It highlights updates from our nurses and student nurses, and with these exciting contributions, we keep abreast of clinical practices, new technologies and arm ourselves with the latest and greatest from the nursing world. Through Raffles Nursing Times, we also hope to interact with Raffles nurses and stay in touch with our nursing friends from all walks of the field. With this current global health concern, the need for specialised facilities can never be overstated. Situated in the heart of the community, Raffles Dialysis Centre (RDC) commenced its journey with the intent of addressing the growing prevalence of patients diagnosed with ESRD. 5 Nursing Times Vol 4, No 4 ‘BEHIND-THE-SCENES’ STORIES Raffles Dialysis Centre In view of the increasing number of ESRD patients requiring dialysis, RDC has recently expanded its operations from five to 10 beds to accommodate the growing need of such facilities nationally. RDC offers a comprehensive suite of dialysis services that are approved by the Ministry of Health (MOH). This allows us to better suit the needs of the vast ESRD population. Beyond traditional Haemodialysis (HD), we also include Haemodiafiltration (HDF), High Dependency Haemodialysis (HDHD), and Peritoneal Dialysis (PD). Concurrently, RDC also provides dialysis services to both inpatients and tourists who require dialysis treatments while they are on holiday in Singapore. The comprehensive dialysis services provided by RDC also means that we are equipped with cutting edge dialysis machines, allowing us to provide various dialysis treatments to the extensive ESRD population. These dialysis machines are manned and managed by a team of highly skilled and professional dialysis nurses who have had years of renal experience to ensure that all patients receive the best possible care. RDC optimises the dialysis process by combining both technology and the competence of our nursing team to ensure our patients receive efficient and effective treatments that are comfortable for them. What truly differentiates RDC is our team of experienced and compassionate dialysis nurses and nephrologist. Besides being skilful and knowledgeable at what they do, they are also empathetic and understand what an ESRD patient has to undergo on a long-term basis – an arduous journey that is both mentally and physically challenging. Therefore, to enhance our patients’ experience at RDC, our team of nurses and nephrologist not only provide medical expertise, but also Patients who receive treatment at RDC can be assured that their well-being is always our utmost priority. Individualised treatment plans are drawn and catered to each patient’s unique needs to ensure the best possible outcomes. This includes scheduling of patient’s dialysis sessions, managing and titrating their medications, providing dietary advice, and modifying their treatment regime according to their blood test results. The dialysis nurses also commence and conclude patients’ dialysis treatment in the safest manner according to stringent standards of procedure every time patients come for their treatment. Our focus remains firm on patients’ overall health and comfort to ensure a good quality of life despite having to be on dialysis three times a week. All in all, RDC is more than a medical facility. It is a beacon of hope for ESRD patients who are battling against renal diseases. Over the years, we have earned the trust of our patients as we provide them with compassionate quality care. 6 Nursing Times Vol 4, No 4 ‘BEHIND-THE-SCENES’ STORIES Hospital Management Asia (HMA) Award Behind-the-scenes Hospital Management Asia (HMA) Award At the recent Hospital Management Asia Awards 2023 (HMA Awards) held on 6 September 2023, at Shangri-La Kuala Lumpur in Malaysia, Raffles Hospital (RH) was recognised with these two awards: Chief Nurse of the Year Award (Gold Winner) – Ms Lilian Yew, Chief Nurse Gold Award Winner for Patient Experience Improvement Category – Based on RH’s project titled “Implementing Person-centred Care Approach Using “WECARE” Project” The ceremony was attended by Ms Lilian Yew and Senior Nurse Clinician (SNC) Norazlina Binte Hassan, with the latter receiving the Gold Award on behalf of Raffles Hospital’s inpatient nursing team.


