4 Introduction / Background

Data will last longer than systems themselves.

Healthcare should be driven where and when it is need. Policy drawn to strengthen healthcare
systems should be based on data which can describe and demonstrate inequity in access to
healthcare. The world today is overflowing with data; and thus, the ability to utilize it, understand
it, process it, extract value from it and finally successfully communicate it is an important skill
set for all leaders to possess. Data can be converted into information, and this information
only has value if it is able to inspire those in position to make a difference, to develop insights
that can improve service delivery. In order to strengthen surgical services, it is crucial to instil a
work culture of maintaining real time health data that can be utilised to improve infrastructure,
workforce, service delivery and finance. The sustainability of this data driven culture is
dependent on end users understanding the long-term benefits and goals.

1n December 2016 during the surgeons’ annual meet, a resolution was made to collect data
which can be used to describe the burden of surgical services. Data set to be collected
was decided and data collection template was designed using an excel sheet. A medical
officer(MO) was appointed in all specialist surgical unit who was tasked with data collection.
Outlier data was re-examined and validated. The report has thus far highlighted several
variation and challenges faced in the provision of surgical services. We were able to identify
the capabilities and vulnerabilities in the system. There are numerous obstacles leading to gaps
in service, common challenges being lack of experience, maldistribution of specialists and lack
of resources. Information gathered and measured were reflective of workload in the respective
regions.

This report is divided into 2 sections:
• Section A : Inpatient admissions, Outpatient Census, Total Endoscopy, Total Surgery
• Section B : Surgical Procedures (Refer to Appendix 1)

Hospital categorization

Although the hospitals included in this report are all Specialist Hospitals, they differ greatly in

terms of bed numbers, facilities and resources. Therefore, these hospitals are classified into :



a. Big Category Hospital : > 80 surgical beds

b. Medium Category Hospital : 50 – 80 surgical beds

c. Small Category Hospital : < 50 surgical beds

Categorization of the participating Specialist Hospitals into the Big Medium Small (BMS)
category allows for a more objective comparison on performance and outcome.

55 Methodology

Parameters Selection

This report aims to reflect the workload of the surgeons and thus, data collected mainly focuses
on surgeon’s related procedures.

In section A, total numbers for Admission, Outpatient encounters, Endoscopy and Surgery was
selected to reflect the burden of General Surgeons. Each of these parameters are subsequently
used in management and distribution of resources and manpower.

Section B collates the total number of selected surgeon-specific procedures which would be
used as denominator to calculate Post-Operative Mortality Rate (POMR) in the effort to monitor
outcome.

Data Source

The IOES report collates various quantitative Surgical data from Malaysia’s MoH Specialist
Hospitals. Data was collected from 53 Specialist Hospitals. Non-specialist and District Hospitals
are not included in this report as the aim of this report is to reflect specialist workload only.

Data from other surgical specialities (Urology, Neurosurgery, Pediatric Surgery, Cardiothoracic
Surgery, Plastic Surgery) are not included.

As not all hospitals have an electronic medical record (EMR) system, a large proportion of our
data are collected manually via census kept by the ward sister and OT (Operating Theatre)
book census. The heterogeneity of data source is leads to some discrepancies of data which
had to be verified several times before endorsement by the each Hospital’s Surgical Head of
Department (HoD)

Data Collection

Each of the 53 Specialist MoH Hospitals have a appointed Data Coordinator, which consists of
Medical Officers. They oversee collecting each hospital’s department set of data.

A template designed and used for data collection (Appendix 2) is distributed via the central
committee to all the data coordinator to facilitate this process. The completed template is then
reviewed and verified by each HoD before it is e-mailed back to the central committee.

The central committee which consists of the Chief editor and 2 Associate Editors, are in charge
of collating and tabulating these data. Any discrepancies noted during initial analysis of data
will be sent back to the respective Data Coordinator for verification and correction.
