Our Policies

Booking Appointment: We require all appointments to be reserved online with

a secure debit or credit card. Any appointment that is not secured with a card at least
24hours before the reserved appointment, it will be canceled.

Late Arrivals: Plese make sure you arrive with on time for your appointment. If you

arrive 15min late, we will have to reschedule or cancel appt, since we will not be able to
extend your appt time.

Cancelations: We require 24hour notice of cancelation or rescheduling of

appointment. Failure to provide notice of cancelation will result in a cancelation fee of half
the amount of the service scheduled charged to payment on file.

Guest/Small Children: We politely ask that you refrain from bringing an

infant/children guest to your appointments. We strive to provide a relaxing environment.

FRIENDLY REMINDER BAE

Please come to your appointment with clean eyelashes (no mascara) if
you are unable to do so, we can clean your eyelashes for you. However,

this will limit the amount of time spent on your full set or infill.

LASHES GENERAL HEALTH & SAFETY RECOMMENDATIONS/ AFTER CARE

Eyelash extensions should be applied by an experienced certified and/or licensees Eyelash
Technician. Which should take place in a Sanitary setting that has been properly disinfected.

With the application of eyelash extension there are many safety and health hazards to be
placed into consideration. Not everyone’s eyes and skin are able to handle certain ingredients

in the adhesive which can cause an allergic reaction. An allergic reaction to formaldehyde is
very common, one may experience itching, burning, redness and swelling of the eyes. If you
have a known sensitive of your eyes it is best to advice your eyelash technician prior to your

application.

Therefore, it is very important for your eyes to remain closed during the entire
procedure and follow all after care instructions provided by your technician. In some case a
patch test will be provided to test your sensitivity to adhesive. Eyelash extensions are to only be
applied to each individual natural lash and should not encounter physical contact with your

skin. Properly maintained eyelash extensions increase the Longevity of your extensions.

Avoid rubbing, picking and/or pulling at your extensions and natural lashes. This can
cause damage to your natural lashes. Don't use oil-based and/or waterproof mascaras

or eyeliners. The lint can snag your lash extensions and pull your natural lash out.

Baeilashes Technician recommends appointments every 3-4 weeks, to fill in the areas where
the natural lashes have fallen out and new lashes are growing in. Please comb through the
lashes each morning to maintain a good appearance, remember there just like hair. Wash your

lashes regularly to maintain healthy and clean lashes

EYELASH EXTENSION CLIENT CONSENT FORM

______ I understand that eyelash extensions are semi-permanent. I agree to have eyelash extensions applied to
my natural eyelashes.

______I understand that in rare occasions there are risk associated with having eyelash extensions, I have read the
LASHES GENERAL HEALTH & SAFETY RECOMMENDATIONS/ AFTER CARE form provided by the BAEILASHES
Certified Technician. I have given an opportunity to ask questions about the products, the application procedure,
and any risk involved.

_____I understand & agree to the after-care instructions provided by the BAEILASHES technician. I realize and
accept the consequences of failure to adhere to these instructions may cause eyelashes to fall out and/or decrease
the time the lashes last.

_____ I understand and consent to having my eyelashes closed and covered for the whole duration of
approximately 1-3 hours. Times may vary depending on the type of procedure.

_____ I give permission to BAEILASHES to take before and after pictures for use of social media branding/or
advertising.

This agreement will remain in effect for this procedure and all future follow-ups conducted by the licensed eyelash extension
technician professional. I read and understand that this consent agreement is legal and binding. I have read and fully
understand all information in this agreement to be true and correct. I am over 18 years of age and consent to the agreement
and to the eyelash extensions application procedure.
