Our Service Commitment





An appointment will be arranged for you to have an

assessment with our care manager in the comfort of your
own home, hospital or nursing home.




This assessment ensures that we have a full understanding of

your care/support needs and the environment in which the
carer will be working. We will also agree the days, times and

length of your care visits.

We will write your care plan from the information given at

your assessment. This will ensure the carers understand what

you need and when.

Your care package will be reviewed by our care manager.
This is to ensure that your needs continue to be met and that

you are happy with the services we provide you with.































Your family or representatives are very welcome to attend
the review meeting.

We aim for one of our care managers to introduce you to

your care team so you know who is coming to see you.
Carers will be always in uniform (unless you decide otherwise)

and they will all carry an ID Badge at all times.

































We will call you if there are any changes to your care call.

Your carer will complete a report after each care visit

detailing what has been done to ensure that all tasks
are completed.




























This report is written in your Daily Record Book which

stays in your home for other health professionals to

see.

These services are bespoke and can be delivered at a time to suit your
needs. We have broad range of timescales for you to choose from,
anything from 30 minutes to 24 hours.

Examples of Service Provided





Our Care Team are qualified to prompt, assist and administer

prescribed medications that have been dispensed by a

pharmacist.
Our Care Team can also deliver specialist medication such as:

o Via a peg

o Support with Controlled Drugs

























All medicaton will be recorded within your care plan and

signed for by the carer.

DEMENTIA CARE

For people suffering with dementia, the familiarity of their own home
can make a world of difference. They will feel safe, secure and

comfortable in their own home. Our carers are trained to understand
what a person with dementia may be feeling.





RESPITE CARE

This service can be provided in the customers own home or on a
supported holiday away from home. We understand that caring for a

loved one is 24/7 and this service gives the main carer a break should
they require one.





RE-ENABLEMENT

This service is available should you require a short spell of care whilst
recovering from an illness, operation or difficult time.

END OF LIFE CARE


 The majority of people with a life threatening illness would prefer to
be in their own home with their personal possesions and memories
surrounding them.





COMPLEX CARE
Our complex care service is most suited to individuals who:

 Have been discharged from hospital and have significant nursing

and clinical care needs that need to be continually monitored.
 Are in hospital wards such as intensive care or high dependency units

and are due to return home; faced with long term recovery process.

 Suffer from degenerate conditions and have on-going care needs.
 Have physical or learning disabilities and require long-term support in

their home.

 Have complex care needs and are dependant on life sustaining
technology.

Paying for your Care






Customers have different payment options when it comes to
paying for care.



























1. Private Funding


You pay for care yourself and the contract is between you and The

Care Company.
This could be paid weekly, fortnightly or every 4 weeks.

2. Self Directed Support

This is where people are in control of the support they receive and
when they receive it. It is also referred to as personal budget or a

personalisation plan. This is where you receive funding following a
financial assessment by social services who then agree:




• A personal budget

or
• An individual budget

or
• A direct payment




This is funded by a local authority but The Care Company will invoice
you for your care.



You can choose to be invoiced weekly, fortnightly or every 4 weekly.

3. Funded Care/ Part Funded Care through Local Authority


This is where the contract is held between The Care Company and
the local authority. This is usually set up between yourself and Social

Services following a financial assessment.