7 Nursing Times Vol 4, No 4 The Chief Nurse of the Year Award was introduced at the HMA Awards 2023 for the first time since its inception in 2002. Lilian was awarded the title in recognition of her contributions to the nursing profession, and for her successful efforts in leading the RH nursing team to receive multiple internationally-recognised nursing accolades and accreditations. During her winning speech at HMA 2023, Lilian shared: “I’m deeply honoured and humbled to win this prestigious award. This is truly a momentous milestone for me. It is not just a personal achievement, but it also represents the dedication, passion, and resilience of many nursing leaders. I would like to thank HMA for including this award category to recognise the work that a Chief Nurse does, which includes providing effective leadership and counsel to improve patient outcomes and the standards of nursing care.” ‘BEHIND-THE-SCENES’ STORIES Hospital Management Asia (HMA) Award The “WECARE” project, which won the Gold Award at HMA Awards 2023, was implemented by the RH nursing team with an aim to increase nurse-patient contact time by anticipating patient needs. The “WECARE” project saw a reduction in fall rates and an increase in customer satisfaction. For SNC Norazlina, the Gold Award was a reminder of the important work that nurses do. Receiving the award on behalf of the RH nursing team, she said: “This award wouldn’t be possible without the strong support and encouragement from Ms Yew, senior nursing leaders, and fellow colleagues whom I derived strength from and to ensure the success of the project. We hope our “WECARE” project can inspire more healthcare professionals and continue to improve patient safety and satisfaction in healthcare. Thank you once again to HMA for this recognition.” My name is Norazlina Hassan, I am a Senior Nurse Clinician. I started my career as a nurse in 2001 in one of the restructured hospitals. After 15 years, I decided to venture into private healthcare and that was when I joined Raffles Hospital in 2016. I have been in this profession for almost 20 years and I am still enjoying it. With the experiences I have gained during my service in a restructured hospital, I continue to nurture, share, and cultivate the good practices in Raffles Hospital. I am privileged to be given the opportunity to participate in the HMA Awards 2023.

9 Nursing Times Vol 4, No 4 In conjunction with the Worldwide Pressure Injury Prevention Day, we are glad to have successfully organised our inaugural Raffles Pressure Injury Prevention Awareness Day event on 8 November 2023. This event would not be possible without the help of a team of colleagues from Raffles Hospital – nursing educators, inpatient nurses, and occupational therapists. External vendors such as B Braun, Nestle, and 3M had volunteered to be a part of this event too. The main goal of this event is to incorporate the new gold standard framework with the “aSSKINg” acronym into our patient care standards to prevent Hospital-Acquired Pressure Injury (HAPI). To help participants learn better, we have organised interactive and informative booths for each letter of the acronym! The event has instilled a deeper understanding of the Braden scale assessment tool – accurate scoring and interventions, and various types of seat cushions, wedges, and sliding sheet for patients’ use including proper technique when using these products. Additionally, we have also learnt about the importance of nutrition, such as nutritional values and the types of available supplements, which some came with samples for those who were adventurous. Participants of the event were also taught the roles that “incontinence” and “giving information” play in preventing pressure injury within the hospital and the community. 13 Nursing Times Vol 4, No 4 The event was a good beginning to kick-off future events to share knowledge and skills, and help each other to stay updated with the latest standards of patient care. We would like to express our heartfelt gratitude to Chief Nurse Lilian Yew and Senior Manager Jaslyn Yeo for their invaluable support and encouragement in organising this event. Stay tuned for more teaching events and workshops next year. In appreciation of our colleagues’ attendance, we have prepared refreshments and gifts for them. The effects aimed to create an immersive experience of the chosen theme for everyone.


15 Nursing Times Vol 4, No 4 What inspired the theme or concept for this year’s dinner and dance, and how did you bring it to life? For this year’s Dinner and Dance, we wanted to choose a theme that was familiar or current to most people; be it a movie title, sports, or world event. It happened that “Dungeons and Dragons”, which is a very popular and well-known title, was screened earlier in the year. We decided to adopt it as the theme. Its abbreviated form is also the same as the short form of Dinner and Dance (“D&D”). Additionally, “Dungeons and Dragons” is a fantasy genre. For staff who planned to dress to the theme would have a variety of choices such as thieves, wizards, warriors, and elves as depicted in the movie. Tell us about the culinary experience at the event. We have selected Fairmont Hotel as the event venue. The organising committee has paid close attention to ensure our staff get to enjoy a delicious spread of dishes that also catered to our Muslim colleagues. Kudos to the D&D committee for working hard behind the scenes to improve the overall D&D experience for everyone who attended. What were some key highlights from the D&D? We were happy to engage Kyle Ravin as the emcee for the event. Kyle is an experienced emcee and a professional DJ with ONE FM 91.3. Better known as The People’s Emcee, Kyle has been entertaining audiences across Asia Pacific by fusing magic and performance art with hosting. For the opening act, he performed the ‘fire eating’ act that thrilled the audience. Dr Phuah Sheng, General Practitioner from Raffles Medical, co-hosted the event with Kyle. The duo created a lively atmosphere that kept all the guests engaged throughout the event. Many of them later expressed that it was one of the best D&Ds they have attended. Prior to the event, Dr Phuah was involved in providing suggestions that helped the organising committee refined the activities planned for the D&D. ‘BEHIND-THE-SCENES’ STORIES RMG Dinner & Dance 2023 Dr Loo and Dr Loh made a grand entrance with a skit about slaying a dragon. This was followed by an opening speech by Dr Loo. “Raffles Got Talents” showcased performances by six teams formed by staff from the Singapore and overseas offices. What were the memorable moments or surprises during the event? There were many. For me, an unforgettable sight was when the emcee gathered all the guests near the stage for the grand lucky draw prize. The atmosphere was very infectious, and everyone was hyped up. That was the first time I saw the dance floor being filled with so many people at the same time during a D&D. Our colleagues did a “catwalk” wearing a new set of uniforms for our fontliners, which is slated to be launched next year.


