Thank you for selecting TutorByDenise to help your daughter with her 8th grade
Math. We know Middle School can be a little challenging at time for some students.
We understand how important Math is in the building blocks for her High School
years. We are confident that you will be very satisfied with the services that we
offer.

You will receive her Welcome Packet with this letter as well as an Authorization
Student Release Form. A 8th Grade Math Pre-assessment Test will be administered

To ensure we assist you as best as possible please complete the
Organizational Skills Profile” as well as the “Student Information” Form
enclosed in your email. She will also receive a 8th grade pre-assessment
quiz during her initial session.

Please bring the following items to each session:

 Highlighter

 1 ½ inch 3 ring note binder with labeled tabs and dividers (label names
included in email)

 Notebook with pockets

 Index Cards on ring

 Calculator

 Any questions regarding homework or assignments given by

Hwy 92 McDonalds which is located at 5330 Campbellton-Fairburn Rd. Fairburn,
Ga. unless otherwise notified. Additional sessions are available on a first come
basis.

Late and Cancellation Policy
 Please notify us at least two hours in advance if you are going to be late
 Please notify us of any cancellations at least 24 hours in advance
 Please call 404-939-3806 for notifications
 Failure to notify us will result in a fee of $20.00

Notification of Scheduled Sessions
 You will receive email notification reminders one day prior to scheduled session
 You will receive text notification reminders 1 hour prior to scheduled session

Payment Policy
 All payments are due prior to the session
 ‘Cash, Credit, and Debit are the only forms of acceptable payment
 Credit and Debit Card payments can be made by:
o Phone at 404-939-3806
o Submitting payment at session
o Paying Invoice (sent day before session)
o Completing PayNow at www.tutorbydenise.com (Tutoring Services
offered page, scroll down the page until you see PayNow, click button)
 Your payment for each session is $40 (1hour session)

Follow-Up

You will receive an analysis of her strengths and areas of opportunity after
each session. You will also receive recommendation(s) to help her practice
Math concepts and application discussed during her homework sessions. She
will receive 15-20 minute of homework given by TutorByDenise which is due
daily. We will create an Improvement Action Plan, Milestones, and
Organizational Time Plan her initial session. The Organizational Time Plan
will include note-taking skills.
