Serdang in 2006 till 2014. Dr. Fadzlinda than set foot into the administrative side in
2014 to the Medico-Legal Section under the Medical Practice Division of the Ministry of
Health. She is the organiser of Expert Witness Training and also a member of the Medico Legal
Society Malaysia as well as the Malaysian Medical Association. Dr. Fadzlinda is currently the Head of
Medical Malpractice Sector in the Medico-Legal Section of MOH.

.PROFESSOR DR. ART HIRANYAKAS
Prof. Art Hiranyakas graduated in 1997 from the Prince of Songkhla University. He was
than a General Surgery Resident from 1999 till 2002. Prof Hiranyakas pursued his
fellowship in Colorectal and Pelvic Floor Physiology at the National University of
Singapore. He pursued his training in Endoanal and Endorectal Ultrasound Training
in Italy in 2004. Prof. Hiranyakas is a person thirsty for knowledge. Subsequently, every year, he
pursued different types of trainings in Colorectal Surgery from around the world. Advanced
Colonoscopy Training in Japan, Advanced Laparoscopic Colorectal Surgery Training in Australia as
well as multiple trainings from the United States of America; Advanced training in Colorectal Surgery,
Transanal Endoscopic Microsurgery (TEM) Workshop, Echodefecography Workshop, Single-Port
Laparoscopy, Robotics and NOTES to name a few. He also had Robotic Colorectal Surgery Training in
Korea. Prof. Hiranyakas completed his Fellowship of the Society of Colon and Rectal Surgeons,
Thailand in 2017. in 2019, he also proceeded with his Doctor of Philosophy (PhD.), from Australia.
Prof. Hiranyakas is currently Consultant Colorectal Surgeon, Samitivej Sukhumvit Hospital,
Bangkok, Thailand since 2017. Not only is he a Chairman in multiple committees, but he is also
reviewer and supervisor in many Scientific Committees
DR. AHMAD KHAIRIL FAIZI BIN ROSLI
Dr. Faizi Rosli received his medical degree from Edinburgh. He also has a Bachelor in
Science from the same centre. Dr. Ahmad Khairil also did his law (Jurisprudence) at
the University Malaya and MSt Practical Ethics from Oxford. He is the Director of
Ethics and Legal Division of Malaysian Medical Council (MMC). Dr. Khairil has
extensive knowledge and experience in handling medicolegal and ethical related issues involving
health care providers in Malaysia. He has a strong understanding of current principles, procedures
and laws to determine and regulate the conduct and ethics of registered medical practitioner. Before
MMC, he was the Deputy Director and Head of Training Unit of Medicolegal Department, Ministry of
Health Malaysia. He has years of experience in handling ethical and medicolegal complaints against
healthcare providers and organizing medicolegal conferences and workshops for doctors, lawyers and
nurses around the country.
DATO’ DR. MOHAMED HASSAN BIN MOHAMED ARIFF
Dato‘ Dr. Mohamed Hassan was awarded the Colombo Plan Scholarship student from
the Australian Government to complete his MBBS at the Monash University, Australia.
He then went on to do Fellowship in Anaesthesia from the Royal College of Surgeons in
Ireland (FFARCSI). Dato’ Dr. Hassan was the Head of Anaesthesia, Universiti Sains
Malaysia (USM), Kubang Kerian, Kelantan. He did his subspecialty in cardiothoracic anaesthesia and
clinical perfusion at the Royal Prince Alfred Hospital, Sydney, Australia. He is also a member of many
fellowship programmes across the globe. He pursued his special interest in law and completed his
law degree from the University of London and proceeded with Master of Laws (LLM) (Healthcare and
Medical Law) from Taylors University. Dato’ Dr. Hassan also has a Certificate in Mediation from the
Bar Council Malaysia. He is currently a lecturer in medical law, UiTM for the Master of Medical
Ethics and Jurisprudence.
