For more than a century, Community First has grown
exponentially, pioneering new and diverse programs to support
the evolving needs of our friends and neighbors. Our collective
impact permeates thousands of clients, dozens of programs, and

At the beginning of the 20th century, visionary local leaders recognized the need for “a home to which people
might go who were no longer able to work… of which they could be such a vital part that they could still maintain
their independence.” During an era when older adults were confined to poverty, almshouses and sanitariums, it
was a nationally innovative concept.

“Hamilton’s ‘Wonder Spot’ is the new Community Home for the Aged located at 315 Dayton Street. It is the most
wonderful home in this part of the entire country. Everything possible to make the place homelike for the many
residents has been done, and in fact the impossible seems to have been accomplished…”

For more than a century we have built upon this mission - developing life plan communities that foster vitality and
independence for residents of all levels of care. Through the years, we have also expanded beyond our residential
campuses to support older adults throughout our region in maintaining independent, active, healthy lives.

Our roots run deep, through our commitment to the older adults we serve. Today, Community First Solutions is a
not-for-profit network based out of Butler County, Ohio that provides a broad continuum of high quality services
to meet the diverse health and wellness needs of more than 30,000 individuals throughout the region.

We are an ecumenical system, and as a not-for-profit network we share similar convictions as faith-based providers.
Commitment to our mission drives our organizational priorities, and we reinvest into expanding programs and

A Rich History

Established in 1918, Community First Solutions is a not-for-profit organization that touches the lives of more
than 30,000 Butler County residents annually. We operate two highly-rated Life Plan Communities, are one
of the leading providers of mental health and behavioral services in the region, the largest provider of home
delivered meals in Butler County, the founder of the second not-for-profit pharmacy network in the nation, and
one of the largest employers in the city of Hamilton.

1918 | The single gift of a Dayton Street home established Colonial, a leading provider
of older adult and multi-generational services

1954 | The “You’re As Young As You Think” group is incorporated, establishing
Partners In Prime as the second senior center in the state of Ohio

1964 | A five acre tract of land on the west side of Hamilton was purchased, the site
of a new residence that would be named Westover Retirement Community

1974 |Community Behavioral Health, known first as Buckeye Center opened
a freestanding alcohol inpatient detoxification & rehabilitation program

1984| Colonial School opened at Westover, becoming one of the first preschools in the
nation to embrace intergenerational learning

1989| Berkeley Square Retirement Community opened with 27 Independent
Living Homes

2008 | Community First Pharmacy opened, establishing the 2nd full service retail

Commitment To Growth

Community First has been a recognized leader in Southwestern Ohio for more than a century. However, we know
that in order to sustain and expand our mission, we must pursue opportunities for growth beyond our region.
This could be accomplished by developing new communities or by establishing collaborative partnerships
with existing providers. From our perspective, it makes more sense to band together and strengthen existing
organizations rather than adding more competition into markets.

Throughout the past decade, we have been dedicated to expanding our mission in our existing market area.
When the Community First Solutions parent company unified our not-for-profit network in 2011, our Board of
Directors felt strongly that growth through collaboration and partnerships was a critical component to continuing
to expand our mission and meet the dynamic needs of our those we
serve.

ago our President & CEO of 40 years retired and our Board of Directors led an exhaustive nation-wide search for
a new leader. This was a critical juncture for our organization, and we understand how challenging and uncertain

expenses - to ensure the continued success of the organization with proven leadership.


Healthcare Leadership Expertise

Our leadership experience in both the acute, post-acute/skilled and home and community based service industries
provides valuable insight and support for long-term care providers increasingly reliant on acute care referral
relationships and the movement to value based care in a dynamic healthcare environment that may require expansion
of their continuum of services.

BRETT KIRKPATRICK President & Chief Executive Officer
Brett is a proven leader with more than 20 years of progressive experience in strategic planning,
innovative program expansion, and leadership of operations throughout the healthcare continuum. He
is passionate about developing strong cultures that promote quality, growth and successful outcomes.

