In this specialist hospital, patients coming from the Nurpur, Indora, Jawali, and Bhatiyat Assembly constituencies are having trouble getting the tests they need done. Patients are compelled to go to private laboratories in Pathankot (Punjab), Jassur, and Nurpur in order to have the test because the state Health Department has been unable to either appoint a radiologist or make other arrangements. The ultrasound examination is provided at no cost by public hospitals, however commercial laboratories often charge between Rs 700 and Rs 800 for it. Immunotherapy: This is definitely one of the path breaking discoveries in the past decade. Tumors grow in our body by evading / misleading our immune system. By 2025 the annual incidence of cancer in India is projected to be 1.5 million. Every year 8.5 lakh patients die due to cancer in India. This makes cancer a big problem. There are both non modifiable risk factors in the form of gene associated and familial cancer and modifiable risk factors which may predispose to cancer. With latest advances in health care practice, many of these cancers are curable if they are detected in early stages. Around 45% of cancers arise mainly from oral cavity, breast, female reproductive system, lung, stomach. These cancers are mainly caused by many modifiable risk factors like tobacco consumption, alcoholism, lifestyle problems, social factors, and also infections like human papilloma virus. Worldwide cancer prevention strategies focus on these modifiable risk factors through primary prevention and secondary prevention. Primary prevention involves the avoidance of risk factors that may cause cancer. This includes cessation of smoking, reduced consumption of alcohol, healthy lifestyle changes, vaccination against Human Papilloma virus. Diet plays a major role in cancer prevention. American Institute of Cancer research (AICR) recommends Low alcohol, Low Calorie, Low fat, High dietary fibre, Low red meat and Low processed meat in diet for prevention of cancer. Government of India in 2003, passed Cigarettes and other tobacco products act (COTPA act) which prohibited advertisement of tobacco products, smoking in public places, sale of tobacco products near educational institutions and also mandated depiction of statutory warnings on tobacco packs and tar and nicotine contents along with maximum permissible limits on tobacco pack. Human Papilloma virus infection is the most common cause of cervical cancer in women. Various vaccines are developed in bivalent, quadrivalent and nanovalent forms providing protection against human papilloma virus infection. 80% of cervical cancers can be prevented with HPV vaccination to girls between 9-14 years of age. Government of India launched first Indigenous Cervical vaccine which is a quadrivalent vaccine in September 2022 thereby reducing the cost burden of vaccination on individuals. Whenever we are treating a patient with cancer, there are three goals that we want to achieve - first, save the life; second, save the eye; and third, save the vision of the patient. Despite our best efforts to save a life in a significant percentage of patients, eye removal surgery is required. Generally, this is because the patient comes to us at an advanced stage. Eye cancer can also spread from one eye to the other eye or to other parts of the body if not diagnosed early and treated appropriately,” says Dr Swathi Kaliki, Head, Operation Eyesight Universal Institute for Eye Cancer, L V Prasad Eye Institute. “Certain eye cancers may be related to smoking, virus infection and excessive exposure to ultraviolet light. But as soon as you notice these symptoms it is important to get your eyes checked by a doctor and seek timely treatment and care. It is important to be aware about these symptoms that otherwise could be easily dismissed as an infection. Treatment The treatment of cancer depends on the stage at which the patient presents. Different forms of therapy are available. For smaller tumours, medical therapy or laser would suffice. The study has also shown that India still has a long way to go to reach the goal of zero catastrophic costs for all patient groups, as stated in the END TB Strategy. As an outcome of the understanding, the scientists have recommended a few strategic solutions to reduce the catastrophic impacts of TB in India. Reducing delays in diagnosis and treatment initiation (patients visit on average 12 healthcare providers before getting diagnosed with TB) can reduce the costs associated with TB treatment, its aftermath, and its related economic consequences. Creating awareness about the symptoms of TB in the community and availability of free diagnostic and treatment facilities in the government sector is crucial in this regard, they say. They also recommend that policy changes must ensure that the livelihood of TB patients is not disrupted and hence, the changes should include the provision of job security, additional food support, better management of direct benefit transfers, and improved medical insurance coverage. Other than The George Institute for Global Health, India, researchers from Indira Gandhi Government Medical College in India, the University of New South Wales (Australia), and the London School of Hygiene and Tropical Medicine (UK) also participated in this study. Microbioz India, February 2023 | 40 The study, conducted between 2019 and 2021, examined the experiences of 829 adult patients who had TB that is responsive to medication (drugsusceptible TB). Study participants were from general population and from two groups who are at high risk of having TB because of their poor living conditions and nutrition: people living in urban slum areas and tea garden areas. Each participant was interviewed three times: once at the beginning of treatment (0-2 months) then during the continuation phase of treatment (5-6 months) when they were almost finishing their treatment, and finally post-treatment i.e., 8-12 months after treatment was completed. Elaborating on the details of the survey, Dr. Susmita Chatterjee said, “The collected data covered socio-economic conditions, employment status, patient and household income, out-of-pocket expenses, and time spent on outpatient visits, hospitalization related to TB, TB drug pick-up, medical follow-ups after initiation of TB treatment, expenses on additional food during TB treatment, and coping strategies such as dissaving, borrowing, and selling assets.” The observations were disturbing as the total cost of TB treatment from the onset of symptoms to one-year post-treatment ranged from Rs. 26,500 – 30,500 per patient despite free diagnosis and treatment provided by the government. Of this, 32% to 44% of costs were incurred in the pretreatment phase (i.e., from the initiation of TB symptom till initiation of treatment), while 6%-7% were incurred in the post-treatment phase. The unemployment rate was seen to increase from pretreatment (before TB) to the intensive phase (i.e., at the beginning of TB treatment), but decreased during the continuation phase (i.e., at the end of TB treatment), although it remained above the baseline even after about one year of treatment completion. The low income of the patients along with high unemployment because of TB forced them to borrow money and/or sell / mortgage their personal belongings during treatment which resulted in a high outstanding loan in the posttreatment phase. Further, to manage the outstanding loan, they had to borrow/sell/mortgage during the post-treatment period as well. Apart from that, there were other economic consequences that included reduced food consumption for family members as they ensured proper food for patients; inability to pay regular bills such as electricity, mobile, cable etc; buying regular groceries on credit; not being able to attend events like weddings or other social functions as they did not have money to buy gifts or for travel both during treatment and post-treatment period.
