Annual Report

Dear Friends,

  36,000 people access homeless
As I look back on 2018, I remember a year of change. Staff

continued to be enthusiastic in providing care to people services in Houston each year.
affected by Hurricane Harvey, from shelter to permanent

supportive housing, and in establishing continuity of care HHHWith your support,
with many people who have previously relied on
emergency rooms for primary care. Although one of our
programs in permanent supportive housing closed due
to a shift in funding, two other parallel programs opened
at the same time, and we are now providing care to more
people in these programs than ever before. HHH has
been recognized at the local, state and national levels as

providing exemplary integrated care that improves serves 1 out of every 4 people
health, stability and social outcomes and leads to reduced experiencing homelessness.
burden on the community, especially in terms of reduced

emergency room visits and hospitalizations. At the heart

of each interaction with our patients, staff remember that

in promoting health, hope and dignity, we can change

the lives of those we serve. Thank you for your

contributions in helping us to achieve this mission!

“The connection between In 2018
housing and health is
coldly logical. The sick
and vulnerable become
homeless, and the
homeless become sicker
and more vulnerable."

- Seiji Hayashi, MD, MPH, The Atlantic

Together, we raised

$6.3M to combat homelessness
through access to care

Which served

8,262 Houstonians
affected by homelessness

Through

38,130 medical, mental health, social & support services,
dental and outreach visits.

"I love this organization.
It keeps me healthy
and plays a huge part
in my recovery."

4,226

people received medical, outreach and
case management via 30,630 patient visits.

Before & After “I would look in
the mirror, and
1,029 all I could see
was this
people served via 7,395 broken person
dental visits who had made
all of these
mistakes.
Getting my
teeth - it
restored my
dignity and my
sense of hope.
It changed my
life.”

For those who have been chronically

homelessness and who are now in

Supportive Housing, our on-site care teams

have decreased hospital emergency room
71%utilization by
reducing public costs and

improving the lives of those who are vulnerable.

The wrap-around care HHH provides within
Supportive Housing is a cost-effective and
compassionate solution to ending chronic
homelessness. Supportive Housing serves those
who have a diagnosed disability. For them,
access to effective, long-term care is an
essential element for breaking the cycle of
homelessness.

Dawn has lived in Supportive Housing for 410
almost two years. Before that, her complex
health issues kept her on the street, unable to people served in Supportive
gain housing or access to the type of consistent Housing
care she needed. She can now rely on HHH’s
on-going, on-site care to help her stay healthy,
stable and with a roof over her head.

“I never thought I would end up
on the streets. I almost gave up.
But I want people to stop
thinking that no one cares about
them, because there are people.
I don’t want people to lose hope.
Hope is out there, and places like
Healthcare for the Homeless –
Houston can help you find it.”

In 2018, for the first time in
seven years, Houston saw an

increase in homelessness,
which was largely attributed

to the lingering effects of
Hurricane Harvey.

237

individuals who were homeless post-Hurricane Harvey received on-going
medical, mental health and support services.

2nd Annual Homeless
Success Day

On the longest day of the year – the summer solstice –
more than 100 people from across the community came
together to celebrate the men and women who have
moved beyond homelessness and into health and housing.

Volunteers mean the
world to us.

602 Volunteers

provided 22,387 hours of services with an
in-kind value of $568,314.

HHH serves as a teaching site for
hundreds of students each year seeking
careers in health-related fields. This is
part of our commitment to fostering the
next generation of health professionals.
