Thank you for selecting TutorByDenise to help your daughter with 8th grade Math.
We know Middle School can be a little challenging at time for some students. We
understand the importance math plays as a building blocks for his High School
years. We are confident that you will be very satisfied with the services that we
offer.

Included in this packet are procedures and policies as well as her Diagnostic Testing
logistics. Zaharia will complete the Diagnostic during her initial session. Please
review the procedures and policies as well as the expectations. If you have questions,

• 1 ½ inch 3 ring note binder with labeled tabs and dividers (label names

included in email)
• Notebook with pockets
• Index Cards on ring
• Any questions regarding homework or assignments given by

• Sunday Sept 20, 2020 3:45pm – 4:45pm Virtual Classroom

Zaharia’s General Sessions are scheduled
• Days: TBD Time: TBD Location: Virtual Classroom

Please mark this date and time on your calendars. The virtual classroom is located
on Zoom Cloud Meeting App. Please download it prior to the start of class. Your
zoom classroom is zoom.us/j/2460782438 passcode 429875. Additional sessions may
be requested. Session availability is based on a first come firs serve basis.

We can discuss when her next session will be after the diagnostic testing results.

Late and Cancellation Policy
• Please notify us at least two hours in advance if you are going to be late
• Please notify us of any cancellations at least 24 hours in advance
• Please call 404-939-3806 for notifications
• Failure to notify us will result in a fee of $20.00

Notification of Scheduled Sessions
• You will receive email notification reminders one day prior to scheduled session
• You will receive text notification reminders 2 hours prior to scheduled session

Payment Policy
• All payments are due prior to the session
• ‘Cash, Cash App, Credit, and Debit are the only forms of acceptable payment
• Credit and Debit Card payments can be made by:
o Phone at 404-939-3806
o Submitting payment at session
o Paying Invoice (sent day before session)
• Your payment for Diagnostic Testing session is $65/hr. (1-hr session)
• Your payment for each session is $45/hr. (1-hour session)

Follow-Up
You will receive an analysis of Zaharia’s strengths and areas of opportunity
1-day (business days) after her Diagnostic Testing. You will also receive
recommendation(s) to help her practice areas of opportunities discussed
during the sessions, as well as daily homework.
