We have noticed that many patients are being discharged from hospital with a urostomy pouch to manage their nephrostomy. Their risk of infection could be raised further due to: ● Non-sterile pouches being placed over the open wound. ● The nephrostomy drain/catheter is immersed in urine and possible sediment. ● Bacteria can back-track up the drain/catheter and ultimately into the kidney. ● Bag capacity is smaller leading to more frequent emptying and the need for a leg bag extension. ● Urostomy pouches usually need changing 2-3 times a week with an impact on patient quality of life and nursing time. Nephrostomy care and reducing the risks Associated risks of a nephrostomy: ● Urine infections. ● Severe cases of Sepsis. ● Dislodging of nephrostomy drains/catheters. ● Unnecessary re-siting of nephrostomy drains/catheters. During 2023, Manfred Sauer UK was contacted by four NHS Trusts in connection with nephrostomy patients who suff ered severe cases of Sepsis. According to the Sepsis Manual, Sixth Edition, there are approximately 250,000 Sepsis cases per year in the UK of which 50,000 (20%) are associated with the urinary tract and approximately 9,700 deaths. Individuals with indwelling lines, drains or catheters are already at a higher risk of Sepsis without increasing the risk further.
● Skin integrity may be compromised due to the large adhesive area around the site and the more frequent changes. ● The inability to fi t a nephrostomy securement device. Is this cost-eff ective? Maybe not when the cost of hospital stays, drain/catheter re-siting, antibiotics etc, are considered. Do you use a connector/adaptor to fi t a leg bag? This may increase the risk of dislodging the nephrostomy drain/catheter and increase the risk of infection. ● The long tube length (between 50cm – 1m long), increases the risk of catching and pulling which can dislodge the drain/catheter. ● Disconnecting the leg bag daily to connect a night bag, increases the risk of infection. ● Two, if not three, connection points (areas for bacteria to enter and/or hibernate) increase the risk further. ● If connecting a night bag to the leg bag at night, there is an increased risk of infection and dislodgement, and it can be a trip hazard.
