ELIGIBILITY
Teammates are eligible to begin receiving benefits on the first day of the month following or coinciding with two months of continuous
employment. (In some cases, where months have 31 days, you will be eligible on the 90th day.)

• Domestic Partner benefits are subject to taxation per the Internal Revenue Code. Affidavit of Domestic Partnership is required.

• D ependent children must be under the age of 26 or incapable of self-support due to physical or mental disability (proof of disability
status will be required).

CONSUMER DRIVEN ACCOUNTS
HSA (Health Savings Account) is funded by DaVita. Teammates may also choose to contribute pre-tax dollars. It is to pay for eligible
medical, dental and vision expenses and is only available to teammates enrolled in the High Deductible Health Plan.
HRA (Health Reimbursement Account) is funded by DaVita to pay for medical deductible and co-insurance. Available only to
teammates enrolled in the PPO w/HRA plan and earning less than $50,000 per year.
FSA (Flexible Spending Account) contributions allow you to pay for certain healthcare and dependent care expenses with tax-free
dollars. They are available regardless of medical plan participation; however, if you are eligible for an HSA, your Healthcare FSA would
be limited purpose for dental and vision expenses only.

LIFE/AD&D INSURANCE
Basic Life and Accidental Death & Dismemberment:
• Full-time: 1 times annual base salary up to $500,000.
• Part-time: 1/2 times annual base salary up to $250,000.
Voluntary Teammate Life Insurance is available in increments of $10,000; up to a maximum of $1,000,000 (evidence of insurability
may be required).
Voluntary Spouse Life Insurance is available in increments of $5,000, up to $500,000, or 50% of teammate’s total coverage amount
(evidence of insurability may be required).
Voluntary Child Life Insurance is available in the amount of $10,000 or $20,000 per child.
Voluntary AD&D is available in increments of $25,000 up to the lesser of 10 times your annual base salary or of $500,000. The
percentage of benefit payable depends on the type of loss and who is covered.
SHORT-TERM AND LONG-TERM DISABILITY (pre-existing conditions may apply)
Basic Short-Term Disability provides full-time teammates 60% of weekly earnings up to a maximum benefit of $1,000/wk, from day
8 through day 180 of your disability.
Supplemental STD allows teammates to purchase an additional 15% of coverage up to 75% of weekly earnings up to a maximum
benefit of $1,250/wk.
Basic Long-Term Disability provides full-time teammates 50% of monthly earnings up to a maximum benefit of $10,000/month.
Teammates are automatically enrolled in the LTD Basic benefit. Teammate will have to opt-out if they do not wish to participate,
75% paid by DaVita.
Supplemental LTD allows teammates to purchase an additional 10% of coverage up to 60% of monthly earnings up to a maximum
benefit of $12,000/month.
VOLUNTARY BENEFITS
Pet Insurance through payroll deduction. Discounted services and thousands of covered medical issues including accidents, illness and
poisonings.
MetLife Benefits:
• Auto & Home allows you to insure your home and/or vehicles through convenient payroll deductions.
• Critical Illness provides additional cash benefits of $10,000 or $20,000, to use for medical or other expenses if you or one of your
dependents is impacted by a serious illness. Pre-existing conditions may apply.
• Hyatt Legal provides unlimited telephone advice and free office consultations for most personal legal matters.
Village Market Place is an online site sponsored by DaVita that provides links to a wide variety of products, services and discounts.
WAYS DAVITA HELPS
401(k) Retirement Savings Plan: Teammates are eligible to participate upon date of hire (must be 18 years of age). Automatic
enrollment at a default 4% contribution rate will begin shortly following 45 days of service.
Adoption Assistance is available after an adoption is final. DaVita will reimburse adoption expenses up to $5,000.
DaVita Village Network provides teammates, or their immediate dependents, with financial assistance
for out-of-pocket expenses during a time of crisis such as a natural disaster, life-threatening emergency,
unexpected medical or funeral expenses, or financial hardships as a result of military deployment.
DaVita Village Trust aims to improve community health, wellness and vitality, including chronic kidney
disease education and prevention. It also supports strong communities, education, organizational culture
and diversity, the arts, and environmental causes in the United States and around the world.

Education Assistance is offered through DaVita and includes several programs to assist teammates with their educational goals.
Eligibility rules vary by program.
Employee Assistance Program is available to all teammate households. Confidential toll-free hotline for counseling, information and
referrals to a wide range of services.
Employee Stock Purchase Plan provides an opportunity to purchase DaVita stock at a discount. Eligible teammates must be regularly
scheduled to work 20 hours per week and employed by DaVita for three (3) continuous months prior to the start of each purchase
right period.
Little Star Gift: As a family friendly Village, we are excited to offer the Little Star Gift, providing
financial support to eligible DaVita parents after the birth or adoption of a child(ren).
Paid Time Off: Accrual for sick, vacation and holidays based on regular hours worked. Increases based on years of service with
DaVita. Accrual begins on day of hire and may be accessed for use after 90 days of continuous employment.
Triple Crown Profit Sharing Program is a three-part program designed to 1. VILLAGE – Everyone
recognize teammates and share profits when the company does well. Payouts 2. VICTORY – Team
vary in dollars and timing. Must meet eligibility requirements to participate. 3. VALUE – Individual
Village Care provides discounts on regular daycare at contracted providers and five days per year of back PEOP LE SERV

Vitality Points is DaVita’s wellness incentive program that encourages health awareness and
accountability, while rewarding you and your participating spouse or domestic partner, for taking action.
How it works: Each year DaVita sets base premiums for our medical plans. To qualify for the base
premium we ask teammates and their covered spouses/domestic partners to complete a few healthy
actions. When healthy actions are completed Vitality Points are automatically earned, which ensures
teammates receive the base medical premium the following year.