Prior to joining Community First, Brett held leadership roles with the TriHealth system, serving most
recently as Senior Executive for TriHealth’s McCullough-Hyde Memorial Hospital and Bethesda Butler
Hospital. Mr. Kirkpatrick successfully integrated McCullough-Hyde Hospital into the TriHealth Health System.
McCullough-Hyde Hospital operated as an independent hospital for over 60 years prior to joining TriHealth, and this
independence was deeply rooted within their heritage. The key to this integration was creating a compelling vision
of the future and engaging the physicians, team and community. Mr. Kirkpatrick managed this while undertaking
an extensive capital-repositioning project. Community confidence grew as did engagement, physician engagement
in the top decile and team member in the top quartile nationally. Following the integration, Mr. Kirkpatrick began
operational and strategic oversight of a second hospital with that market, integrating both hospitals under a common
strategy that leveraged their strengths and minimized duplication to increase efficiency while meeting the needs of
the community.

In his tenure with TriHealth, Brett also led the Senior Services division and later became the Executive Director of
Ambulatory Services before assisting with the integration of TriHealth’s Physician Partner network. Brett served as a
Nursing Home Administrator for both Deaconess Long Term Care and HCR ManorCare prior to joining TriHealth.

Brett is an active community leader serving on the Greater Hamilton Chamber of Commerce Board of Trustees,
Hamilton Vision Commission Advisory Board and the Rotary Club of Hamilton. He holds an M.S. in Organizational
Leadership from Mount Saint Joseph University and a B.S. in Health Services Administration from University of
Cincinnati.

BRIAN KRAUSE Chief Financial Officer
With more than 20 years of progressive advancement as a strategic financial leader, Brian brings
considerable experience to support the financial leadership of our growing organization. Brian has
expertise in all aspects of healthcare finance, including forecasting, budgeting, revenue cycle, and debt/
capital planning.

Brian began his career with TriHealth holding several key leadership positions during his tenure including
Financial Analyst, Controller and Vice President of Finance for the health system. Prior to joining
Community First, Brian served as Chief Financial Officer for S&S Strategies, a Cincinnati based healthcare organization.

Brian has extensive experience in optimizing partnerships under many different structures. At TriHealth, he successfully
integrated the financial operations of 3 hospital acquisitions, and numerous physician practice acquisitions as the
organization added over 500 employed physicians in a short period of time. In addition, Mr. Krause played a lead
role in developing the finance support model for a joint venture cardiac cath lab, a joint venture radiation oncology
business, and was the Board Chair for a best-in-class healthcare laundry cooperative owned by 3 separate heath
systems. The successful integration of these various partnership models was predicated on leveraging the scale of
the combined operations while also determining which functions were best deployed at the local or site level.

Brian earned a Bachelor of Health Sciences degree and an MBA in Finance from the University of Kentucky.

Below are a just a few examples of how we have created unique opportunities for innovation in our health
services highlights:

• Our health services team is nearly 100% paperless. We are using technology to better monitor and report
our clinical outcomes and quality measures in real time. We were recently selected to pilot a new technology
with TriHealth to integrate referrals directly into our electronic medical records system.

• Established Service Excellence Program to round on patients throughout their stay to address any concerns
and ensure high resident satisfaction.

• As a result of our high quality of care, we have been selected by Mercy Health to pilot a Congestive Heart
Failure program to ensure a more positive outcome for patients and reduce readmissions.

• Several years ago we were among the first to pilot a Chronic Joint Replacement program with Kettering
Health Network.

• To support our recruitment and retention initiatives, we developed an in-house STNA Program and have
taught 11 classes with a total of 58 students. This is also a testament to our healthcare leadership expertise
and quality as we must maintain outstanding surveys to continue to provide this program.

