Golden Rules - What are they?

• A set of basic rules that we must follow at all times - workers have
been seriously hurt because these basic rules were not complied
with

• Address the high risk activities and common hazards in the work
place – cause of majority of serious incidents and HiPo’s

• Apply to all CHOC work sites and business activities

Golden Rules – Why?

• Analysis shows that the vast majority of injuries and HiPo’s in CHOC
and the broader Oil & Gas industry are the result of non-compliance
with the basic rules related to a small set of high risk activities

• The Golden Rules set the expectations around performance of high
risk activities

• Identified and summarized into ten simple rules that are clear and
easily understood by all

• Gives all personnel the ability to recognize unsafe behaviours
associated with high risk activities and the authorization to
undertake a Safety Stop

Management Expectations

• All personnel must be briefed on the Golden Rules
• The Golden Rules will be included in all induction processes
• Policies, work procedures, practices and programs must be

in place to address the Golden Rules
• All personnel will comply with the requirements of the

Golden Rules at all times
• Non-compliance with the Golden Rules will be investigated

and appropriate actions taken as per the CHOC Fair
Treatment Assessment Procedure

Stop For Safety

I WILL do a Safety Stop if
I see work is not being
performed safely

Safety is not only about taking precautions,
it’s also about taking responsibility.

MINIMUM EXPECTATION

• Stop for Safety is not an option but an obligation - exercise your authority
to stop / intervene when a job is unsafe or potentially unsafe

• Hold pre-job safety meetings with all involved to confirm knowledge,
understanding of the risk and responsibilities associated with a work
activity

• Constantly remind everyone to look-out for the safety of ourselves and
each other

• Re-assess the risk when there is any temporary or permanent change and
ensure a management of change process is followed

• Resume work only after both parties agree the concern has been
addressed; if agreement is not reached the work must remain stopped
until the PIC authorises restart

Approved Permits

I WILL only work with
Approved Permits and work instructions

The permit must be signed off by a
manager or supervisor before work can
begin, ensuring that certain checks are
met and creating a restriction on the task
rather than allowing it to be carried out
by anyone at any time

MINIMUM EXPECTATION

• Ensure work scope is clear, authorized and communicated to affected
parties

• Raise permit or work Instruction that have been risk assessed and
authorized by PICs

• Ensure work is properly planned and regularly supervised / monitored /
audited from start to finish

• Ensure workers are knowledgeable, skillful, competent and medically fit to
perform the assigned task safely

• Look-out for simultaneous and peripheral activities and planned /
unplanned changes so they can be risk assessed

• Have an effective emergency response plan in place for critical, high risk
and non-routine activities

GOLDEN RULE #3

Energy Isolation

I WILL ensure Energy Isolation
before starting work

Stored energy sources shall be identified,
isolated, tested and communicated to
appropriate personnel before work
shall proceed.

MINIMUM EXPECTATION
• Step back to identify potential energy sources such as mechanical,

electrical, hydraulic, thermal, chemical, pneumatic, ionizing and radiation
prior to undertaking work

• Conduct a risk assessment before commencing any isolation work

• Ensure the Lock-out Tag-out (LOTO) or physical removal system is
adequate

• Periodically verify / test each energy source to ensure:
• Stored energy has been discharged
• Isolation remains effective

• Ensure that de-isolation is performed correctly and communicated to all
relevant stakeholders

• Ensure effective communication and handover between shifts, work
groups and at the end of work requiring isolation

GOLDEN RULE #4

Confined Space Entry

I WILL get approval
for Confined Space Entry

Confined spaces shall not be entered
unless authorized by a written permit.

MINIMUM EXPECTATION

• Have all other options been that do not require personnel entry into the
space been considered?

• Ensure the approved CSE permit, risk assessment and rescue plan are in
place and tested before commencing work

• Identify and isolate all energy sources that can affect space to be entered
including:

• Harmful fumes from equipment in or near the space

• Gases in hoses

• Have a standby person stationed outside the entrance with direct
communication with the work party and rescue team

• Conduct initial atmospheric testing in the confined before the work
commences and periodically during the work

GOLDEN RULE #5

Working At Height

I WILL protect against a
fall when Working at Heights

ground Personal fall protection equipment shall
be worn when working 2m or higher above
ground

MINIMUM EXPECTATION

• Identify, assess and control all working at height hazards

• Equip elevated platforms with guards, lower toe boards and handrails

• Use approved personal fall arrestor (PFA) when a fall of 2m or more is
possible

