Participant Response/Alumni Tracking Contract
The goal of the Summer Health Professions Education Program at Columbia
University Irving Medical Center (CUIMC) is to help students enhance and
improve their chances of becoming successful applicants and students at the
health professions school of their choice. We take pride in our mission and in
order to assist us in helping you and others in the future, it is important that you
complete certain evaluations. The evaluations provide information on how we
can improve our program as well as how successful we are in aiding you
toward your goals. In addition, they help us track your career as it progresses
and possibly changes (not all of our students decide to make a final
commitment to medicine, dentistry, physical therapy or nursing and that is
okay!).
Please read the statement below and provide your name to signify that
you understand and consent to the below.
By confirming that I will be a participant of SHPEP at CUIMC, I hereby agree to
respond to and complete any and all evaluation requests, present and future,
made by Columbia University and/or the National Program Office of the
Summer Health Professions Education Program (SHPEP) by the deadline
presented. This includes, but is not limited to, program evaluations, class
evaluation, and alumni tracking surveys.
I also pledge to provide updates to the National Program Office of SHPEP and
the Columbia Vagelos College of Physicians and Surgeons Office of Diversity
and Multicultural Affairs if/when my contact information changes so that they
may remain in contact with me.

Course Description:
This course is designed to introduce students to the relationship between the form and function of the human
body. Topics covered will include the basic organization of the body and an introduction to the major body
systems. The course outline will consist of traditional lecture followed by a clinical correlation session in which
students will collaborate in groups to problem solve and reinforce the material presented in lecture. The
objective of the course is to synthesize a foundation of anatomy & physiology knowledge, as well as develop
practical clinical skills.
Course Requirements:
Graded weight Assignments

50% 2. Weekly quizzes:
• Must be submitted on Courseworks by 11:59PM on
Sunday.
20% 3. Group submission of Clinical Correlate problem set.
• Must be submitted on Courseworks by 11:59PM on
Sunday.
20% 4. Final assessment