16 Nursing Times Vol 4, No 4 WorldSkills ASEAN (WSA) 2023 NURSING EDUCATION WorldSkills ASEAN WorldSkills Singapore organised the 13th WorldSkills ASEAN (WSA) 2023 competition from 23 to 25 July 2023 at the Suntec Singapore Convention & Exhibition Centre. Over the course of three days, the Education and Career Guidance (ECG) sub-committee collaborated with various industry partners to organise ECG activities that targeted over 10,000 secondary school students, 6,000 PSEI students, and the public. Raffles Hospital was invited to come on board as one of the industry partners to conduct interactive ECG activities to engage students and the public to raise awareness of career opportunities and progression pathways in nursing. The main objective of our participation was to provide exposure about the healthcare industry, raise brand awareness of Raffles Hospital, and connect with youths and public to dispel any misconception about the healthcare industry. 17 Nursing Times Vol 4, No 4 Hand Hygiene Workshop – Seconds Save Lives The Nursing Education Department and AANC Arine from ward 10 worked together as a team to participate in the WSA. Clinical Educator Nur Shida and AANC Arine were led by Senior Nurse Educator Hong Wei who delivered eight sessions of hand hygiene workshops for more than 300 secondary school students. AANC Arine assisting a student to check if hand disinfection is done correctly using the Coucou box. Pledge cards were used as part of engaging students in remembering to perform the seven steps of hand hygiene. Administering Intramuscular Injection (IM) Hands-on Activity Acting Nurse Educator Jai Vaishnavi and Clinical Educator Hui Wen were led by Nurse Educator Dira to participate in a three-day exhibition booth to display career information on nursing and offer hands-on activity for participants, which was administering Intramuscular injection (IM). This helped to educate students and visitors about career opportunities and showcase the relevant skills needed for a nursing career. The students appreciated the valuable experiences and insights gained from the ECG activities. Education career guidance and networking are vital components in helping each student to navigate their academic and professional journeys. The career guidance provided students with insights into various educational paths and helped them to make informed decisions about their future. It also offered them valuable information about various nursing career paths and the skills required to succeed. Through networking with the students and teachers from various schools, it allowed us as educators to connect and empower them to make informed choices, develop their skills, and build valuable relationships that can shape their educational and career paths for years to come. Of course, the event was not without challenges. They were mostly preparing the overwhelming amount of information that needed to be presented and the logistics involved. However, we overcame this by working together and executing our respective roles as educators. Overall, the Nursing Education Department’s involvement at the WSA event was a successful and transformative experience. It provided us with essential insights, connections, and motivation. We are truly grateful for the exciting opportunity that was given to us and look forward to similar enriching experiences in the future. 18 Nursing Times Vol 4, No 4 CLINICAL UPDATES Implementation of aSSKINg Skin Care Bundle Implementation of aSSKINg Skin Care Bundle in Reducing Hospital Acquired Pressure Injury (HAPI) at Raffles Hospital In 2019, Raffles Hospital recorded an average incidence rate of hospital-acquired pressure injuries (HAPI) at 0.83 per 1,000 patient days, surpassing the targeted rate of 0.5. This prompted the initiation of a quality improvement initiative, leading to the implementation of the SSKIN care bundle that focuses on five key aspects of preventive care (Surface, Skin inspection, Keep moving, Incontinence, and Nutrition). The SSKIN care bundle, which was originally devised in 2004 by the Ascension Hospital system in the USA, has since become a widely recognised and respected standard for the prevention and management of pressure injuries. Since its implementation, the pressure injury incidence rate at Raffles Hospital has seen a remarkable 71 per cent reduction in 2020 and has consistently remained low to the present day. However, in a recent root cause analysis of HAPI incidence, wound care nurses observed that nurses encountered difficulties when conducting Braden scale assessments, leading to inaccurate Braden scoring and subsequent delays in implementing preventive measures. As a result, wound care nurses embraced the aSSKINg framework, which incorporated assessment and giving information components to address Written by: ANC Koh Jia Qian the importance of risk assessment and providing information to patient and caregiver on the care plan. This framework was subsequently adopted and improvised by the United Kingdom in 2012, which supported by robust evidence-based practices.