• Anchor all PFA to a point capable of withstanding a 2000kg load

• Perform visual check of PFA prior to use and dispose of any damaged parts

• Prevent drop objects - Secure all tools/equipment when working at height

• Put-up barriers and signs around drop zones (incl. due to deflection)

• Use ladders only for light duty and short duration work – select the right
type of ladder for the type of work being undertaken

• When there is potential for suspension trauma, ensure rescue provisions
are clearly identified in the work plan / PTW

GOLDEN RULE #6

Lifting Operations

I WILL ensure all Lifting
Operations are conducted safely

Lifting operations shall be correctly planned,
thoroughly risk assesses and carried out by
qualified personnel using certified equipment
which has been pre-use inspected and is
appropriate to the lifting operation it will be used on.

MINIMUM EXPECTATION

• All lifts require a plan, risk assessment, JSA and control measures based on
the complexity of the lifting operation

• All lifting operations must be supervised by a qualified person other than
the crane operator

• Ensure all laydown areas are clearly marked
• Test communication method between all personnel involved in the lifting

operation prior to commencement
• At no stage during a lift should personnel be positioned under load or in

the line of fire
• Check equipment certification and suitability, safe work load, weather

effect, center of gravity and rigging arrangement prior to lifting a load
• Ensure all personnel involved in lifting operations are trained and

competent
• Ensure lifting & rigging equipment and safety devices are operational

before and after use

GOLDEN RULE #7

Safety Device

I WILL not tamper with
Safety Devices

Never tamper with, remove or bypass any
safety devices, failure to comply may result in
death and serious injury.

MINIMUM EXPECTATION

• Obtain authorization to disable, override or bypass (i.e. bypass/override
certificate) a safety system prior to implementing

• Do not tamper with safety critical devices, including:
• Safety interlock protective system (SIPS)
• Pressure and vacuum relieve devices
• Detectors and alarms
• Emergency release systems, remote actuated emergency isolation
valves
• Uninterruptible power supply (UPS)
• Turbine over speed trips
• Fire protection and security devices
• Fire Fighting equipment, including bunker gear

• Personnel must have appropriate approval prior to putting any automatic
system in manual, and must return it to automatic on completion of the
task

Ignition Sources

I WILL ensure no unauthorized
potential Ignition Sources in Process Areas

A written work permit is required for all
hot work in designated areas

MINIMUM EXPECTATION

• Conduct risk assessment prior to commencing any hot work
• Obtain a work permit prior to commencing hot work
• Conduct flammable gas testing where known or suspected explosive

environments may be affected by any hot work. This testing shall be
conducted prior to and periodically during the work.
• Keep combustible materials a minimum of 10m from the work area and/or
protect materials from slag, sparks and other ignition sources
• Position fire protection equipment in the immediate vicinity of hot work
• Assign a fire watch to monitor hot work
• Check equipment used complies with the site Zoning principles

Transport / Mobile
Equipment

I WILL follow rules when operating
Transport/Mobile equipment

Identify all driving threats associated
with vehicle activity prior to putting vehicle
in motion

MINIMUM EXPECTATION

• Ensure transport/mobile equipment such as cars, buses, vans,
motorcycles, forklifts, mobile plant / cranes are:
• Designed for intended use and within specifications
• Inspected, certified and maintained in a safe working condition
before and after use

• Comply with any journey management plan requirements

• Ensure operators of vehicles are authorized, competent, medically fit, not
under the influence of drugs or alcohol

• All occupants must wear seatbelts whilst the vehicle is in motion

• Operators or drivers of all vehicles must:
• Be licensed
• Use seatbelts
• Not use cell phones whilst driving

GOLDEN RULE #10

Approved PPE
I WILL use required
and Approved PPE

PPE is equipment that will protect the user
against health or safety risks at work.
It can include items such as safety helmets,
gloves, eye protection, high-visibility clothing,
safety footwear and safety harnesses

MINIMUM EXPECTATION

• Use only approved PPE such as safety boots, hard hats, masks, goggles, life
jackets, safety harnesses, high visibility waistcoats and respirators

• Always inspect PPE prior to wearing and discard any damaged or expired
PPE immediately

• As per PPE User Guide, ensure workers are trained in:
• When they must wear PPE (hazards present)
• What type of PPE is necessary (selection)
• How to properly don, fit, adjust and wear the PPE
• Limitations of the PPE

• PPE is the ‘last line of defense’ to protect against harm. Refer to the
Hierarchy of Control below and always consider elimination, substitution,
engineering controls and safe work procedures and administrative controls
before PPE.
