Dear Parent of Laylah Manning:

We welcome Laylah to TutorByDenise...

We are dedicated to helping meet her needs and building her confidence. In
order to develop a Learning Plan specifically to meet her skill set and build
her confidence, a 3rd – 5th Grade Math Diagnostic Test will be given to her
during her initial session on Saturday in September 5, 2020 at our virtual
classroom Zoom.us/j/2460782438 with a password of 429875.

You will receive a detailed Math Diagnostic Analysis as well as her
Diagnostic Results in a separate email within 3 days. If you have questions
regarding her assessment please feel free to call, email, or text. Her next
steps as well as her Improvement Learning Plan will be included with her
Diagnostic and Analysis results.

After both you and she review the Learning Plan, send an email or text
stating that you both agree with the Learning Plan. I can forward the
Learning Plan to her teacher if you like.

• Pencil/pen
• Highlighter
• 1 and ½ inch 3- ringed notebook binder
• Paper
• Index cards
• Any math homework or math assignments
• A willingness to learn

Maury’s Diagnostic Session is scheduled for September 2020:
• Day: Sat. 5, 2020 Time: 1:00pm-2:00pm Location: Virtual Online

General sessions will also be held weekly online in our virtual classroom in
Zoom.us/2460782438 with password 429875 during the same day and time
unless noted. If you do not already have the free Zoom Cloud Meeting app on
your phone or tablet please download it now. Additional sessions may be
added upon request or recommendation

Late and Cancellation Policy
• Please notify us at least two hours in advance if you are going to be late
• Please notify us of any cancellations at least 4 hours in advance
• Please call 404-939-3806 for notifications
• Failure to notify us will result in a fee of $20

Notification of Scheduled Sessions
• You will receive a reminder via email the day prior to scheduled session
• You will receive a reminder via text notification 2 hour prior to scheduled session

Payment Policy
• All payments are due prior to the session
• ‘Cash, Cashapp ($TutorByDenise), Credit, and Debit are the only forms of
acceptable payment
• Credit and Debit Card payments can be made by:
o Phone at 404-939-3806
o Submitting payment at session,
o Completing Invoice (sent day before session)
• Your payment for pre-assessment session is $65 (1-hour session)
• Your payment for each standard session is $45 (1-hour session)

Follow-Up
You will receive an analysis of her strengths and areas of opportunity after
each general session. You will also receive recommendation(s)to help her
learn each concept, when to apply the formulas, how to calculate problems,
and math strategies as well as 10-15-minute homework due daily.

To ensure we assist you as best as possible please complete the enclosed
forms: “Student Information” Form enclosed in your email.
