Needlecraft School Policies

Children’s Classes
"   One Session = 5 Classes. From our scheduled classes, choose the day and time that work best and
student will attend that class for 5 consecutive weeks.

Missed Classes
"   I must be notified of absences if your child cannot attend class. (Make-up Class - Your child can attend
class on a mutually agreed day or time within the 5-week session.)
"   If I am not notified prior to class start time that your child will be absent, you will be charged for
the missed class.

Class Registration
"   In order to secure a spot for your choice of class day and time, registration fee and form must be
submitted one week prior to start date.
"   Registration fee includes class reservation, sewing supplies (scissors and metal gauge), and materials
for first project. Material costs not included for additional projects.
"   Tuition for the session is due on the start date.

Try-It-Out Class
"   We offer a one-time trial class for $25.00.
"   Please call to reserve a spot (201) 444-2976.

Adult Classes:
"   I must be notified if you are not attending a scheduled class. If I am not notified prior to class start
time that you will not be attending, you will be charged for the missed class.
"   Any missed classes may be made up on an alternate day during the 5-week session.
"   Single-class reservation requires a $15.00 deposit. Deposit non-refundable if not notified of
cancellation or absence before the start of class.

Private Lessons:
"  Private lesson reservation requires a 50% deposit. Deposit non-refundable if not notified of
cancellation or absence before the start of lesson.

Waiver of Liability, Release, Assumption
of Risk and Indemnity Agreement

The purpose of this Agreement is to exempt, waive and relieve Needlecraft School LLC
from liability for personal injury and property damage, including negligence, if any, of
Needlecraft School, LLC. Needlecraft School, LLC includes its members, officers,
employees, agents, and volunteers.

The Student and the parent(s) or legal guardian(s) of the Student, if applicable, waive,
release and relinquish, any and all claims for liability and cause(s) of action, including
for personal injury, property damage or wrongful death occurring to the Student,
arising out of participation in any activity of Needlecraft School, LLC or any related
activity, whenever and wherever they occur, and for such period the activities may
continue, and by this Agreement, any such claim, rights, and causes of action that the
Student and his/her parent(s) or legal guardian(s), if applicable, may have, are hereby
waived, released and relinquished, and the Student and his/her parent(s) or legal
guardian(s) does (do) so on behalf of The Student, Student’s parent(s) or legal
guardian(s), and their heirs, executors, administrators, and assigns.

Student and his/her parent(s) or legal guardian(s) acknowledge and understand all
risks relating to participation in any activities of the Needlecraft School, LLC, and
understand that such activities involve risks to Student’s self, including bodily injury,
partial or total disability, paralysis and death, and damages, which may arise
therefrom and that Student and his/her parent(s) or legal guardian(s), if applicable,
have full knowledge of said risks. These risks may be caused by the negligence of the
Student or the negligence of others, including Needlecraft School LLC. The Student
and his/her parent(s) or legal guardian(s), if applicable, further acknowledge that
there may be risks and dangers not known or reasonably foreseeable at this time.
Student and his/her parent(s) or legal guardian(s), if applicable, acknowledge,
understand and agree that all of the risks and dangers described throughout this
Agreement, are included within the waiver, release and relinquishment described in
this Agreement.

If the law in any controlling jurisdiction renders any part of this Agreement
unenforceable, the remainder of this Agreement shall nevertheless remain enforceable
to the full extent, if any, allowed by controlling law.

Student and his/her parent(s) or legal guardian(s), if applicable, agree if any claim for
Student’s personal injury or wrongful death is commenced against any or all members

of Needlecraft School LLC, Student and his/her parent(s) or legal guardian(s), if
applicable, shall defend, indemnify and save harmless Needlecraft School LLC from
any and all claims or causes of action by whomever or wherever made or presented for
the Student’s personal injuries, property damage, or wrongful death.

The Student and his/her parent(s) or legal guardian(s), if applicable, acknowledge that
they have been provided with and have read this Agreement and have not relied upon
any representations of Needlecraft School LLC, that they are fully advised of the
potential dangers of participating in Needlecraft School, LLC activities or related
activities, and have read this Waiver of Liability, Release, Assumption of Risk and
Indemnity Agreement and fully understand its contents. The Student and Student’s
parent(s) or legal guardian(s) signs this Agreement of his/her own free will.
