reporting. These guidelines are to help create a
safer working environment for us all.

Hello, I am Ralf Faltermann, Managing Director
of the Electrical Carbon business. Today I would
like to talk about our quarterly e-brochure which
is about ‘Slips, Trips and Falls’.
40% of Morgan accidents last year were due
to slips, trips and falls. Most of our work places
have hazards from spills, dust and weather.
What can we do? We have to report the
hazards; we have to clean up the spills, and we
have to take care when we work from outside
to the inside and we have to practice very good housekeeping. Our Shanghai facility sets
a very good example.

Near miss What is a ‘Near Miss’?
Near Miss:
A ‘Near Miss’ is an unplanned event or situation (e.g. a loose floor tile, a spilled
liquid, an incorrect setting, etc.) that did not result in harm to people, the
environment or property e.g. injury, pollution, process failure, damage to
equipment, etc. on this occasion but had the potential to do so and where
harm could result in future if the situation hazard is not fixed/the behaviour is
not changed.
Hazard:
A ‘Hazard’ is a source or situation that has the potential to harm a person, or
the environment or cause damage to property hazards also include all unsafe
conditions, unsafe acts, find and fixes, etc.
Significant Near Miss:
A ‘Significant Near Miss’ is a near miss event or situation which had the
potential to cause one or more fatalities or life altering/ threatening injuries or
illnesses.

How do Near Miss reporting systems prevent future incidents?
Morgan believes in:
• Creating a proactive &
transparent safety culture
with its thinkSAFE initiatives
• Encouraging a “NO BLAME”
culture
• Not waiting for losses to occur but taking steps to prevent a recurrence
• Investigating the faulty process or management system which is invariably the root cause for
the increased risk that leads to the near miss and should be the focus of improvement
• Recognizing & empowering us all to report the near miss incidents to significantly
improve our safety and enhance Morgan’s safety culture
You can approach your supervisors or the person responsible for EHS to report the near misses on
your behalf by using the near miss app or thinkSAFE portal from the safety corner.
Supervisors and managers must ensure they give appropriate feedback about the near misses in daily
meetings. When closing the near misses / hazards the focus shall be to prevent the reoccurrence of
such near misses; we believe in the quality near miss reporting rather than quantity of near misses.

What is Morgan’s practice in establishing a Near Miss Reporting System?
Morgan believes in:
• Morgan has established a transparent reporting culture
reinforcing that every opportunity to identify and
control hazards, reduce risk and prevent harmful
incidents by our Near Miss App and thinkSAFE portal
• The reporting system is based on the Global Positioning
System (GPS) of our sites
• You can log in with your Morgan email id and
Morgan email password
• You can check the name of site that you are reporting for & select the category of the
near misses
• You can take a photo and write a small description (here you can also add the name of the
worker who has reported it) of the near miss and save it
• We must form the investigation team to identify the root cause and the weaknesses in the
system that resulted in the circumstances that led to the near miss
• Use investigation results to improve safety systems, hazard control, risk reduction, and lessons
learned. All of these represent opportunity for training, feedback on performance and a
commitment to continuous improvement

What is our role in Near Miss Reporting Systems?
Near Miss Reporting Systems:
• We need to empower our employees on the reason why near miss reporting is a necessity,
and the important role that they play in sustaining the ZERO HARM Culture
• We need to provide a convenient opportunity for “employee participation,” a basic
component of a successful safety management system with the support and help of all
managers and supervisors
• We need to create an open culture whereby everyone shares and contributes in a responsible
manner to their own safety and that of their fellow workers
• You need to consider this to be a leading indicator of performance used in balance with
other leading and lagging measures. Include the e-brochure training for new employees as
a part of their orientation

What are Best Practices within Morgan?
Sites who excel at safety and have close to zero incidents, utilise near-miss
reporting in various ways:
• You might need to consider having leading indicators as one of the elements in your
performance management system
• You might need to refresh hazard identification or risk assessments as a mechanism for
engaging and empowering employees at different levels
• Many sites in Morgan have sharing of lessons throughout the organisation as a part of
“ONE POINT LESSON” or “SAFETY KAIZENS”
• Sites do this in periodic management reviews or safety committees
So don’t walk by, practice TAKE 5 and do encourage the reporting of near misses the
next time you find or observe it, with an aim to sustain our Zero Harm culture.
