technician. I have had the pleasure of working alongside Mrs. Khlot for 2 years in the dental field and her
demeanor and bedside manor with patients was exceptional. Her personality was accepted by many
patients and she was very well liked by all those that sat in her dental chair.
Maria is reliable, hardworking and a team player. I feel that she will be a great asset to the lash industry,

To whom it may concern,
I have had the opportunity to receive eyelash extension services from Mrs. Maria Khlot. I was pleased
with her bedside manner and the way she explained the entire process to me. I had never had this type
of service in the past and she made it a comfortable and very relaxing experience. I was so pleased with
my results that I could not stop mentioning to her how beautiful my eyes looked. I couldn’t wait to come
see her again for a refill. Upon returning for a refill she maintained the same routine, being thorough
and gentle and still explaining what she was doing and why.
I feel she has gained a client for a life time, I know I will continue to see her for my routine eyelash
extension services. Anyone who adds her to their team is gaining someone who values the client.

Business Policy

The most important rule is NO TALKING during your appointment, please relax
and enjoy your lash nap!

All devices must be turned off during appointment time! This is a very crucial step
in us being able to service you properly and efficiently

Cancellation/No Show Policy: We require a 24-hour notice to CANCEL or
RESCHEDULE an appointment, a 50-dollar fee will incur, we will do our best to
notify you of any upcoming appointments via preferred method of contact. It is
very important for you to notify us of any changes in contact information. If an
appointment is confirmed and you simply NO SHOW, there will be a 50-dollar
charge. If you consistently NO SHOW to appointments MARIK LASHES reserves
the right to FIRE you as a client. This will be determined at MARIK LASHES
management discretion.

Late arrivals: We will allow a 15-minute grace period, if you are more than 15
minutes late please be aware that your appointment WILL be rescheduled.

Extra Guests: We respectfully ask that NO CHILDREN be brought to your
appointment, NO extra guests will be allowed in our lash rooms other than the
one being serviced, NO EXCEPTIONS

We will NOT provide refills to any work not done by MARIK LASHES; we will
recommend the complete removal of existing work before any service is provided
by a MARIK LASHES technician

If within 24 hours you are unhappy with your service, begin to have an allergic
reaction we will remove eyelash extensions FREE of charge. After 24 hours we will
charge appropriate fees.

There will be NO REFUNDS for any services

General Health and Safety recommendations

Eyelash extensions are a beauty service that at times cannot be provided for
everyone. If this is your first time receiving eyelash extensions please give us
some information about your health history. There are instances where we may
be unable to provide eyelash extension services the same day. For certain
health issues we may need a medical clearance from your MD.
I have/had the following (circle all that apply)

Auto-immunodeficiency
Chemotherapy
Radiation
Allergies to medical grade adhesive
Hair loss
Diseases of the skin around the eye area
Latex allergy
Sensitive skin

After Care

Now that you have received a full set of eyelash extensions please
follow these simple steps at home:

• DO NOT get eyelash extensions wet for the next 24 hours
(All water related activities can resume after this time)

• wash them with a MARIK LASHES recommended lash shampoo
daily

• Brush them with provided spoolie brush in order to keep them
from getting tangled

• DO NOT wear mascara, as MARIK LASHES doesn’t recommend the
use

• Please return every 3 weeks for a refill in order to maintain a full
look

If you notice swelling/itching around the eyes, please notify us
immediately as this may be a reaction that may require immediate
attention!

EYELASH EXTENSION APPLICATION

I acknowledge that if this is my FIRST time ever receiving EYELASH EXTENTIONS there is a

possibility of a reaction to the materials that are used. If during the time of the

application such reaction occurs, I will let my technician know immediately and

services will be stopped. The technician will do their best to address what could be

causing such reaction, however there is a possibility I could in fact not be a candidate

to receive EYELASH EXTENSIONS. If this is the case, there will be NO CHARGE for the

appointment. Initials

I am aware that as with any NEW SERVICE provided one may not have knowledge of

any known allergies or reactions until one tries it, therefore I release MARIK LASHES of

any liability for any issues that may arise DURING and AFTER my EYELASH EXTENSION

appointment. Should I have severe complications I am aware that I may have to seek

If After my EYELASH EXTENSION appointment, I am unhappy with my results I will let my

technician know and I will allow her to remove all extensions at no cost to me. Due to

time if I want the technician to re-apply a full set of EYELASH EXTENSIONS, I will have to

re-schedule for another day/time initials

If after 24 HOURS I decide that I am unhappy with my EYELASH EXTENSIONS I will notify

MARIK LASHES immediately and they will remove all extensions and a 25-dollar

REMOVAL FEE will be charged. Initials

During my consultation my MARIK LASHES technician will explain the growth process
and how this affects my EYELASH EXTENSIONS. My technician will discuss home care
and after care products that will help with the maintenance of my EYELASH EXTENSIONS.

I give my MARIK LASHES permission to apply a full set
of lashes, I have read and acknowledge that there could be
issues/circumstances that arise, however MARIK LASHES will do their best to give me
optimal care and results. I am aware that there a NO REFUNDS for any service provided

LASHES of any LIABILITY during or after my EYELASH APPICATION APPOINTMENT.

FAQ's

How long do eyelash extensions last? Indefinitely with proper maintenance and care.
Does the process hurt? There should be no discomfort during this procedure, please inform your technician of
anything that doesn’t feel normal the issue can be addressed immediately.
Will my extensions fall out? You may notice an eyelash extension fall off along with your natural lash, please
remember our natural lashes go through a growth cycle, therefore it is normal for shedding to happen.

Does it hurt to get them taken off? No, during the removal procedure we will do our best to make it as
comfortable as possible.
Can allergic reactions happen? Yes, unfortunately there could be a possibility that you may be allergic to the
materials we use the day of or even months after. We make sure to use products that are hypoallergenic on all
our clients.
Will my natural lashes be damaged? Our technicians are trained and certified to apply eyelash extensions in a
manner that will not hurt or damage the natural lash line.

How often will I have to return for touch ups? Our adhesives range from 2-4 weeks, if a sensitive adhesive was
used then you will be seeing your eyelash technician every two weeks. Typically we can see you back at week 3
but no later than week 4. By week 4 you will have lost about 50% of your lashes.