DATO’ DR. ZAKARIA BIN ZAHARI
Dato’ Dr. Zakaria is a Senior Consultant Paediatric Surgeon at Hospital Tunku Azizah
(Women and Children), Kuala Lumpur. He has just retired as the Head of Paediatric
Surgery, Hospital Tunku Azizah on the 30th October 2021. Dato’ Dr. Zakaria obtained
his medical degree under the Irish Medical Degrees programme which conferred 3
Degrees (MB, BCh, BAO) from the National University of Ireland (NUI) and Licentiates from the Royal
College of Surgeons, Ireland (LRCSI) and Royal College of Physicians in Ireland (LRCPI) in 1986. He
then went on to do his Fellowship in Royal College of Surgeons in 1991 and Fellowship in Paediatric
Surgery from the Royal Children’s Hospital, Brisbane Australia in 1995. At present, he is still a
member of the liver transplantation team of Hospital Selayang and Hospital Tunku Azizah since
2002.

DR. MOHAMMED AZMAN BIN MOHAMMAD RAFFIQ
Dr. Mohammed Azman received his MBBS from the International Islamic
University Malaysia and went on to do his Master of Surgery in Neurosurgery
from University Sains Malaysia. Dr. Azman did his Clinical Fellowship in Spine
Surgery AO/KOSC. He is also a Coordinator in the Masters of Neurosurgery
Academic Training & Examination Committee as well as Neurosurgery Masters Training &
Board Examination Committee. He is also an Associate Member Medical Specialty Cover for
Neurosurgery in the Federation of International Motorcycles (FIM) Motorsport, Malaysia.
Currently, Dr Azman is the Head of Department & Consultant Neurosurgeon at the Penang
General Hospital with a special interest in Vascular and Skull Base Neurosurgery as well as
Spine Surgery.
DR. AZMI BIN ALIAS
Dr. Azmi did his MBBS at University Malaya in the year 1989 and proceeded with
Master of Surgery in 1997 from University Kebangsaan Malaysia. Dr. Azmi also
has a Fellowship in Royal College of Surgeons, Ireland in 2003 and did his
subspecialty training in Neurosurgery in 2005. Apart from that, Dr. Azmi also has training in
Minimally Invasive Surgery from the University of Mainz, Germany in 2005 as well as training
in Paediatric Neurosurgery & Craniofacial Surgery from Women & Children Hospital, Adelaide,
South Australia in 2007. Dr. Azmi is a committee member in Safe Surgery Save Life (SSSL 2.0),
Ministry of Health, Malaysia. He is also an honorary lecturer and examiner for multiple
trainings in the field of Neurosurgery. Dr. Azmi is also actively involved internationally with the
World Federation of Neurosurgery as well as World Academy of Neurosurgery, to name a few.
Currently, Dr Azmi is the Head & Senior Consultant Neurosurgeon in Hospital Kuala Lumpur
and Head & Senior Consultant Neurosurgeon at the Hospital Tunku Azizah, (Women and
Children Hospital Kuala Lumpur).
DATO’ SERI DR. MOHAMED YUSOF BIN ABDUL WAHAB
Dato 'Seri Dr. Hj. Mohamed Yusof pursued his medical degree at Universiti Sains
Malaysia and graduated in 1987. He obtained his postgraduate degree (F.R.C.S.)
in April 1995 at The Royal College Of Surgeons Of Edinburgh. He was appointed
as the Head of the Surgery Department at Tengku Ampuan Rahimah Hospital (HTAR), Klang in
2002 until 2017. He was also the Head of the Selangor General Surgery Service from 2002 to
2017. He was the Head of the Malaysian General Surgery Service since 7 December 2016 until
just recently. He received the Mahkota Darjah Kebesaran Dato ’Paduka Mahkota Selangor
(D.P.M.S) award in December 2010 and the Darjah Gemilang Pangkuan Negeri (D.G.P.N.)
award on 14 July 2018.
