A Non-Profit 501(c)(3) Member Share Health Plan

• To be a member, you must want to be part of a group like-minded people who want to be a part of a
community that helps other people share in their health care expenses.

• To be a member, you only use alcohol in moderation and do not put yourself or others at risk if
intoxicated.

• To be a member, you must care about others and their entire well-being: physical, spiritual, emotional,
and financial.

• To be a member, you are willing to learn alternate forms to access the care they need outside of
traditional methodologies in a way that keeps sharing costs low for all members.

• To be a member, you either have a U.S. mailing address or consistent, reliable, secure Internet service
through which you can receive documents with confidential information (via attachment through an
active email address, drop box/FTP capability, etc.) See Guideline C.5 for additional Guidelines for
missionaries.

WHAT IS ASH?

Alliance for Shared Health (ASH) is a non-profit 501(c)(3) program designed to provide a member sharing
program for access to specific health care needs. Members share in Medical needs per the ASH guidelines. A
health shared plan is “not insurance”, therefore, terminology and regulation are not the same as traditional
insurance products. ASH collects members monthly contribution amounts and pays the Providers, minus any
applicable “Member Responsibility Amounts” (in traditional insurance, this would be called a “copay”
amounts).

Members voluntarily submit contributions to the program on a monthly basis in order to maintain eligibility
for sharing of medical needs, and for the funds they submit to help share in the needs of others per these
guidelines. Alliance for Shared Health acts as a neutral third party to facilitate the need request payments, and
may sue vendors, at its discretion, to strengthen and support member benefits.

WHAT ARE THE ADVANTAGES AND LIMITATIONS?

A Medical Cost Health Sharing Plan is not for everyone. Members are guaranteed to be accepted when they
apply for the MedWise Medical Sharing Plan. The plan is designed to pay for many outpatient benefits that are
paid after a minimum,” Member Responsibility Amount” like a co-pay in traditional insurance. There is a pre-
existing waiting period on in-hospital medical expenses before claims are paid for any individuals that were
treated for a medical condition in the last 12 months prior to enrollment in the plan. Please refer to the “ASH
Member Guidelines” for more details.

The MedWise Sharing Plan benefits are designed to provide an individual with an affordable health protection
alternative. Benefits pay for the most basic preventive and wellness coverage and provide in-hospital
catastrophic coverage up to a maximum annual and lifetime limit that you choose. If an individual does have a
pre-existing condition that requires immediate attention or treatment, this plan may not be for you. Benefits
in the MedWise Health Sharing Plan are not unlimited, but the MedWise Share Plan does offer families a range
of affordable health care options that may be less comprehensive than traditional major medical plans but
may fit the family’s needs and budget better.
