SafeSTEPS
Digest

October - December 2020

2 SafeSTEPS Digest

Welcome to our fourth quarterly print newsletter,
SafeSTEPS Digest, which includes highlights
from our SafeSTEPS Pulse e-newsletter over the
last three months. As we begin a new year it is a
time to reflect on how far we have come on our
SafeSTEPS journey, particularly with the extra
challenges the world faced in 2020.

When we launched SafeSTEPS almost two years ago, we had a vision in mind for the future.
We wanted Ma’aden to be a place where safety comes first, always. A place where safety is the
most important precondition for any work we do and where people are able to speak up if they
feel unsafe. To achieve this, we committed to putting safety at the heart of every decision we
make, ensuring that the only production at Ma’aden is safe production.

2020 brought several challenges to test the
strength of this conviction. Not only did the
COVID-19 pandemic test our crisis management,
but the Ma’aden family suffered three fatalities
which is heartbreaking for their loved ones and
everyone here at Ma’aden.

These terrible events cannot be allowed to happen
again which is why we undertook a root-and-
branch examination of our individual and collective
behaviors that created a work environment where
fatalities were possible. It is clear that we must all
“commit”, both on a personal and corporate level,

October - December 2020 3

to change our behaviors and manage-out the risk of fatalities, injuries, and every EHSS incident
at Ma’aden. This is not a small commitment.

We are expecting a huge amount from every one of you to ensure our safety processes and
rules are followed. At the same time the leadership team has committed to taking the lead in
this ongoing pursuit of sustaining an injury-free work environment, constantly asking three key
questions

(a) are our people at the front line sufficiently trained on the tasks they perform?

(b) do we have the required set of procedures and work instructions to ensure all activities are
done safely?

(c) do we have the check-and-balance systems that ensure everyone is following the rules?

Despite 2020 being a challenging year I am thankful and appreciative of the progress we made
together. We improved our organizational effectiveness, we strengthened our EHSS systems
and increased the leveraging of our assets to achieve higher level of performance. I am also
pleased that we all realize that we only scratched the surface and need to do much more to
achieve our vision.

The response and enthusiasm from you all to help drive this vision forward has been fantastic.
From adopting our Life Saving Rules, to entering our We Speak Safety quizzes, participating
in activities as part of safety campaigns, and much more, the way we are thinking and
talking about safety has changed for the better and although we have a long way to go it
is encouraging. Our progress is clear through our achievements – for example, our Total
Recordable Incident Rate (TRIR) target for this year was 0.29, while our actual TRIR was below
the target at 0.26. This lower TRIR shows that incidents across Ma’aden are happening less
frequently.

Speaking of driving the vision forward - as you all know, we recently launched the MSHEM
system (Ma'aden Safety, Security, Health, and Environment Management), our organization-
wide standards for EHSS management. The adoption of the system will remain a major focus
for us in the year ahead. Thank you all for the wonderful response so far, I am confident we'll
see the same level of commitment going forward as the implementation picks up pace.

So, as you enjoy this quarter’s Safety Digest please remember that safety is all of
our responsibility. It’s not just up to the EHSS team, supervisors, or your leaders
to ensure you are safe - every one of us has a duty to protect not only ourselves
but everyone around us. We have proven during an extremely challenging year
that we are stronger when we work together, so let’s stay united and stay safe!

SafeSTEPS
in Action

In Quarter 4 we launched our new SafeSTEPS campaign – SafeSTEPS in Action
What is it all about?

This campaign is all about SafeSTEPS in action: having safety as a
mindset, following SafeSTEPS rules and guidelines, and applying
our understanding of safety to everything that we do. Especially
in light of the ongoing global COVID-19 outbreak, it is crucial that
we keep up the good work by social distancing, wearing masks,
sanitizing, and following all other health guidelines.

October - December 2020 7

As part of the SafeSTEPS in Action campaign,
banners, posters, digital screens, and more were
displayed across HQ and all our sites. If you
haven’t already looked at them, check them out!

What are
the campaign
messages?

We are all health
and safety role models

Safer together,

To kick off the campaign, our CEO, Mosaed
Sulaiman Al Ohali also sent a message to
all employees. If you didn’t see it, you can
read it again below:

As you know, here at Ma’aden we prioritize EHSS - Environment, Health,
Safety, and Security. In line with this focus and following the great success
of our Safety is Personal Campaign, we are excited to launch our latest
SafeSTEPS campaign, SafeSTEPS in Action.

Through this campaign, we remind ourselves that safety is our top priority by
putting SafeSTEPS in action: having safety as a mindset, following SafeSTEPS
rules and guidelines, and applying our understanding of safety to everything
we do.

The management team and I are fully committed to making Ma’aden the
safest place in the world for you to work. Following my recent site visits
across our operations to discuss P-EHSS I was extremely pleased that this
commitment is shared throughout the company and that we are seeing
our safety culture take root. SafeSTEPS in Action is the next stage in the
transformation of our safety culture.