DATUK DR. SOON RUEY
Datuk Dr. Soon Ruey graduated from Universiti Kebangsaan Malaysia in 1987. He
started his training in Obstetrics and Gynaecology in Hospital Kuala Lumpur in 1989
and passed his membership in 1994. In 1999, he was awarded a fellowship post in
Minimally Invasive Surgery at the KK Women’s and Children’s Hospital in Singapore. He
subsequently returned to develop Minimally Invasive Surgery in Hospital Kuala Lumpur, Hospital
University Kebangsaan Malaysia as well as for the Ministry of Health in Malaysia. Datuk Dr. Soon
Ruey has been actively involved in in many committees in the development of the O&G training in
Malaysia. He is also the first Malaysian to receive the Young Gynaecologist Award from the Asian
Federation if O&G in 2002. In 2002, he started work as Head of Department and Senior Consultant
in Hospital Queen Elizabeth in Sabah. The Department of O&G subsequently moved its current
premises at Hospital Likas, Sabah’s Women and Children’s Hospital in 2004. Datuk Dr. Soon Ruey is
also active in extracurricular activities. In 2013, together with a team of Divers, they created a
Guinness World record for the longest continuous Underwater Clean Up. In 2016, he received a
Federal award “Panglima Jasa Negara” which carries the title “Datuk”.

DR. MOHD YUSOF BIN ABDULLAH
Dr. Mohd Yusof currently practices at the Hospital Tunku Azizah and the Head of
Paediatric Surgery. He received his Medical Degree from the University
Kebangsaan Malaysia in 1997 and went on to proceed with his Masters in Surgery
from the same university in 2004. He served as a general surgeon at the Hospital Kuala
Lumpur and Hospital Kuala Terengganu. Dr Mohd Yusof subsequently continued to do his
fellowship in Paediatric Surgery under the Ministry of Health programme and also completed
his training in Pediatric Surgery, Burn and Trauma from the Red Cross War Memorial
Children’s Hospital, Capetown, South Africa. At present, Dr Mohd Yusof is the Head of Service
for Pediatric Surgery Malaysia with Special Interest in Pediatric Urology, Pediatric Burn and
Minimally Invasive Surgery. He is also in charge of Pediatric Urology Clinic in the Department
of Pediatric Surgery, Hospital Tunku Azizah, Kuala Lumpur.
DR. VICKNESH VISVALINGAM
Dr Vicknesh did his MBBS in1993 at the Madras University in Tamil Nadu, India.
Subsequently, he proceeded with Masters in OBS & GYN at the National University
of Malaysia in 2001. Dr. Vicknesh pursued his Fellowship in Gynaecological
Oncology in Malaysia and Singapore at the KK Women and Children Hospital, Singapore,
which is an accredited centre by the Royal Australian and New Zealand College of OBS & GYN.
In 2007, he was gazzetted as a Gynaecological Oncologist by the Ministry of Health, Malaysia.
He is currently the Head of the Gynaecological Oncology Unit, Department of OBS & GYN at
the Selayang Hospital which is one of the Referral Centre for the State of Selangor, Malaysia as
well as a training centre for Gynae-Oncology Fellowship Training. He is also a Committee
member as well as Advisor in Professional Organizations seeking to improve Gynaecological
Oncology Services in Malaysia. Not only that, Dr Vicknesh was also the Past President of the
Malaysian Gynaecological Cancer Society (MGCS).
DR. ANDRE DAS
Dr. Andre Das did his MBBS in University Malaya in in 1986 and subsequently
pursued Masters in Surgery in 1997 at the National University of Singapore. He
was the past Head of Department of Surgery and Senior Consultant Surgeon,
Kajang Hospital, Selangor of the Ministry of Health, Malaysia from October 1998 till 2019. Dr.
Andre is an Honorary Lecturer and Examiner at the Medical Faculty of University Putra
Malaysia since 2008 till present. He is also an Examiner for Conjoint Board for Masters in
Surgery Malaysia since the year 2010. Dr. Andre is currently an ATLS Course Director and
Trainer for Malaysia under the American College of Surgeons as well as an Instructor for
NOTSS (non operative technical surgical skills) course in Malaysia by the Royal College of
Surgeons Edinburgh.
