Every situation is unique. In order to provide the best possible services, a
healthy living coordinator works with each resident to complete an initial,
comprehensive assessment. This will allow you and your healthy living
coordinator to put together the best possible plan for care, identify wellness
opportunities and set goals for the future.
Healthy living coordinators also offer accessibility and availability to residents
and their families after hours, and on weekends and holidays.

Healthy Living Coordination Services:

• Develop short and long term healthy living care plans
• Coordinate medical appointments and medical information
• Communicate healthcare services between residents, family and providers
• Evaluate in-home care needs and provide care personnel
• Assist with the monitoring of medications
• Provide transportation and accompany resident to medical appointments
• Liaise with families living at a distance, overseeing care and quickly alerting

families to potential problems
• Emergency crisis management, intervention and support
• Conduct ongoing assessments to monitor and implement changes in care

Shell Point Medical Team

While it is rare for most retirement communities to have a single
onsite staff physician Shell Point has a team of multiple full-time
physicians and nurse practitioners who work exclusively with
residents. These healthcare professionals spend quality time with
each individual to get a complete understanding of his or her
unique situation and craft a personalized care plan.

SHELL POINT PHYSICIANS

Gary Goforth, MD, Medical Director
Dr. Gary Goforth leads the medical team at Shell Point. He
graduated with honors from Vanderbilt University School of
Medicine and has almost 40 years of experience, especially in
family practice. Dr. Goforth has a passion for medical education
and international medical missions. He has also received many
military decorations from the U.S. Army as well as humane and
rural education awards, including an international award for the
development of medical residency training in Afghanistan.

Rebecca Kimpel, MD
A Pennsylvania native, Dr. Rebecca Kimpel completed her
undergraduate work at Grove City College, and attended medical
school at Hahnemann University in Pennsylvania. Dr. Kimpel also
brings extensive experience in residential care, private practice, and
emergency room care. Most recently, she served as a physician at
Whitehaven Center in Pennsylvania.

Murilo Lima, MD
In 2012, Dr. Murilo Lima obtained a bachelor’s degree in biology
from Florida Atlantic University in Boca Raton, Florida, followed
by a doctorate from the American University of the Caribbean
School of Medicine in Saint Maarten. Dr. Lima is currently
completing his board certification in family medicine through
Florida State University.

David Nesselroade, MD
Dr. David Nesselroade graduated from West Virginia University
School of Medicine. He also completed his internship and residency in
family practice at West Virginia University School of Medicine.
Dr. Nesselroade became board certified in Family Practice in 1979, and
began practicing at Shell Point in 1986. His primary area of care is the
skilled nursing facility.

Sajid Wazir, MD
Originally from Pakistan, Dr. Sajid Wazir received a robust education
in English, biology, physics and chemistry in Punjab, Pakistan before
graduating from King Edward Medical College. After gaining volunteer
experience through the World Health Organization, participating in
a polio eradication campaign in Pakistan, Dr. Wazir worked at the
Mayo Hospital in Pakistan. He also brings prior experience with NCH
Healthcare System in Naples, Florida, where he specialized in internal
medicine.

Kathy Thompson, MD
Dr. Kathy Thompson joined Shell Point in 2019 from a geriatric
fellowship at Florida Hospital in Winter Park, Florida. She received
a bachelor’s degree in chemistry from Southern Illinois University, a
master’s degree in science education from University of Central Florida,
and her medical degree from Ross University School of Medicine in
Portsmouth, Dominica. Dr. Thompson also completed a family medicine
residency program at Piedmont Columbus Regional in Georgia.

Nurse Practitioners

Lewis Caldrone, APRN
Lewis Clark received a master’s degree in nursing from Franciscan
University of Steubenville in Ohio, with a specialization as a family
nurse practitioner. He also received a bachelor’s degree in nursing
from Wheeling Jesuit University in West Virginia. He specializes
in adult care, geriatrics, women’s health, mental health and acute/
chronic illnesses. He was honored as the Lee Physicians Group’s
2017 Advanced Provider of the Year. He also served as a clinical
preceptor for advanced practice nursing students.

