2. Sequence of events

- After finishing cleaning inside of the reaction tank, Mr. B & Mr. C came out first and followed by Mr. A (Victim) from the tank
- While Mr. A came out through the ladder , he initiate to remove the lamp with holding the live part of lamp and then got electric
shocked
- Leader turned off the electric power and then Mr. A falls into the tank
- Mr. A immediately evacuate from the tank and brought the hospital for further treatment but unfortunately he passed away

- This lamp use for additional lighting during cleaning inside the tank
- PPE of Victim : Safety Helmet, Glove (wet condition) , Safety Shoes
- Electric power source for the lamp without ELCB

3. Causes of accident & Countermeasures

Method - Light bulb and fitting were not suitable for wet area. - Adding lighting point in floated area (2 points to 8 points)
- Low capability to recognize electrical hazard & and floodlight toward the reaction tank
Environment safety awareness --> don't have competence in this
area. - Revise reaction tank maintenance standard
- Unclear work instruction and safety key point for (adding safety point and control of appropriate work
cleaning reaction takin adequate Unclear rule for equipment)
working in confined space - Create Rule and Control Check sheet for working in
confined space
- 4S condition in reaction tank area - Improve 4S in reaction tank area