DR. RASHIDE BIN YAACOB
Dr Rashide Yaaacob graduated from Universiti Kebangsaan Malaysia in 1995. He
than did his Master of Surgery in 2002 from the same university. Dr. Rashide was
as clinical specialist at Hospital Sungai Petani, Kedah from 2002 till 2007. He was
than posted to Hospital Sultan Abdul Halim , Sungai Petani in 2007. He was elected the Head
of Department of Surgery, Hospital Sultan Abdul Halim in 2011. Dr. Rashide has also been
contributed a lot to the Sungai Petani community. He spear heads some of the community
work in and around Sungai Petani. Not only is Surgery his passion but he also loves woodwork
and is capabale of making his own furniture. In 2021, Dr. Rashide was elected the Head of
Surgical service for the state of Kedah.

DR. MELOR BIN MOHD MANSOR
Dr. Melor received his MBBS degree in University Malaya in 1986 and
subsequently pursued Masters in Anaesthesia in 1995 from the same university.
He was the past Head of Service of Anaesthesia from 2018 till 2021. Dr. Melor
has 25 years of working experience as a Specialist in Anaesthesia and Intensive Care and has
worked in more than 5 major specialist / state hospitals in Malaysia. He is also a member of
the Conjoint Board between MOH and the Local Public Universities and a member of the Safe
Surgery Save Lives Steering Committee from the year 2020-2022. Dr. Melor is also currently
still an examiner for the Master of Anaesthesia Programme as well as an assessor for Parallel
Pathway Program (post FCAI).
DR. MOHD SANY BIN SHOIB
Dr. Mohd Sany was the Deputy Director (Medical) of Miri Hospital from 2003 till
2007. during the same time, He was also the chairman of Medical & Dental
Advisory Committee and Training & Professional Development Committee of Miri
Hospital. Head of Anaesthesiology & Intensive Care Department, Miri Hospital
(Sarawak Northern Zone Reference Center): Year 2003 – 2007. Dr Mohd Sany also
served in Ampang Hospital subsequently as Consultant Clinical
Anaesthesiologist. Currently, Dr. Mohd Sany is the Head of Anaesthesiology &
Intensive Care Department in Ampang Hospital since 2018.
DR. ISMAIL TAN BIN MOHD ALI TAN
Dr. Ismail Tan received is medical degree from Universiti Kebangsaan Malaysia in
the year 1999. He subsequently proceeded with Masters in Medicine
(Anaesthesiology) from the same centre in 2007. Dr. Ismail pursued his
Fellowship in Adult Intensive Care in 2011. He is currently a Consultant Intensivist attached
to Hospital Kuala Lumpur. Since the year 2012, he has special interest in CPR, Sedation and
resuscitation. Dr. Ismail is a Member of the Malaysian Society of Intensive Care as well as a
Member of the National committee for resuscitation training. He is also a member of the ALS
Subcommittee (NCORT KKM).
DR. FAIZAH BINTI MUHAMAD ZIN
Dr. Faizah received her Medical Degree from University Putra Malaysia in 2004
and obtained her Master (Science) in Community Health Medicine from the
National University of Malaysia. She has been working in the Ministry of Health
for the past 18 years. At the moment, she is the Head of Clinical Audit Unit in
Medical Care Quality Section of the Medical Development Division. The Unit is in charge of
Clinical Audit Activity of the Medical Programme, COVID-19 Mortality, Perioperative Mortality,
Surgical Site Infection as well as the Pain Free Programme.
SR. SERIPAH NOR BINTI MAT NOR
Sr. Seripah Nor did her Diploma in Nursing under Hospital Muar from 1994 till
1997. She was initially posted as a staff nurse to Hospital Kuala Pilah, Negeri
Sembilan till 2003. She did her Post Basic in Intensive care Nursing in 2002. Sr.
Seripah was than posted to Hospital Kajang and Hospital Serdang in 2003 and
2009 respectively. She was promoted from staff nurse to sister in the year 2011. After 24 years
in the clinical setting, she has now shifted her interest to the administrative side of Nursing.
Sr. Seripah is currently attached to the Clinical Audit Unit under the Medical Development
Division of the Ministry of Health.
