About the David T.K. Huang Memorial Scholarship David Ta-Keng Huang was a bright and gifted young scholar who died tragically at the age of 24, during the end of his second year of medical school at Columbia University. He is remembered by his family, colleagues, and peers for his academic excellence, keen interest in athletics, music, and literature, but most importantly, for his altruistic spirit among his local community, Washington Heights. David was the definition of a well-rounded individual who was committed to humanitarian efforts and community service. In remembrance of David, we are proud to offer the David Ta-Keng Huang Memorial Scholarship in the amount of a $1,500 stipend to a deserving and likeminded student. David T.K. Huang

Three-part Application Process: APPLICANT Submits complete scholarship application, including essay component and names of 2 Recommenders RECOMMENDERS Submit letters through Qualtrics portal using survey link GUIDANCE COUNSELOR Submits Nomination Form for up to 10 High School Seniors on track to graduate in 2023

Guidance CounselorNomination Forms • Submission ofstudents' names & emails • Limit: 10 nominees per Guidance Counselor ScholarshipApplication Form Checklist 1. Official Guidance Counselor Nominations Form – submit before or after the Scholarship Application has been submitted. 2. Official High School Transcript – submitthrough the Scholarship Application form. 3. FAFSA/SAR Form – submit through the Scholarship Application form. 4. Official Copy of ACT or SAT scores – optional; submit through the Scholarship Application form. 5. (2) Recommenders – submit the names and email addresses of (1) Teacher and (1) Community Service Personnel through the Scholarship Application form. 6. Academic Standing, Honors, and Awardssection – provide GPA, Class Rank, Honors/Awards and their corresponding dates. 7. School & Community Activitiessection – list any student involvement and community service experiences. 8. Employment/Professional Experience section – if applicable,submit the names of employers, provide the dates of employment, and describe the professional duties. 9. Essay Component – Upload one PDF that responds to both of the following questions (1/2 page,single-spaced for each question, no heading, write first and last name in PDF file) a) How will I use my talents to serve my community today and in the future? b) Please indicate any unique background, qualities, or atypical circumstances you would like the Committee to consider regarding your application. Recommendation Form • (2) Recommenders: (1) Teacher and (1) Community Service Personnel; Relatives are not qualified to provide recommendations. • Student'sGeneral Information – enterthe name and email address of the designated student through the form. • One-page, Typewritten Evaluation Letter – upload through form, and address the following four points (Letters must include organizational letterhead and recommender's signature): 1. How long and in what capacity have you known the student? 2. What are some of the student's greatest personal strengths? 3. How has the student made their community a better place? 4. Please write a brief statement about the student's potential as a college student.