19 Nursing Times Vol 4, No 4 CLINICAL UPDATES Implementation of aSSKINg Skin Care Bundle Therefore, Raffles Hospital updated its current hospital policy in August 2023 to incorporate the aSSKINg framework, along with a concise summary of guidelines. It is now mandatory to initiate relevant preventive nursing interventions based on the aSSKINg framework once the total risk score is ≤15 and ≥16 but non-ambulating, unable to control body position and requires assistance in moving. The Braden Scale reassessment should be conducted every 72 hours for patients scoring ≤16 but non-ambulating, unable to control body position and requires assistance in moving and on a weekly basis for those scoring ≥16 who are ambulating independently. However, in the event of a change in medical condition or significant occurrences like post-operative recovery or prolonged procedures, reassessment should be promptly performed regardless of the initial score. Pressure injury brochures from the Health Promotion Board have been integrated into the hospital policy to facilitate the continued education of patients and their caregivers on pressure injury care. The turning chart has been revised to include a section for monitoring the condition of the skin. If there is any compromise in the integrity of the skin, nurses are required to document it in the wound chart for monitoring purposes. The purpose of this update is to help nurses maintain a heightened awareness of skin integrity during each patient turning, with the goal of achieving early detection and administering treatment before pressure injuries worsen. Additionally, an online training session has been conducted to acquaint nurses with the new aSSKINg guidelines. A well-structured training programme concentrating on the care of patients with skin damage will be scheduled bi-monthly, commencing in January 2024. The primary objective of this training programme is to enhance nurses’ understanding of caring for patients with skin tears, managing incontinence-associated dermatitis, and preventing and managing pressure injuries. Outcomes and Impacts: A monthly monitoring of the HAPI incidence rate assures the provision of high-quality care. In the event of a sudden increase of HAPI incidence rates, we will implement Plan, Do, Check, Act (PDCA) cycle to identify the root causes to improve on the interventions.

Kelly and Dr Chu were very kind and helpful with my last-minute appointment request for my child. The appointment confirmation was swift and everything impressed me. During my son’s recent stay at the hospital, I was truly impressed by the professionalism, compassion, and tireless efforts of the nursing staff. She created the human touch and care that was impressionable. My recovery got better by the day after coming home too. I have made the right decision to come to Raffles Hospital. Rose and Amutha stood out in their caring attitude. Both of them were very bright and cheery while attending to patients. There was an obvious difference in the way they carried out their duties by going above and beyond to make their patients feel comfortable. Well done and keep up the good work! Opon Rose Mae Barino Inpatient - Ward 10 I visited the clinic with my daughter who was suffering from an acute allergic reaction from something she ate. Even as we walked into the clinic, Angeline who was at the counter noticed that my daughter was unwell and attended to us immediately. Both Dr Chong and the nurse were not only very professional in taking care of my daughter, but they were both very kind and attentive. Due to my daughter’s symptoms, we had to stay in the clinic for about an hour as Dr Chong wanted to ensure that the medication was taking effect. During this time, both Dr Chong and Angeline checked in on us constantly. She took extra care of my daughter and ensured that she was comfortable. 24 Nursing Times Vol 4, No 4 I wish to extend my deepest appreciation to Angelyne for taking care of me and providing me with medication just in time when I suddenly felt dizzy at the clinic. She attended to me immediately and measured my blood pressure. As my reading was slightly elevated, she brought me a cup of warm water and told me to take my blood pressure medication. Upon realising my discomfort, she arranged for me to lie down on a bed and came to check on me every now and then to ensure that I was doing fine. She even provided me with plastic bags in case I felt like throwing up. After my doctor’s consultation, Angelyne offered some advice regarding my diet, and called to check in on me later in the day. Thank you, Angelyne, for the care and attention you gave me at the clinic. I truly appreciate it. It was not easy because I was badly bruised from previous attempts by an RMO. Mercy and Sarah came back after and tried. They found a small vein that could be used. It was challenging but they were gentle and managed to get the cannula in one attempt! Special thanks to them who were very responsive and cheerful in attending to me each time I “buzzed” for assistance. I needed to use the toilet frequently and they were very brisk in coming around to assist me with that. I would like to compliment the ICU team of nurses for their very warm attitude. Their continuous care, reassurance, and teamwork were admirable and are very important for my recovery. My wife and I would like to thank Sister Yu, SN Gillian, SN Lim Chia Shing, and SN Kiranjit for their care and help during our stay. It was a dramatic night for my wife and I. We were actually feeling hopeless, but they did a brilliant job taking