We are all health and safety role models, and we inspire those around us to do
the same. I encourage all of you to lead by example, following our established
safety directives while continuously adapting to new ways of working in line
with the evolving COVID-19 situation.

As part of SafeSTEPS in Action, we are exploring new virtual initiatives that we
can all be part of. Please keep an eye on our weekly newsletter, the SafeSTEPS
Pulse, for updates.

If we excel in our safety performance, we will excel in everything else we
do. That is why I would like to thank you all for your ongoing support of our
SafeSTEPS Program – our people are what makes us great.

Introducing our new Ma’aden
Safety, Security, Health &
Environment Management
standard (MSHEM)

Ma’aden has developed comprehensive EHSS management standards for
the entire organization. The standards will be called MSHEM (Ma’aden
Safety, Security, Health & Environment Management).

These standards cover the applicable regulatory requirements, applicable national
and international EHSS standards, and the best industry practices. There are 15
elements and about 70 sub-elements covering key EHSS subjects for effective EHSS
management. MSHEM was developed with the active involvement of EHSS subject
matter experts from Ma’aden Corporate and all the Affiliates.

These standards were developed with the aim to create “One EHSS management
system for Ma’aden’s business“ with a best-in-class EHSS culture, and to achieve
our vision of being globally recognized as a leading company responsible for the care
and protection of human capital working at Ma’aden, its assets, the environment,
and the neighbouring communities.

As part of the initiative to engage with the key stakeholders who will be involved in
the MSHEM system’s implementation, the Ma’aden corporate EHSS team organized
ongoing workshops to introduce the system.

So far, we have seen great engagement with the system. We have
already held sessions for over 130 element and sub-element
administrators and other key individuals in 2020, conducted in
a safe (socially distanced) in-person manner or through Webex
to maximize the number of attendees. Keep an eye out for 2021
updates, as our training sessions continue to pick up pace.

Our new system which standardizes all of our safety procedures across Ma’aden.

This system covers all elements of environment, health, safety, and
security (EHSS) to help us achieve a best-in-class safety culture. The
MSHEM system consists of four key pillars; there are 15 core elements
and 70 sub-elements designed to enable our EHSS vision.

Safety Attitude

There are many excuses someone
will give for not working safely,
which often lead to workplace
accidents or injuries.

Some common excuses include I didn’t
know; I didn’t have time; I lost my PPE, etc

“It can’t happen Has a mindset that is set on not completing a task safely
to me” mindset Relies on luck to keep safe while on the job
is when someone:

Sooner or later, “If only I had worn my safety glasses, I wouldn’t have
the person lost my eye“
with a “It can’t
happen to me” “If only I had walked instead of run, I wouldn’t have
mindset will tripped and broken my leg“
end up saying:
“If only I had taken my ring off, I wouldn’t have lost my
finger on the machine“

The next time you find yourself saying, “It can’t happen to me,”
remember that anything can happen to anybody, anytime, anywhere, if
they act in an unsafe manner or are exposed to an unsafe condition. If
you see someone working in an unsafe manner, speak with them about
it. If you see an unsafe condition, report it.

October - December 2020 19

We all have a role to play in
SafeSTEPS so we asked members of
the Ma’aden community, including
safety ambassadors, to tell us a little
bit more about themselves and why
SafeSTEPS is important ...

20 SafeSTEPS Digest

“I wanted to be a Safety Ambassador
to give my support to driving the
safety culture at Ma’aden and help
explain and enforce the rules with
others.

“My top tip for keeping safe at
work is to focus on teamwork and
good communication with your
colleagues.“

Florival Domingos Lifanica

Central Workshop Supervisor

“I wanted to become a SafeSTEPS
Ambassador because it allows me to
seek more knowledge and widen my
experience while boosting my self-
confidence, especially when it comes
to safe decision making. I also wanted
to become a role model for my fellow
workers at JRAR and Ma’aden As’suq
site. For me, SafeSTEPS is one of the
best guidelines for ensuring every
individual is working safely.“

Radzfar Gawran Sirajan

Safety Officer

October - December 2020 21

“I wanted to become a Safety
Ambassador to spread awareness
about SafeSTEPS and help prevent
any incidents. To me, SafeSTEPS is
a program that will prevent harm
or incidents, Insha’Allah, if everyone
working at Ma’aden follows it.“

Huda Al Mutiri

Internal Auditor

“Being a safety ambassador is like a
calling, to spread and share a good
practice of safe working habits to
everyone with proper guidance through
SafeSTEPS. I consider SafeSTEPS to be
a powerful tool for everything,
in terms of safety.“

Winter Driving

Winter can be a challenging season due to
unpredictable climate and driving conditions,
so we must be prepared and proactive.

Before driving, check traffic reports and the weather
forecast. If you plan ahead, you can stay safe.

Driving in Fog

Foggy weather is when visibility is > 200 meters.
If you have to drive, take the following precautions:

Drive with lights on low beam
Reduce your speed, and watch your speedometer
Listen for traffic if you cannot see it
Use wipers and defrosters as necessary
Use painted road markings as a guide
Never stop on a road

October - December 2020 31

Three key elements of safe winter driving:
Stay alert | Slow down | Stay in control

Driving in Rain

Skidding occurs due to slick conditions on the road.

