Students, Teachers and non-faculty staff have to submit the online COVID-1 9 Daily
Screening before coming to school in order to screen the risk of infection and contacting to
the infected person.

1.2 การตรวจวัดอณุ หภมู ิร่างกาย

การตรวจวัดอุณหภูมิร่างกาย นักเรียน ผู้บริหาร ครู บุคลากร และผู้ปกครอง ก่อนเข้าสู่บริเวณ
โรงเรียนและกำหนดให้ทุกคนสวมใส่หน้ากาก การล้างมือด้วยแอลกอฮอล์เจลหรือสบู่ การเว้นระยะห่าง และ
การทำความสะอาดพืน้ ผิวสัมผัสรว่ ม การลดความแออดั รวมทั้งการทำความสะอาดแบบ Deep Clean ทกุ วัน
ศุกร์ กำหนดเวลาในการพักรับประทานอาหารเหลื่อมเวลา จัดอาหารปรุงสุกใหม่ ตามหลักโภชนาการ

Students, Teachers and non-faculty staff test the body temperature before entering
school area, wear a mask and clean hands frequently with alcohol-based hand sanitizer or
soap and water. Keep physical distance at least 1 metre from others, even if they don’t
appear to be sick. Avoid crowds and close contact. Clean the frequently touch surface and
deep clean materials every Friday. Manage the overlapping time for lunch and zone for
playground for each class that students will not be cross. School will provide nutritious and
hygienic food in each individual container.

Coordinate and encourage the teachers, staff, students and parents to get the COVID-
19 Vaccination.
1.5 กำหนดแนวปฏิบัติสำหรบั ผูบ้ รหิ าร ครู และบคุ ลากรในสถานศึกษา
กำหนดแนวปฏิบัติสำหรับผู้บริหาร ครู และบุคลากรในสถานศึกษาในการดำเนินการเกี่ยวกับโรคติด

Issue a COVID-19 protection and prevention policy and mention in teachers and school

1.6 การปิดสถานศึกษาทเี่ กิดการระบาดและควบคุมการระบาดในสถานศกึ ษา
การปิดสถานศึกษาที่เกิดการระบาดและควบคุมการระบาดในสถานศึกษา ตามแนวทางการบริหาร

Follow the guideline for managing and operating school during the COVID-19 pandemic
situation.

รายงานการประเมินสถานการณ์ผลการดำเนินงาน ปัญหาอุปสรรค และให้ข้อเสนอแนะแก่หน่วยงาน

Report the assessment and the situation to the School Board, Education Office and
the Chiang Rai Communicable Disease Control.

การคัดกรองผ่าน COVID-19 Daily Screening โดยกำหนดให้ครู บุคลากรและนักเรียนตอบแบบคัด
กรองเพื่อประเมินความเสยี งในการติดเชือ้ และสมั ผัสผตู้ ิดเชอ้ื ทกุ วนั ก่อนมาโรงเรียน

Students, Teachers and non-faculty staff have to submit the online COVID-19 Daily
Screening before coming to school in order to screen the risk of infection and contacting to
the infected person.

ในกรณีที่ มีความเสี่ยงจากการสัมผัสผู้ติดเชื้อ หรืออาศัยร่วมบ้านกับผู้ติดเชื้อผู้ป่วยยืนยัน
โรคติดเช้ือไวรัสโคโรนา 2019 (COVID-19) นักเรียนจะไม่ได้รับอนุญาตใหม้ าโรงเรียน และจะต้องปฏิบัตติ น
ดงั ต่อไปน้ี

In case of the family member or students has a risk to contact the infected cases,
student will not be allowed to come to school and follow below protocol.

2.1.1 แจง้ โรงเรยี นทนั ที โทร. 088-2591280

Please immediately notify school at phone no. 088-2591280.

2.1.2 แยกตวั จากผอู้ นื่ ทันที และสงั เกตอาการอยู่ท่ีบ้าน

Home Isolate and observe the symptoms.