Rocio Campbell, APRN
Rocio “Rosie” Campbell began working at Shell Point in 2008 and
served as an LPN – and later an RN. She is the first-ever recipient of
the Edith C. Frazer Scholarship, which was generously provided by a
Shell Point resident to benefit a healthcare employee. She graduated
from the University of South Florida (USF) in 2014 with a bachelor’s
in nursing with a focus on leadership and management. Rocio was
then accepted into the Acute Care Nurse Practitioner program at
USF, and went on to obtain her master’s degree in nursing from USF.
She is also certified through the National Alliance of Wound Care
and Ostomy.

Carol Clark, EdD, APRN
Carol Clark received her baccalaureate, master’s, and post-master’s
degrees in primary care from Indiana Wesleyan University. She also
received a doctor of adult and community education degree from
Ball State University, and holds certification as a gerontological
nurse practitioner from the American Nurses Commission on
Certification. Carol came to Shell Point in 2003, and currently
serves as the director of outpatient services at the J. Howard Wood
Medical Center, as well as remaining involved in clinical practice.

Amy Dudley, DNP, FNP-BC
Amy Dudley received a bachelor’s degree in Spanish and
communications from Oakland University in Rochester, Michigan. Her
nursing education spans two decades and includes an associate’s
degree in applied science (nursing) from Piedmont Virginia
Community College in Charlottesville, Virginia, a bachelor’s degree in
nursing from Old Dominion University in Norfolk, Virginia, a master’s
degree in nursing from Florida Gulf Coast University in Fort Myers,
Florida, and a doctorate in nursing practice from the University of
South Alabama. Amy has been an ANCC board-certified family
practice APRN since 2010, serving residents in the Larsen Pavilion as
well as the J. Howard Wood Medical Center.

Susan Moran, APRN
Susan Moran received her master’s degree in nursing from William
Paterson University, and her bachelor’s degree in nursing from
Trenton State College in New Jersey. She brings extensive, diverse
experience in the medical field, including primary and urgent care for
independent living residents, and medical management of subacute
rehab inpatients, long term care patients and assisted living residents.

Behavioral Health

The Behavioral Health team at Shell Point engages with residents
to support their overall concept of self, exploring the emotions,
behaviors and biology relating to mental wellbeing and their
ability to function in everyday life.

Chris Votolato, PsyD
Director of Behavioral Health
Dr. Chris Votolato obtained a doctoral degree in clinical
psychology from the American School of Professional
Psychology, a master’s degree in psychology from the
University of Baltimore, and a bachelor’s degree from Loyola
College in Maryland. Throughout his education and training,
Dr. Votolato has been dedicated to psychological practice
involving older adults. He joined Behavioral Health in 2007,
and is passionate about enhancing the future of patient care
by participating in groundbreaking research projects.

Kathy Fratrick, LCSW
Kathy Fratrick, LCSW, is a native of Appomattox, Virginia.
She received a bachelor’s degree in psychology from
Westhampton College/University of Richmond, and a
master’s degree in social work from Virginia Commonwealth
University. Her early career included a specialty with children,
adolescents, and adults in both inpatient and outpatient
psychiatric settings. Kathy has been with the Department of
Behavioral Health at Shell Point since 2002. Currently, Kathy’s
interests and expertise are in the areas of depression, anxiety,
Alzheimer’s and other dementias, caregiver stress, grief
counseling, and transitional care across the continuum.

Established in 1997, the Shell Point Pharmacy provides over-the-
counter products, durable medical equipment and prescription
services. A team of registered pharmacists fill thousands of
prescriptions every month while also offering professional
consulting, as well as medication therapy management for all Shell
Point residents.
Prescription services are available for all levels of care offered at
Shell Point, including The Larsen Pavilion, King’s Crown, The Arbor,
The Springs, and independent living residents. For most orders, the
pharmacy will deliver your prescriptions to your door on the same
day, free of charge. Most insurance plans and all Medicare Part D
plans are accepted.

The Rehabilitation Center

The Rehabilitation Center at Shell Point is an Heather Jones
important component of the continuum of healthcare, Director of Therapy
providing occupational, physical and speech therapy
services. The state-of-the-art facility opened in 2011 to
provide both inpatient and outpatient rehabilitation
services through an interdisciplinary team of
professionals.

