In 1988 the first Proton Pump Inhibitor (PPI) omeprazole was launched and till date the PPIs
are amongst the most (over) prescribed drugs in clinical practice. This is due to excellent safety profile claimed in
earlier trials. Later studies which looked into PPI prescription noted that PPIs are prescribed by default with
NSAIDs, anti-diabetics and cardiovascular medications. It was found that up to 70% of prescriptions for PPIs were
inappropriate. US FDA raised a lot of safety alerts for PPI, especially in cardiac and elderly patients:
Hypomagnesemia in 2011, Clostridium Difficile associated diarrhea in 2012, Vitamin B12 deficiency in 2014 and
Acute interstitial nephritis in 2014. Besides, PPI interaction with clopidogrel was reported in 2011, and PPI
consumption was shown to be associated with increased myocardial infarction in 2015. It was in 2016; PPIs
association with CKD, progression to ESRD was reported. The Renal side effects of PPIs are less often reported and
may go unrecognized.

UPDATES FROM RECENT CLINICAL TRIALS

§ Vitamin B12 supplements may reduce the risk of diabetes.
§ As standard TB-drug treatment are ineffective of nearly 1,30,000 of the

2.8million TB patients in India, two relatively-new and powerful drugs,
Bedaquiline, and Delamanid could benefit such patients with Drug-Resistant
(DR) TB.
§ U.S. researcher's report that for every 10 grams of fiber consumed per day, the
risk of developing heart disease may decrease 14% and the risk of dying from
heart disease may decrease 27%.
§ Methylphenidate approved for the treatment of ADHD and narcolepsy,
improves cognition, quality of life in epilepsy patients.
§ Methyldopa helps in the prevention and treatment of Type I Diabetes.
§ Use of Clarithromycin may lead to increased potential cardiac problems or
death in patients with heart disease.
§ Nasal powder formulation of Sumatriptan is best than the oral tablets in
reducing nausea.

DISEASE UPDATE

MARBURG VIRUS DISEASE (MVD)

MVD is formerly Marburg haemorrhagic fever, severe illness of
humans and non-human primates caused by Marburg virus, results from
prolonged exposure to mines or caves inhabited by Rousettus bat
colonies. It spreads through human-to-human transmission via direct
contact (through broken skin or mucous membranes) with the blood,
secretions, organs or other bodily fluids of infected people, and with
surfaces and materials (e.g. bedding, clothing) contaminated with these
fluids. Sexual transmission of the virus can also occur.

Incubation Period: 2 - 21 days.

Symptoms:

§ Severe headache, Malaise, High fever, Progressive and rapid debilitation

Diagnosis:

§ Antibody Enzyme-Linked Immunosorbent Assay (ELISA), Antigen Detection Tests, Serum
Neutralization Tests, Reverse-Transcriptase Polymerase Chain Reaction (RT-PCR) Assay; and Virus
Isolation by Cell Culture

Treatment:

Supportive care – rehydration with oral or intravenous fluids – and treatment of specific symptoms,
improves survival. There is as yet no proven treatment available for Marburg virus disease. However, a
range of potential treatments including blood products, immune therapies and drug therapies are currently
being evaluated.

ORPHAN DRUG STATUS

REGISTRATIONAL PHASE 3 TRIAL - FAST PATH TO MARKET

Soft tissue sarcoma (STS) is a malignant cancer cells originating in soft tissues
like muscles, tendons, fat, blood vessels, lymph and nerves. These tumors can develop
anywhere in the body but are found mostly in the chest, abdomen, arms and legs.

Incidence of STS comprises about 1% of all adult malignancies and about 15% of all pediatric
malignancies. First line sarcoma treatment options are still limited to chemotherapy. Emphasis on emerging
genomic and molecular pathway targets to combat this confounding disease.

Doxorubicin remains as a gold standard first line chemotherapy for patients with soft tissue sarcoma.
Despite modest activity and significant toxicities, no cytotoxic monotherapy has yet yielded superior overall
survival over doxorubicin for advanced STS therapy.

Aldoxorubicin is a prodrug of doxorubicin that binds to the cysteine -34 position of serum albumin
immediately after administration through an acid sensitive hydrazone linker and it is subsequently reaches the
tumor tissues; the acidic environment of the tumor cleaves the acid sensitive linker and facilitates doxorubicin
release in the targeted-tumor tissue. Because of passive accumulation within tumors, this agent may improve the
therapeutic effects of doxorubicin while minimizing systemic toxicity.

In clinical studies, there has been evidence of mitigated cardiotoxicity and less alopecia while compared
to doxorubicin. Pivotal, global phase 3 clinical trial data has supported aldoxorubicin in STS patients and did not
cause clinically significant cardiac, renal or hepatic toxicities.