2.1.3 ติดต่อสำนักงานสาธารณสุขหรือศูนย์ปฏิบัติการฉุกเฉินโควิด -19 จังหวัดเชียงราย
โทร. 053-910384-5 เพื่อตรวจคัดกรองโควิด-19 หากพบว่าเด็กมีประวัติเสี่ยงต่อการสัมผัสผู้ป่วยยืนยัน
โรคติดเชื้อไวรัสโคโรนา 2019 (COVID-19) ให้แจง้ เจา้ หน้าท่สี าธารณสขุ ภายใน 3 ชัว่ โมง

Contact the Local Health Organization or CR Hospital at the phone no.
053-910384-5 for further investigation within 3 hours.

2.1.4 ทำหนังสือแจ้งผู้ปกครอง ปิดห้องเรียนอย่างน้อย 3 วัน เพื่อทำความสะอาด และบริเวณ
สมั ผัสร่วม

Close the suspected class for 3 days and inform the parents.
2.1.5 หากพบนักเรียน ครู หรือบุคลากร ที่ติดเชื้อ ประสานความร่วมมือกับทางโรงพยาบาลเชียงราย
ประชานุเคราะห์ เพื่อเข้าสู่กระบวนการรกั ษา และพักรักษาตัวอยา่ งนอ้ ย 10 วัน และส่งผล ATK ก่อนกลับมา
เรยี นตามปกติ

If the student. Teacher or staff has COVID-19 positive tested, coordinate with
CR Hospital for Medical Treatment and quarantine for 10 days. Submit the ATK result
again before returning to school.
2.1.6 นกั เรยี นร่วมหอ้ งทเ่ี ปน็ ผูส้ ัมผสั ผตู้ ิดเช้ือใกลช้ ิด ส่งประสานโรงพยาบาลศูนย์ตรวจหาเช้ือ วันที่ 5
และวันที่ 10 หลังพบนักเรียนติดเชื้อและกักตัวเองสังเกตอาการ อย่างน้อย 10 วัน และส่งผล ATK ก่อน
กลับมาเรียนตามปกติ

The close contacted students will be sent to CR Hospital for further
investigation on day 5 and day 10 after the day that an index student attended school.
The close contacted student quarantine at home for 10 days. Submit the ATK result
again before returning to school.
2.1.7 นักเรียนร่วมห้องแต่ไม่ได้ที่เป็นผู้สัมผัสผู้ติดเชื้อใกล้ชิด หยุดเรียนและกักตัวเองสังเกตุอาการ
ตัวอย่างนอ้ ย 3 วนั และสง่ ผล ATK ก่อนกลับมาเรียนตามปกติ

The classmate students who have low risk (not a close contacted students
home isolate and observation for 3 days. Submit the ATK result again before returning
to school.
2.1.8 ปิดห้องเรียนตามคำสั่งของศูนย์บริหารสถานการณ์แพร่ระบาดของโรคติดต่อเชื้อไวรัส
โคโรนา 2019 เชียงราย

Close the index class as recommended by the government.

2.2 การปฏบิ ตั หิ น้าท่ีของครเู วรรบั นกั เรยี น

การปฏิบัติหน้าที่ของครูเวรรับนักเรียน เวลา 07.30 – 08.30 น. โดยปฏิบัติหน้าที่คัดกรองเบื้องต้น
ดว้ ยการตรวจวัดอณุ หภมู ิรา่ งกายโดยใช้เคร่ืองวัดอุณหภมู ิ จดั เว้นระยะระหวา่ งคน 1 เมตร - 1.5 เมตร และกด
แอลกอฮอล์เจลลา้ งมือก่อนเข้าบริเวณโรงเรยี น

Every morning between 07.30-08.30 at the wooden gate, there will be a morning duty
teacher test student’s temperature at 07.30 - 08.30, apply the alcohol-based hand rub and
organize the physical distance of at least 1 metre from others.