Short term rehabilitative services include physical,
speech, and occupational therapy; indoor and
outdoor aqua therapy pools, a full-size occupational
therapy suite and beautiful outdoor transition therapy
paths. Specialized programs for dysphagia, pain
management, stroke and low vision are also offered.

A leading-edge approach focuses on successful
functional outcomes that maximize recovery from
multiple post-surgical and complex medical problems,
helping residents reach their maximum potential and
level of function. Post-operative care is available for
orthopedics, cardiac, pulmonary, neurological, and
other specialties, including stroke and memory care,
dysphagia, pain management and low vision.

The therapy gym contains a wide range of equipment
designed in a circular shape to build camaraderie and
motivate residents to engage in the rehab process.
Private treatment rooms are also available as needed.

A dedicated occupational therapy suite features a full
kitchen, bathroom and bedroom to practice “activities
of daily living” in preparation for a safe discharge
home. Two aquatic therapy pools, heated to 92
degrees, include underwater treadmills and a lift for
those unable to use stairs.

Appointments for therapy are made through one’s primary physician. Upon
receipt of a doctor’s prescription, the therapy team will call each patient to
schedule appointments.

Hospice Care

If desired, hospice care is available on the fourth floor of the Larsen
Pavilion Skilled Nursing Center.
Both palliative care and hospice services are available. Palliative care
provides pain management to alleviate pain during prolonged illness,
while hospice services focus on preventing, relieving and reducing
symptoms to allow the individual to experience comfort and peace at
end of life.
Care is administered by a professional, experienced care team that
offer social, emotional and spiritual support for both the patient and
the family. Care is offered by onsite physicians as well as RNs that are
certified by the National Board of Hospice.

Radiology

Florida Radiology Consultants has been serving the Shell Point
community for many years. The group provides x-ray and ultrasound
services from their location in the J. Howard Wood Medical Center on
The Island at Shell Point. Shell Point residents’ results are expedited
immediately to onsite medical staff.
On-call x-ray services are also provided on weekends during
designated hours.

Specialty Medical
Consultants at Shell Point

A comprehensive array of specialty medical providers serve
patients at the Arbor Medical Specialty Clinic, located in the
Woodlands at Shell Point. These caring professionals, who are
contracted by Shell Point as consultants, add another dimension
of care at Shell Point by affording residents the opportunity to
receive even the most specialized care without the need to leave
the community.

Audiology & Hearing

Jack Adams, MS, CCA-A, audiologist and owner of Audiology
Consultants of Southwest Florida, has been helping people
improve their hearing for more than 30 years. At the Arbor
Medical Center, he can provide a full range of audiology
services, including hearing tests to determine a patient’s
level of impairment and the cause of hearing loss. “Hearing
tests are simple, quick and painless – and they are covered
by Medicare,” said Dr. Adams. “I always encourage people
to seek help as soon as they notice any communication
problem.”

Chiropractic Care

Dr. Charles Fulton, a fourth-generation native of Fort Myers,
has been in private practice for more than 20 years. While
working predominantly as a hands-on chiropractor, he will
also incorporate other techniques for those who may prefer
alternate care options. His ultimate goal is to get his patients
out of pain and back to regular activity as quickly as possible.

Dentistry

A native of Fort Myers, Douglas Milsap, DDS, attended
Emory University and completed his dental training at Temple
University in 1977. After a year on the faculty in the Department
of Fixed and Removable Prosthetics, he returned home to Fort
Myers to begin a private practice. He is a member of the FDA,
ADA and Lee County Dental Society. Dr. Milsap enjoys serving
the residents of Shell Point and finds satisfaction in knowing
that he has been able to improve their quality of life through
good dental care.

Deborah Streater, DDS, graduated from the University of North
Texas in 1988 and received a dental degree from Baylor College
of Dentistry in 1992. Dr. Streater joined Dr. Milsap’s practice in
1996 and began serving the Shell Point community in 2001.
Dr. Streater travels to attend classes and obtain updates on
implants, as well as cosmetic and family dentistry. She is a
member of the FDA, ADA and Lee County Dental Society.

