initial notification of COVID19 Probable Case in Gas
Processing Santong to all workers

Details of Personnel : One (1) Construction Engineer of TBK

Incident : Probable Case COVID19

Summary : One (1) C19 probable case was reported yesterday at

1830hrs involving CMI sub-contractor personnel after

performing RTK Antigen test which turned out positive. IP is

now being quarantined under KKM at Kuala Terengganu and

set to undergo 2nd test (RT-PCR) tomorrow morning for

confirmation.

He paid a visit to his sister in KL

Jan 11, 2021 He attended HSE induction at CMI site office together with the other 13 personnel

Participated in JHA review session at NAP PMT office. Attended by 9 other personnel.

He has been informed that his sister was positive after performing Covid-19 test earlier on.
Jan 14, 2021
He was immediately being brought to Kuala Terengganu to perform self quarantine and
underwent RTK-antigen test.

Sample taken from IP was positive and quarantined at Kuala Terengganu by KKM. He’s now
waiting for 2nd test on 14th Jan 2021.
Undergo RT-PCR test in Kuala Terengganu.
All those 22pax close contact personnel also undergo COVID19 screening at Klinik Kesihatan
Kuala Dungun / Ibnu Sina Clinic Kemaman
