BELL SCHEDULES OUTLINED

Note: The first day after Progress reports go out will be FLEX SCHEDULE DAY. This will allow for a 75-
minute block of FLEX time. During this time, we will provide a Lock-In for all students passing all subjects
who have returned their progress reports signed by their parents. All other students will be assigned by
the team leader to core academic classes to complete 75 minutes of tutorial time. Our goal is to make this
an incentive for students to want to pass at the mid-point of the six weeks in order to attend the lock-in. It
will also provide individual one-on-one help for students who are failing at the mid-point of and thus
increase their chances of passing for the six weeks. During the Lock-In, a variety of activities will be
planned to include music, refreshments, and social time. Each Lock-In will be different.

Students who stay organized and plan ahead do better in school. Start the year off right by using this agenda to stay
organized. You can do many things with an agenda planner. Here are some suggestions:
Write your daily homework assignments and check them in the evening. Make a list of due dates for upcoming
projects, quizzes and tests. Keep up with when the next school holiday is scheduled. Make a list of events you want
to attend like sports games, meetings and scheduled competitions and performances. Many students like to fold
down the page after the day is done for quick reference to the next day. This can also help you keep up with how
many school days you have left in the year. It matters not how you choose to use this agenda as long as you choose
to use it as a planning tool to make you a better student. Have fun planning and refer to the list below for special
incentive days for all students who have their planner up to date in class.

SCHOOL NURSE INFORMATION

GENERAL POLICIES

Immunization Updates:

All incoming 7th graders must have their immunization record updated with the following
immunizations before they can attend class: Tdap-within the last 5 years, Varicella #2 or
documented previous illness, Meningococcal (MCV4). All students enrolled in TISD must be
in-compliance with the state law regarding their immunization status. Students will be sent
home from school if their immunizations are not current. Please call me if you have any
questions on your student’s immunization record.

Medications or Special Procedures at School:

Students are not allowed to carry any medication with them at school. However, asthma inhalers or
epi-pens-may be allowed only IF self- administration form is filled out by the physician and the parent.
If your student needs to take medication or have a special procedure done during the school day- please
contact the school nurse. A parent permission slip must be on file in the nurse’s office. Non-
prescription medications and special procedures require a physician signature on the parent
permission form per TISD policy. The nurses clinic does not have any over the counter medication
to dispense to students unless a doctor’s order is written.

• Only authorized employees, in accordance with TISD policies, may administer:
o Prescription medication, in the original, properly labeled container, provided by the
parent, along with a signed parent permission slip.
o Prescription medication from a properly labeled unit dosage container filled by a
registered nurse or another qualified district employee from the original, properly labeled
container.
o NON prescription medication, in the original, properly labeled container, provided by
the parent along with a parent medication permission slip signed by the parent and
physician.

SCREENINGS

Vision and Hearing:

All 7th graders will have a vision and hearing screening done at the beginning of the school year
during their A1 classes.

All new students to TISD will have a vision and hearing screening done within 120 days of
enrollment.

Spinal Screening:

All 6th and any new students to TISD will have a state mandated spinal screening done for
scoliosis, or if requested by parent.

Parent or guardian will be notified of abnormal finding from vision, hearing, and spinal
screenings.

Referrals:

If a referral is mailed home for any of these screenings, please have your student seen by a
physician at the earliest possible date. Please take the referral form with you to the physician and
have him complete it. Please return the referral form to the nurse’s office, so that the referral
process can be completed. If finances are a problem, please call or see the school nurse.

Clinic Passes:

Students must have a pass to see the nurse; the only exception are students who come for routine
medications daily.

Accidents or Injuries at School:

If your student is injured at school, please remind them to see the school nurse. If your student is
injured in Athletics, then the appropriate coach must be notified. The coach will inform you of
the proper procedure to follow.
Head lice guidelines and information can be found on the Tyler ISD District home page web site
under parents/students and drop down menu to Health Services.

Illnesses:
Parents/Guardians are encouraged to observe their student for illness and symptoms of
contagious diseases before sending them to school. Students need to stay home until they are
fever free for 24 hours, without medication before returning to school. Students will be sent
home from school if they have a temperature of 100 degrees or above.
Important: If students call or text their parents, family members, or guardian during school
hours because of illness or medication without seeing the school nurse first the absence will not
be excused.
Communicable Disease Transmission:
Communicable disease is one of the most common causes of absenteeism. Diseases can be
transmitted by one or more of the body secretions, such as; blood, feces, urine, vomit or droplet
spray from sneezing/coughing. Please remind your students to wash their hands frequently and
not to share food, drink, make-up or hair care items with other students.

HOGG MIDDLE SCHOOL FIELD TRIP PERMIT

I agree to allow my son/daughter ________________________ to participate in
school-sponsored trips for the 2020-2021 school year.

The group will travel by district approved transportation. Normal precautions will
be taken in the interest of safety and well being. It is understood however, that
neither the Tyler Independent School District nor any employee or sponsor thereof
will be held liable for any accident, injury or illness that might occur.

The privilege to go on these trips carries with it the obligation for a student to
conduct him/herself in such a way that credit will be reflected upon the student, the
school and the home, which is represented. Going is a privilege and may be
refused to any student at the teacher’s discretion. Correct dress and behavior for
the particular trip will be observed.

In case of an accident or sudden illness I authorize a representative of the Tyler
Independent School District to refer the above named child to available medical
service.

I understand that the school will send out individual field trip permits throughout
the year. In the event that a field trip permit is not completed on my child, I give
the school permission to utilize this generic form signed on this date.
