Patient Instruction for Admission Patient

1) General information

To continuous of care. Please provide your basic information to doctor, nurse or hospital staff know the
following information.

 Chief Complaint
 Present Illness
 Underlying (Disease/Illness/Condition) Past and Personal History
 Allergy: allergies to food or other substances.
 Family History
 Current Medications

2) Risks and Side Effects of the Treatment

3) Patient Instructions

1. Please contact the staff of the department indicated on the appointment card. Nurses will do
assessment again such as take a Vital Signs, Do Blood investigation, send you to do Imaging
Investigation according to the doctor's treatment plan before carry you to your room.

2. When you arrive at the inpatient department, you will receive instructions on how to stay in the
hospital in accordance with the Recommendation Form. (Inpatient admission F/M-02.1-IPD-010)

3. In case of admit for surgery

 Staff will give specific advice on preparation before receiving the anesthesia service for
surgery.

 Prepare to clean your body, wash your hair

 No water and food as advised by your doctor.

 In the event of a doctor's appointment to perform procedures Surgery Before your hospital
stay, Please do not makeup your face, nail polish, etc.

4. Giving Consent for Hospitalization
 Your doctor will explain your illness information, treatment plan or procedures to you .Also
Nurses and related staff will give you hospitalization information and expense. Then you
will receive the following form to sign the acknowledgment.
 Consent Form for general treatment When requiring inpatient treatment
 Consent Form for surgery / procedure (in case of surgical / Procedure )
 consent Form the use of anesthesia drugs (In case of need to use anesthesia)
 Consent Form to Disclosure In case you want to receive examination results or medical
documents.

5. Cost Estimation & Billing
Please inform your insurance eligibility by using documents shown or insurance card to the
Registration staff or Inpatient Admission staff. To review of the protection of your insurance costs
and facilitate the payment on the day you discharge from the hospital.

6. Your Belonging valuable items:
 Do not bring your Belonging valuable items. accessories or cash in your Hospitalization
 You can keep your belongings valuable Items in the provided lockers in the room or keep
them according to Advice of the Hospital staff.

4) Visiting/Homecare Guidance

1. On your discharge date.
 If you have home medication, The Pharmacist / The ward nurse will hand over the home
medication and explain about medication intake, side effect and review your
understanding. The ward nurse will review your understanding regards to, post-treatment
care at home, activity, physical limitation, Abnormal signs & symptoms requiring return to
the hospital before the appointment. And provide the appointment slip as prescribed per
physician’ s order

 The nurse will hand over the results of the treatment to you so that you can refer to another
doctor, another hospital in case of an emergency.

2. For those who have given birth Please prepare related document required to report birth as
follows.
 Baby's name (in case of foreigners or a foreign father or mother and want to name a child
that is a name English Language or national language. Spell in Thai Language are required
to inform the birth)
 House registration (for persons with Thai nationality) for adding the child's name in.
 Marriage certificate of both father - mother
 National ID card of both father - mother