ACROSS OUR BROAD RANGE OF SERVICES, OUR TEAMS CONTINUE TO ACHIEVE AND MAINTAIN HIGH QUALITY
INDICATORS FROM ANNUAL SURVEYS, INDUSTRY RATINGS AND ACCREDITATIONS. HERE ARE SOME OF THE
RECENT HIGHLIGHTS FROM OUR HEALTH SERVICES TEAM:

• We continue to maintain 5 Star Centers for Medicare & Medicaid Services (CMS) rating across all three of our
Berkeley Square, Westover and Jamestowne campuses.

• Our Jamestowne, Berkeley Square and Westover campuses have acheived preferred provider status by several
regional acute-care hospital systems including TriHealth, Kettering Health Network, and Mercy Health.

• Because Jamestowne has consistently exceeded quality criteria, we have been designated a “Floor To SNF”
provider by Anthem. This is important because we will now be able to bypass the insurance pre-certification
process, thereby reducing hospital days for the patient and allowing us to accept referrals more quickly.
Both Berkeley Square and Westover are also on the Anthem Floor To SNF provider lists.

• Our Health Services Team successfully implemented CMS’s new Patient Driven Payment Model (PDPM), the
first major change to the SNF Medicare reimbursement model in more than 20 years. Due to our proactive
preparation, our organization has achieved increased revenues consistently since implementation.

• As evidence of our commitment to quality, we developed the position of Assistant Vice President (AVP) of
Quality and Regulatory Compliances to oversee quality assurance and alignment across all three campuses.
This is an example of domain expertise.

Navigating COVID-19

Before COVID-19 made it’s way to Ohio, Community First established a cross-functional task
force to ensure that should the threat spread across the country, we would be well prepared
to protect our residents, clients and staff. Led by our AVP of Quality and Certified Infection
Preventionist, we planned, prepared, readied, and trained across the organization. These
early efforts - and the commitment of our staff on the front lines - have kept COVID-19 out
of our Berkeley Square and Westover communities, and saved countless lives throughout this
pandemic.

The Ohio Department of Health has conducted 5 infection prevention surveys across our three
campuses to date. Not only have we been consistently deficiency free, the ODH surveyor stated 'we
had the most thorough policies and procedures she has seen in any facilities.'

CFS secured a $500,000 gift from The Joseph L and Sarah S Marcum Foundation to support Superhero pay increasing
hourly wages for front line staff, reimburse any staff for medical costs resulting from COVID-19, and secure adequate
PPE for the organization.

Our two life plan communities, Berkeley Square and Westover, provide high quality care at any level. From active
Independent Homes, supportive Assisted Living Apartments, top-rated short-term skilled and long term health

BERKELEY SQUARE
Berkeley Square is located on a 90 acre parcel of land in Hamilton, Ohio and has operated since initial construction
in 1989. Berkeley Square has a total of 133 cottage homes and 18 indepenedent living apartments in one-, two-
and three- bedroom floor plans ranging from 995 sq. ft. to 3,158 sq. ft. Our newest development, Harmony
Drive, will include an additional 14 independent homes. In addition, Berkeley Square features 45 assisted living
apartments and 53 short & long term skilled nursing beds.

WESTOVER
Westover is located on a 17 acre parcel of land in Hamilton, Ohio and has operated since initial construction in
1964. Westover has a total of 33 one and two bedroom independent living homes and apartments ranging from
770 - 1650 sq. ft., 45 assisted living apartments and 53 short & long term skilled nursing beds.

Inpatient & Outpatient Therapy

Opened in February 2017, Jamestowne features 42 private suites
and a 5000 sq. ft. therapy gym. By blending the luxury of five-

partnership affords our clients the full scope of rehabilitation and health services with the highest clinical
expertise.

COLONIAL AT HOME

Colonial at Home is a non-clinical private pay home care service that provides an array of support to assist older

As the second nonprofit retail pharmacy network in the nation, Community First Pharmacy offers personalized
service and affordable pricing to our community. We serve individuals with traditional insurance and uninsured
and underinsured individuals.