2.3 การจัดเตรียมอุปกรณก์ ารป้องกนั ตนเองและและอปุ กรณ์ล้างมือ

จัดเตรียม หน้ากากอนามัย ชุดตรวจ Antigen Rapid Test น้ำยาทำความสะอาดฆ่าเชื้อ และ
แอลกอฮอลล์ า้ งมอื ให้เพยี งพอสาหรบั นักเรยี น ครแู ละบคุ ลากร

Provide the masks, Antigen Rapid Test Kit, alcohol hand sanitizer and other dis-infection
supplies for students, teachers and staff.

2.4 การตรวจวัดอุณหภมู ิ

นักเรียน ครูและบุคลกร ที่มีอุณหภูมิร่างกายปกติอยู่ระหว่าง 36.1 - 37.2 องศาเซลเซียส
หากตรวจวัดอุณหภูมิร่างกายครั้งแรก มีอุณหภูมิร่างกายมากกว่า 37.5 องศาเซลเซียส ให้พักคอยหรืออยู่ใน
บรเิ วณจุดตรวจวัดอย่างนอ้ ย 5 – 10 นาที กอ่ นการวัดอกี ครัง้ และวดั ซ้ำด้วยปรอทวดั ไข้

The student teacher and staff who has body temperature over than 37.5 Celsius, wait
for 10 minutes and test again.

กรณีอุณหภูมิร่างกาย ตั้งแต่ 37.5 องศาเซลเซียสขึ้นไป หรือมีไข้ ร่วมกับอาการทางเดินหายใจ ให้
แยกนกั เรียนในห้องทีจ่ ัดเตรียมไว้ บันทึกอาการป่วยและให้การดูแลเบื้องต้น ซักประวตั แิ ละสังเกตอาการเส่ียง
แจ้งผู้ปกครองรับเด็กกลบั บ้านหากมอี าการไมด่ ีขน้ึ ให้ผูป้ กครองพาเดก็ ไปพบแพทย์

In case of the temperature is over than 37.5 Celsius after the second test, separate
student, contact the parent, check the history if there is a risk to contact the COVID-19 infected
cases.

❖ ในกรณีไม่มีประวัติสัมผัสผู้ติดเชื้อ ผู้ปกครองรับรักเรียนกลับบ้าน สังเกตอาการและพบ
แพทย์เพ่อื ทำรักษา

If the is no history of contact to the COVID-19 infected cases. parent take
student home to observe the symptoms and meet the specialist for further
investigation.

❖ ในกรณีทสี่ มั ผัสผูต้ ดิ เชือ้
1. แยกนกั เรยี น
2. แจง้ ผูป้ กครอง เพอื่ เขา้ ส่กู ระบวนการการสอบสวนโรค
3. ตรวจ ATK นักเรยี นและครูประจำช้ัน
4. ทำหนังสือแจ้งผู้ปกครอง ปิดห้องเรียนอย่างน้อย 3 วันเพื่อทำความสะอาด และบริเวณ

สมั ผัสรว่ ม
5. หากพบนักเรียนที่ติดเชื้อ ประสานความร่วมมือ รพ.เชียงรายประชานุเคราะห์ เข้าสู่

กระบวนการรกั ษา และพักรกั ษาตัวอยา่ งน้อย 10 วนั และส่งผล ATK ก่อนกลบั มาเรียนตามปกติ
6. นักเรียนที่เปน็ ผูส้ ัมผัสผู้ติดเชื้อใกล้ชดิ ส่งประสานโรงพยาบาลศูนย์ ตรวจหาเชื้อ วันที่ 5

และวันที่ 10 หลังพบนักเรียนติดเชือ้ และกักตัวเองสงั เกตุอาการตัวอย่างน้อย 10 วัน และส่งผล ATK
ก่อนกลับมาเรียนตามปกติ

