Whether it’s a suggested friend, connection or follower, nearly all of us have experienced that moment
when social media recommends we make a connection with someone we know, or someone we know
of. Sometimes it’s a reach, and other times we immediately recognize the individual. Whichever the
case, it’s the personal connection that drives us to push the “add a friend” or “follow” button.
Similarly, when a child or a family comes to us, it’s most often because a single obstacle has been
identified to be hurdled. We work with these children and families and help them learn new ways to
become successful. However, it doesn’t end there.
Most often we connect or link them to other areas Four
Oaks can help be a support in their lives.
In this Annual Report you’ll see a theme of connections,
linkages, and ties coming through. It’s very symbolic of
the work we do each and every day. We have always
worked to connect a family to the supports they need.
Five years ago we formalized this this comprehensive
approach through TotalChild. Again and again, we see
this method and its link of supportive services and

service within our family of affiliate organizations, or tie a family to another avenue of support through
another community non-profit, it’s our capability to make these connections that makes us stronger
each and every day.
Just as the programs allow us to strengthen the children and families we serve and assure they
become successful, our donors and volunteers also play an important part in our past year’s success.
We wouldn’t be able to think creatively about and expand our work without the links and ties to our
supporters.
We encourage each and every one of you to imagine yourself as one of these connections
represented on the following pages. Whether you realize it or not, you pushed the “add a friend” or
“follow” button in a very creative way for our children and families this past year by supporting us. And

housing and family supports.
KATIE’S SUCCESS STORY
When Katie arrived in Cedar Rapids about year ago, she
felt like it was her against the world. She knew she and her
four children needed support as she was leaving a difficult
relationship with the kids’ father and everything they knew
was in a different state. All of this change was hard for the children as they struggled with their
behaviors and emotional outbursts and were often sad and fought with both Katie and each other.
Katie heard about Four Oaks’ TotalChild and reached out directly as being a newly single mom to
Jared, Jaiden, Jaylee and Jazzlyn was an overwhelming feeling.
“I wanted to give up. I felt like I had nobody behind me,” said Katie.
Not having their own place to lay their heads was difficult as the Katie and the children shared an
apartment with a family member. The close quarters began to wear on everyone, and it became
apparent the top two critical points of need were housing and therapy for the family.
“Then, I started talking with my Success Manager and I started to learn about a lot of the community
programs. I told them, ‘I want them all!’” said Katie.
With her children being her entire world, Katie spoke openly to her success manager and, in turn,
quickly learned about the available community programs. Thanks to TotalChild she worked with
nearly a dozen different opportunities that have helped her past the immediate crisis point of
needing a place to stay to taking on the next challenge of enrolling at the local Community College
this fall.
Additionally, these Four Oaks and community services have helped the children transition to their
new life. They even enjoyed their first experience at camp this past summer, which allowed them
the ability to “blow off steam,” run, and be kids again. Over the past year, a remarkable difference
can be seen in the children as they have stopped saying hurtful things to each other and now are
bubbling over with stories, sharing with each other and getting along well.
Katie believes TotalChild has been the single most important support to her and her family over
this past year. Some days she felt the difficulties outweighed the positives, but the consistent and
all-encompassing approach gave her the ability to move forward. “The support they gave me,
especially on those days when I couldn’t put on a smile, they helped me focus on what we wanted
and move forward for my kids,” said Katie.
Katie is proud with how hard she and her children have worked as a family this past year. They are
now focusing on the future with the siblings having dreams of being a candy inventor, a professional
athlete, a construction worker and a ballerina. Katie has even bigger dreams for her children though.
“I want to have our own home again. I want them to know that there are good people who are
willing to help you find yourself. And I want to see my kids grow up and be successful,” said Katie.
With TotalChild by her side, she and her children are ready to keep moving forward.
Four Oaks adopted its TotalChild approach over six years ago, and has seen remarkable results from
working with children and families in a comprehensive way. It’s this holistic method that serves as
the main connector for services in our communities. As you can see in Katie’s story above, Four Oaks
plays a strategic role in linking children and their families to supports. It’s having a direct tie to the
four major areas of basic needs, education, family and community, that children and families need to
hurdle the obstacles and become successful. 4

externally with community providers to
create the best outcomes for families and

young people which have allowed them
to become successful adults.
MARY’S STORY Affordable
Being a single mom, working two jobs and Housing
struggling to pay the bills, Mary was concerned MARY Jane Boyd’s Network, Inc
how much time she spent at work and hoped to PATHS
gain a better paying job. Mary got connected to
the Jane Boyd PATHS program which allowed
her to gain the skills and confidence to apply for
school to achieve her Bachelor’s degree. Through
conversations with her PATHS instructor, Mary
and her daughter, Trina, discovered they were also TRINA Jane Boyd
eligible to live in Affordable Housing Network Inc.
(AHNI) housing which provided affordable rent. With
many things lining up, Trina still needed a safe space to go Achievement
Academy
during the day. She was enrolled into Jane Boyd’s Achievement
Academy (AA), providing an enriching after school environment where
Trina can have support in her homework and have the opportunity to play
outside. After six months of enrollment in the after school program the staff
noticed that Trina’s behaviors had shifted. Mary explained Trina had been diagnosed
with diabetes and her blood sugars were impacting her emotions. The AA staff connected
Mary and Trina with Integrated Health Home services and TotalChild. The nurse worked with
Trina to understand and manage her diagnosis while TotalChild supported Mary in balancing the
day-to-day demands of parenthood to achieve their family goals. They are now successfully moving
toward their goals.
CARL’S CARL CARL’S STORY
FAMILY
Carl has struggled with acting out and
Trauma In-Home Residential had tried therapy at multiple treatment
Informed Services Treatment centers in Iowa. With none of them
Care (TIC) effectively working for him, Carl was

Out-Patient Treatment by the Department of

to stay in touch with Carl and staff

recommended the family receive
Integrated In-Home Services to help create a
Health Home safe space for Carl to come home to
as well as connecting Carl’s parents
(IHH) with outpatient services for addiction
with another area non-profit. As a
part of Carl’s Residential Treatment,
together, as a family, they also all
attended Trauma Informed Care
(TIC) training to help understand how
Carl’s past impacts how he reacts and
his brain development. With these
services dove-tailing together, Carl’s
behaviors improved and supervised
visits occurred with his parents. When
it came time for Carl to go home, the
family was enrolled into Out-Patient
Behavioral Therapy, to keep everyone
on the path to success. Finally, staff
saw another level of support for Carl
and he was enrolled in the PAL After
School Program which supports clients
with higher needs as they transition
out of care or are unable to manage in
a standard after school setting. Carl is
now successfully living in his home and
attending fifth grade.
