Policies

Making sure that you and all other guests are provided with a quality experience which is both timely and
professional. To ensure reliable and consistent services for all guests, please be aware of the following booking and
appointment policies.

Appointments

Can be made online, by calling, text, social media, or email.

24 Hour Policy for Cancellations or Changes

All appointment confirmations are sent via text or email. Please honor your appointment. If any changes need to
be made to your appointment, please notify me at least 24 hours in advance. No advance notice will not be
charged however when you book your next appointment there will be a 30.00 deposit.

Please Arrive Early

Every appointment has an allotted time period reserved especially for that guest, I want you to get the most out
of your appointment time. If it is your first visit, please arrive at least 10 to 15 minutes before your appointment
start time in order to complete all necessary paperwork without cutting into your session time. For all returning
guests, please arrive at least 5 to 10 minutes before your appointment time to check in and be ready to go for your
full allotted session time.

Late Arrival

I ask that you arrive promptly for your appointments to ensure sufficient time for quality results. Arriving late
will cut into your session time, while there is no fee for late arrivals, the length of the appointment may be reduced
due to time restraints. If you arrive more than 15 minutes after your appointment time, your appointment will be
canceled, and the cancellation policy will take into effect. Out of respect for other guests, we cannot cut into another
guest’s appointment time to make up for your lost appointment minutes if you are late.

2 Person Max per Room

For purposes of safety and sanitation, I cannot allow anyone else into the service rooms other than (1) the

Lash technician and (2) the person receiving the service. Children under the age of 12 must be accompanied by
an adult who can supervise them in the waiting area while you receive your service.
Children are not allowed in the service room. There is no supervision for your children and cannot be held
responsible for any children, if you bring a child with you who is under the age of 12 and there is no one to
supervise them, your appointment will be canceled and the cancellation policy will take into effect.

Eyelash Services

Make-up removal: Please arrive without any eye makeup and do not curl your lashes on the day of your
appointment. If at all possible, avoid applying mascara on the day of your appointment. If you do have
mascara on, make sure to wash it off thoroughly before your appointment time, and do not use an oil-based
makeup remover, as leftover oil residue may result in premature shedding due to a weaker bond between the
extensions and your natural lashes. Do not use waterproof mascara or eyeliner. If I have to remove eye
makeup, and it may cut into your session time.

Contacts: If you wear contacts, please bring a contact case, as contacts should not be worn during the lash
application process.

Refills:

I do not perform refill services on work that is not my work. No expectations. If you currently have lash
extensions on, you must get them removed before receiving your first new set with me. This is to avoid the risk of
mixing different adhesives and materials from work done in other salons. If at any time you have less than half of
your eyelash extensions, the purchase of a full set will be required. Your natural lashes shed on a regular basis.
To maintain the look of your full set, re-lashes are recommended every two to three weeks

General Health & Safety Recommendations

Eyelash extensions are not for everyone: prior to application you should notify and discuss with
your certified eyelash extension technician if you have recently or frequently experienced any of
the following:

 Unusual sensitivity or skin reaction to cyanoacrylate-based adhesives.
 Moderate to severe allergies in combination with abnormal eye discharge.
 Any eye disease or medical condition, such as conjunctivitis “pinkeye”.
 A compromised immune system due to cancer treatment, hepatitis, or advanced AIDS.
 A recent bacterial or fungal infection in or around the eye area.
 Any type of cancer.
 Alopecia.
 Skin disease.
 Any metabolic or endocrine disorder.
 Blunt trauma in or around the eye area.

Uncured cyanoacrylate-based adhesives or removers should never be allowed to contact the eyelid or eyeball. Any
uncured cyanoacrylate-based adhesive or remover in contact with the eyelid or the eyeball may cause temporary or
permanent eye damage, including temporary or permanent loss of or blurred vision. It is very important that you
not open your eyes until all lashes have been applied and the cyanoacrylate-based adhesive is dry. For any
reason, if uncured cyanoacrylate adhesive or remover contacts the eye area, immediately flush with large amounts of
water and seek immediate medical attention.

Eyelash Extension Aftercare Information

The eyelash cycle is approximately four to twelve weeks. Do not be alarmed if some of your lashes fall out
quickly (they most likely were at the end of their cycle). The majority of extensions will stay through a complete

cycle if you follow these guidelines.

 The first 48 hours following your application are most crucial. Care must be taken, not to rub your
eyes, pull your lashes, or scratch.

 Do not use regular or waterproof mascara. Most brands of mascara contain chemical properties and
oils that will break down the adhesive. Removing mascara from your lashes can cause the extensions to
fall off prematurely as well. Also, any cleansers and cosmetics containing glycols and carbonate can
loosen the bond of the lash extensions. If you want to further enhance your lash extensions, we have a
specially formulated eyelash extension mascara that is ideal for coating your extensions.

 Take special care around the eye area when cleansing your face. Gently cleanse the eye area using the
Novalash’s Aftercare kit or diluted baby shampoo. Do not scrub or tug your eyes. Avoid any type of
oil-based product around the eyes. Again, the oils will break down the adhesive and cause the extensions
to fall off.

 Never use a clamp-style eyelash curler on your extensions. This can permanently damage not only the
extensions, but your natural eyelashes as well. Eyelash combs are great for grooming your lashes every
morning.

 Be sure to schedule to Re-Beautify appointment every 2 to 3 weeks for timely touch-ups. This will
keep your lashes looking full and natural, indefinitely.

 Do not attempt to remove the extensions yourself or do not pull on your lash extensions, it may cause
your natural lash to fall out. Have them removed by your professional eyelash stylist.

 I also retail an After-Care kit that Novalash created. Novalash's specially formulated mascara and
eye makeup remover contain no solvents, and will ensure that you are properly caring for your extensions
at home. With all the time and effort, you have invested into your lashes, you want to be sure that you're
not going home and dissolving the glue with your own products.

I have agreed to have eyelash extensions applied from my eyelashes. Before my qualified
professional eyelash technician can perform this procedure, I understand I must complete this
agreement and provide my consent by signing and dating this 2-page consent form where indicated
below.

I understand there are risks associated with having artificial eyelashes applied to and/or removed
from my natural eyelashes.
I understand that the eyelash extensions will be applied to the natural lash as determined by the
technician so as not to create excessive weight on the natural eyelash thereby preserving the health,
growth and natural look of the client’s natural eyelashes.
I understand as part of the procedure eye irritation, eye pain, eye itching, discomfort and in rare
cases eye infection may occur.
I understand and agree that if I experience any of these issues with my lashes that I will
contact my technician and have the eyelashes removed immediately and consult a physician at my own
expense.
I understand that even though the technician may apply and remove the eyelashes properly, that
adhesive materials may become dislodged during or after the procedure, which may irritate my eyes
or require further follow up care.
I understand and agree to follow the aftercare instructions provided by my technician. Failure to
follow the aftercare instructions can cause the eyelash extensions to fall out.
I understand that to have the eyelash extensions applied to my eyelashes I will need to keep my
eyes closed for duration of 60-100 minutes during the procedure. I also understand that I will
need to be lying in a reclined position. Any medical conditions that might be aggravated by lying
still for a prolonged period may mean I will not be able to have the procedure performed on my
eyes.
This agreement will remain in effect for the procedure and all future procedures conducted by my
technician for one year from the date of this signed form. I understand that this agreement is
binding and that I have read and fully understand all information listed above. I represent that
I am over the age of 18 years. If below 18 years of age a parent or guardian must also sign
this form.