Hydroplaning happens when the water in front of your
tires builds up faster than your car's weight can push it
out of the way.

To prevent an incident, take the following precautions:

Use anti-lock brakes Steer the car in the direction
Never use cruise control in the rain of the skid
Reduce speed
Avoid standing water

Chemical
Processes
Safety

What happened?

Two contractors were performing welding work on top of a
slurry tank
The sparks from the welding work mixed with vapors inside
the tank, causing an explosion
Sadly, this resulted in loss of life for one contractor, and
serious injuries for the other

What were the lessons learned?

An investigation found that the contractors did not have
adequate knowledge of chemical processes
Workers must have a permit and be competent for the task
ahead
Risk assessments must be carried out prior to starting
work; in this case, the tank should have been checked for
flammable vapors

October - December 2020 35

What does this mean for Ma’aden?

This industry peer’s incident shows the
importance of carrying out proper risk
assessments and ensuring workers are
competent and fit-for-work
At Ma’aden, we have clear rules for
handling chemicals safely and for hot
work
We should only ever carry out a task if we
are fully trained and qualified to do so

36 SafeSTEPS Digest

Chemical
Hazard
Accident

What happened?

A gas detector failed to detect a flammable substance on a
sampling tube
Hot work began with the presence of the flammable
substance, resulting in an explosion
Sadly, this resulted in loss of life

What were the lessons learned?

It is important to follow the correct systems for gas
detection
Gas detectors must be properly checked each day
Employees must be properly trained on how to use the
equipment

October - December 2020 37

What does this mean for Ma’aden?

This industry peer’s incident shows the
importance of following chemical safety
rules
At Ma’aden, we have clear rules for
handling chemicals safely
We also carry out risk assessments,
make sure we are competent and fit-for-
work, and follow the 7 Life Saving Rules

38 SafeSTEPS Digest

Loss of
Containment
Incident

What happened?

A loss of event occurred (when a fluid that is usually
contained somewhere escapes from that place), creating a
vapor cloud that turned into a fire explosion
Two employees and a security contractor were injured –
however, fortunately, there were no casualties
The second contractor also fell in trying to save his
colleague; sadly, they both lost their lives

What were the lessons learned?

The pipes were found to have ‘popcorn polymer’, a solid
substance that can form in process vessels containing a high
concentration of butadiene
All pipes and equipment should be checked to ensure no
blockages or popcorn polymer are present

October - December 2020 39

What does this mean for Ma’aden?

This industry peer’s incident shows the
importance of following rules for working
with pipes and equipment
At Ma’aden, we follow the 7 Life Saving
Rules, as well as safety pauses on topics
such as equipment and piping safety
We also follow site-specific precautions;
in the case of the peer site, their fast
evacuation meant there was no loss of life

40 SafeSTEPS Digest

Confined
Space
Accident

What happened?

Two contractors were performing work on a hydro-cracking
unit, raising a pipe injected with nitrogen onto a reactor (in a
confined space)
One of the technicians fainted while picking up a roll of tape,
falling into the reactor
The second contractor also fell in trying to save his
colleague; sadly, they both lost their lives

What were the lessons learned?

Confined spaces should be labeled, including the types of
hazards present
The work permit should have specified that nitrogen was
present
Entry into a reactor requires a specialized team; employees
must be competent and fit-for-work before starting the task

October - December 2020 41

What does this mean for Ma’aden?

This industry peer’s incident shows the
importance of following rules for working
in confined spaces, and of being properly
trained for the task at hand
At Ma’aden, we follow the 7 Life Saving
Rules, which include rules for confined
space entry and being competent and fit-
for-work
We also properly fill out work permits as
per our rules and guidelines

42 SafeSTEPS Digest

Chemical Safety

Chemical exposure may result in serious health problems. This is why we
have guidelines for who can work with it and how to be safe while working.

Keeping safe around chemicals S
S
Always know where the nearest safety shower / e ye wash is in case of an emergency.

Always wash your hands and any other areas that may have come into contact after
handling chemicals.

In case of a spill, remove all clothing and thoroughly wash all areas where the
chemical contacted your skin.

Always wear proper PPE such as a tightly fitted respirator, fit-for-purpose rubber
gloves and arm protection, safety goggles, and protective clothing.

Conduct routine inspection of the area where chemicals a re stored and/or handled
to ensure that they are stored in a safe manner. Correct all non-compliance issues
immediately.

removed his accessories prior to
starting work

What
should
you do?

1. Nothing – it’s none of your business.
2. Politely explain that wearing accessories when

operating machinery of any type is a risk and ask him to
remove it before he begins work
3. Inform the supervisor if your colleague doesn’t listen
4. Both answers 2 and 3

The correct answer is of course 4 –
both politely explain and inform the
supervisor if your colleague doesn’t
listen.