• Institutional: we provide skilled care pharmacy services through LifeLine Rx and fill health care
prescriptions for assisted living and skilled nursing residents at Berkeley Square and Westover.

• Retail: We function as a walk-in service, retail pharmacy.
• Accept 340B Drug Discount Program/Vouchers
• We currently fill over 80,000 prescriptions annually
• 340B partnership with a local Federally Qualified Health Clinic

ELEMENTS WELLNESS CENTER

Our Berkeley Square and Westover locations are home to Elements Wellness Center, a specialized fitness center
serving older adults from the greater Hamilton Community, as well as residents. Elements offers state-of-the-
art fitness equipment, an aerobic studio, rejuvenating massage services, salt water conversion pool, whirlpool,
sauna, and specialized fitness classes for adults fifty and over.

Founded in 1954, Partners in Prime has rich tradition of serving older adults. Today we are among the largest
providers of Meals-on-Wheels and transportation in Butler County, and have expanded our services to support

FOUNDATIONS COUNSELING July 2020

Formerly Cornerstone Counseling, a boutique for-profit mental health provider. Long time leader John Guliano
approached Community First about partnership after our merger with LifeSpan.

Key Drivers
• Foundations Counseling wanted the support of Community First’s shared service model to help expand their

business
• Community First’s ability to assist Foundations Counseling with their growth strategy
• Foundations allowed Community First to expand our existing behavioral health services to a new market

demographic

Terms – Dissolution of for-profit Cornerstone Counseling, CFS becomes records custodian, Cornerstone Counseling
Owner and staff become part of CFS under the newly formed Foundations Counseling.

When LifeSpan’s board began planning for the retirement of longtime President & CEO, they recognized the
opportunity to strengthen the mission of the organization and gain savings and efficiences by joining Community
First’s not-for-profit network.

Terms – Merger of LifeSpan into the CFS Family | LifeSpan Board dissolved but CFS added LifeSpan representation

BRIDGES REHAB AT HOME September 2014

Recognizing the need to expand our healthcare continuum to include skilled home health, Community First
formalized a partnership with Alternate Solutions and Kettering Health Network to establish Bridges Rehab at
Home.

Key Drivers
• Addresses dynamic ACO climate and broaden our market through extending our

scope of services
• Provides individuals in Butler County with a stronger continuum of quality

healthcare services from providers working together to ensure they move
seamlessly through their rehabilitation and recovery process
• Enhances relationships with local acute care hospital systems

Terms – each partner splits the profit equivalent to their initial capital investment

Community First was a supportive sponsor of Partners in Prime as our missions of serving older
adults throughout the Butler County community were well aligned. As the organization struggled with decreased
reimbursement in an increasingly competetive market, their board initiated conversations with Community First
Soltuions to gain the support of our shared service infrastructure, acheiving economies of scale to sustain their
mission.

Key Drivers
• Combine and expand complementary community based services including Meals on Wheels, wellness, and

recreational programming
• Sustain services for older adults in Butler County and extend regional reach
• Gain efficiences and expertise from CFS shared services infrastructure
• Achieve economies of scale by consolidating services
• Expand client support through enhanced programs and services

Terms – Merger of Partners into the CFS Family | Partners Board dissolved but expanded to CFS to add designated
Partners Board members.

In unifying both organization’s Meals on Wheels program, we were able to
secure better rates from our funding sources and outbid for-profit providers in
our region, securing the sustainability of our Meals on Wheels program and the
highest quality product for our clients. Today we are the largest provider of
Meals on Wheels in our region, delivering more than 2,600 meals each week.

Financial Performance

fiscal management,cost effective operations, and long term
planning to ensure we have the capital and operating funds
needed to sustain our mission.

Unlike many providers we have a strong balance sheet and
very little debt. Our dept capacity and strong operating
performance allows for access to capital that will support
growth for a successful future.