Dermatology

Shannen Vixler, C-APRN, graduated from Florida Gulf Coast
University School of Nursing with both bachelor’s and
master’s degrees in nursing, including successful completion
of the nurse practitioner program. Her more than 20 years
of dermatology clinical experience is reflected in both
the quality and thoroughness of her compassionate care.
Shannen recommends yearly skin examinations for adults,
and has passion for providing quality dermatologic care to
the residents of Shell Point.

Allison Tubbs, C-APRN, graduated at the top of her class
from Florida Gulf Coast University with a master’s degree
in nursing and has since achieved clinical excellence as a
registered nurse in critical and post-anesthesia care. She was
also the recipient of the Outstanding Primary Health Care
award presented by Sigma Theta Tau International Honor
Society. Allison cares for patients by utilizing evidence-based
guidelines blended with extensive clinical experience from her
more than 20 years of experience in direct patient care.

Elmquist is an experienced, licensed ophthalmologist who
is board certified by the American Board of Ophthalmology

Sarah Eccles-Brown, MD, is a licensed ophthalmologist who
is board certified by the American Board of Ophthalmology
and a fellow of the American Academy of Ophthalmology.
She graduated magna cum laude with a bachelor’s degree
in biological science and a bachelor’s degree in psychology
from Florida State University. She completed medical school
at New York Medical College, earning her MD, along with an
election to the medical honor society Alpha Omega Alpha.
She served as an ophthalmology resident at Wilford Hall
Medical Center and San Antonio Military Medical Center in
Texas, gaining valuable experience at a Level 1 trauma center
and caring for burn ward patients from all over the state as
well as active duty members injured abroad. She also served
as a major in the U.S. Air Force, where she had the honor of
serving the active duty, retiree and dependent population of
Northwest Florida as a comprehensive ophthalmologist.

Orthopedic

Dr. David Eichten attended medical school at Kansas City
University of Medicine and Biosciences. During that time, he
spent two years on clinical rotations in Fort Myers, where he
worked with Dr. Humbert and Dr. Fenning at Joint Implant
Surgeons of Florida. He also spent time at Shell Point working
with resident physicians and completed an extra year of
fellowship training in hip and knee joint replacement. Dr.
Eichten takes pride in providing the best in joint replacement
care and helping patients get rid of pain so they can return to
the activities they love.

Podiatry

In July 2016, Dr. Richard D. Calhoun, DPM, opened a local
podiatry practice in Fort Myers called Foot and Ankle
Care of the Island Coast. He received his medical degree
from Kent State University College of Podiatric Medicine in
Independence, Ohio, and completed a residency at Mercy St.
Vincent Medical Center in Toledo, Ohio. His training focused
on forefoot and reconstructive rearfoot and ankle surgery, as
well as lower extremity wound care and diabetic limb salvage.
He advocates a multidisciplinary team approach to medical
care, and is fluent in Spanish.

Dr. Robert Dushack is a U.S. Navy veteran of the Gulf
War. He graduated from Saint Vincent College in Latrobe,
Pennsylvania, where he received a bachelor’s degree in
biology. Dr. Dushack completed his formal medical training
at Temple University School of Podiatric Medicine, and also
completed an intensive residency program at the University
of Pittsburgh, Mercy Hospital. Dr. Dushack is trained in the
medical and surgical care of all lower extremity ailments. Dr.
Dushack lives in Fort Myers, where he has his own practice,
APEX Foot & Ankle Center, LLC.

Pulmonary Critical Care

Dr. Sunil Lalla received his medical degree from Topiwala
National Medical College in Bombay, India. Following eight
years of post-graduate training, he opened a private practice
in Fort Myers. He is certified by The American Board of
Internal Medicine for Internal Medicine, Pulmonary Disease
and Critical Care Medicine.

Urology

Dr. Carolyn F. Langford, D.O. is board certified by the
American Board of Osteopathic Surgeons. She is a cum
laude graduate of Rutgers University and a graduate of
the University of Medicine and Dentistry of the New Jersey
School of Osteopathic Medicine, where she completed her
urologic surgery training. Among her special interests is
the treatment of the entire pelvic floor as a whole, as many
urologic problems of incontinence and prolapse coexist. She
is one of a few doctors in Southwest Florida that has been
chosen as a clinical instructor for other physicians by Boston
Scientific.