care of us and encouraging us simultaneously, which made so much difference to us! They kept telling us to think positively, shared their story and kept us warm, which made us feel that there is light at the end of the tunnel (especially sister Yu who made us feel that we were part of her family). This team definitely is an asset to Raffles Hospital and how Raffles Hospital will be able to differentiate from the others. Thank you once again! The staff nurse then offered me a roll of surgical tape to temporarily mend my slippers so that I could continue walking before I get a new pair of shoes. While it seems like a small gesture, it was certainly thoughtful and I am very appreciative of it! 27 Nursing Times Vol 4, No 4 Gilian Poon Inpatient - Ward 8C (Female Surgical) Gillian was one of the first nurses who came to my care when I first got admitted and I have to say she was very attentive to my condition and came in often to make sure I was okay. Whenever I had questions, she would also answer them, which made me feel very assured. I loved how swiftly she moved too, which showed her expertise. It was so fitting that she was the last nurse on my discharge day. Even when she was giving me lots of information, she made sure to pause after every topic so that I would not be overwhelmed. Thank you, Gillian! I would like to congratulate Nurse Rose on the outstanding and exceptional service that she has rendered during my visits to Raffles Skin & Aesthetics. My baby was attended to by Staff Nurse Suni Matthew from the maternity ward / nursery and I would like to recognise and compliment Suni for her care and excellent hospitality, which have exceeded our expectations. She assured and guided us on how to care for our baby properly. Mathew Suni Inpatient - Ward 8C (Female Surgical) I would like to give a special shoutout to Sister Aminah. She was the first familiar face I saw when I went into labour because she was the one who was conducting the antenatal class that I attended back in July. Her infectious smile gave me the reassurance that everything would be okay while I was pushing the baby out in the delivery room and wheeling me into our room after I delivered the baby. She was also the nurse who handed my baby to me once I got stitched up by Dr Han, which I found very touching and even took our first family photo. Aminah Binte Ramli Inpatient - Ward 8 (Labour Ward) Chung Poh Yoke Idrina Binte Ismail Ng Bee Ling Inpatient - Day Surgery The moment I stepped in the day surgery ward I was greeted warmly. I noticed that all the three nurses working there were lively and had lots of passion and in their work. They genuinely cared for the well-being of their patients. They also had good anticipation of patients’ needs, as they constantly kept an eye on me and whether my drowsiness was wearing off. Kudos to all of them. Tracy Teo Senior Nurse Clinician Ward 10 I had the privilege of working at National University Hospital for seven years in the renal ward. During the course of my service, I completed my Advanced Diploma in Nephro-urology Nursing and was also trained as a Clinical Instructor. My next career opportunity was with the National Kidney Foundation, where I worked as a Dialysis Centre Manager for three years, after which I started working as a Peritoneal Dialysis (PD) Community Support Programme Nurse Clinician for 11 years. After that, I joined Khoo Teck Puat Hospital as a Nurse Manager in one of the specialist outpatient clinics. During the COVID-19 outbreak, I was assigned to set up and manage the Emergency Department P3. Prior to joining Raffles Hospital, I was working at Ang Mo Kio Thye Hwa Kwan Hospital as a Nurse Manager. I was fortunate to be able to work in the acute, step-down care, as well as community care throughout my nursing career. It gave me a broader understanding of Singapore’s healthcare system. In my current role at Raffles Hospital, I am one of the ward’s supervisors at Ward 10, as well as a PD Nurse.

I dedicated 15 years of my career in a restructured hospital before I left. During which I completed my Advanced Diploma in Nursing (Medical-Surgical). Over the course of my career, I have encountered both joy and sorrow, achievements and setbacks. But never have I once thought about giving up nursing. Raffles Hospital has bestowed upon me a remarkable opportunity to embark on a fresh journey as a leader. I have joined the Raffles family for six months now. Before joining Raffles Hospital, I have worked in restructured hospitals and in a local polytechnic. I had the opportunity to guide and supervise students when I was a Clinical Instructor, and this made me want to embark on the education track. The presents, friends, blockbuster movies screen on TV, choral singing in church, and NO school! However, things changed when I was called to be a nurse. Working shifts, managing my own finances, missing out social events, and taking steps towards adulting. Looking back, I have been receiving so much from the people I loved. My family who brought me up to be a responsible adult, the church that I have grown up in to be a spiritually mature Christian, preceptors who guided me when I was a student nurse, and Raffles Medical Group that had supported me financially for my studies. Now it is the time of giving to all who I adore. Christmas is not about giving and receiving gifts. It is about the birth of Jesus Christ whom God had sent into this fallen world to be the light for all of us. Therefore, with this spirit of sacrifice, I also celebrate it by giving the light of love to those who need it most.