7. นักเรียนร่วมห้องแต่ไม่ได้ที่เป็นผู้สัมผัสผู้ติดเชื้อใกล้ชิด หยุดเรียนและกักตัวเองสังเกตุ
อาการตัวอย่างน้อย 3 วนั และสง่ ผล ATK ก่อนกลบั มาเรียนตามปกติ

8. ปิดห้องเรียนตามคำสั่งของศูนย์บริหารสถานการณ์แพร่ระบาดของโรคติดต่อเชื้อไวรัส
โคโรนา 2019 เชียงราย

If there is any exposure to the COVID-19 infected cases.
1. Isolate the suspected student.
2 . Inform the parent and coordinate with the CR Hospital for further

investigation.
3. Test students and teacher of the class that the suspected student

belongs for COVID-19 with the Antigen Rapid Test.
4. Close the suspected class for 3 days and inform the parents.
5. If the suspected case has COVID-19 positive tested, coordinate with CR

Hospital for Medical Treatment and quarantine for 10 days. Submit the ATK result
again before returning to school.

6 . The close contacted students will be sent to CR Hospital for further
investigation on day 5 and day 10 after the day that an index student attended
school. The close contacted student quarantine at home for 10 days. Submit the
ATK result again before returning to school.

7 . The classmate students who have low risk (not a close contacted
students) home isolate and observation for 3 days. Submit the ATK result again
before returning to school.

8. Close the index class as recommended by the government.

3. กรณีเดินทางต่างจงั หวัด For students who travelling across the province.
3.1 ตรวจหาเชือ้ ไวรสั โคโรนา 2019 (COVID 19) ด้วยชุดตรวจ Rapid Antigen Test และส่งผลตรวจ

ให้ครูประจำชั้นกอ่ นมาโรงเรยี น
กรณีพบการติดเชื้อไวรัสโคโรนา 2019 หากพบนักเรียนที่ติดเชื้อ เข้าสู่กระบวนการรักษา และ

พักรักษาตัวอย่างนอ้ ย 10 วัน และส่งผล ATK ก่อนกลบั มาเรียนตามปกติ
For students who travelling across the province.

3.1 Test for coronavirus 2019 (COVID-19) with Antigen Rapid Test and submit the result
to teachers before came to school.

If the student has COVID-1 9 positive tested, student follow MOPH Medical
Treatment. Submit the ATK result again before returning to school.
4. กรณีเดนิ ทางต่างประเทศ

4.1 ปฏิบัติตามมาตรการเดินทางเข้าและออกประเทศไทยตามประกาศของกรมการกงสุล กระทรวง
การตา่ งประเทศ
For teacher and student who travelling aboard.

4.1 Teacher and student who travelling aboard has to follow the national guideline for
person who travelling aboard.
5. การดำเนินการเมอื่ พบการติดเช้อื ในโรงเรยี น

กรณพี บนกั เรยี นติดเชื้อ 1 คนขนึ้ ไปในห้องเรียนเดยี วกัน
5.1 แยกนกั เรียนท่ีติดเช้อื และผสู้ มั ผัสผตู้ ิดเชือ้ ใกลช้ ิด
5.2 ประสานความร่วมมือ โรงพยาบาลเชียงรายประชานุเคราะห์
5.3 ทำหนังสือแจ้งผู้ปกครอง ปิดห้องเรียนอย่างน้อย 3 วันเพ่ือทำความสะอาด และบรเิ วณสมั ผัสร่วม
5.4 ตรวจ ATK นักเรยี นและครผู ู้สมั ผัสเชื้อ
5.5 นกั เรยี นทต่ี ดิ เช้อื เขา้ ส่กู ระบวนการรกั ษา และพกั รกั ษาตวั อยา่ งนอ้ ย 10 วนั และสง่ ผล ATK ก่อน
กลบั มาเรยี นตามปกติ

5.6 นักเรียนที่เป็นผู้สัมผัสผู้ติดเชื้อใกล้ชิด ส่งประสานโรงพยาบาลศูนย์ ตรวจหาเชื้อ วันที่ 5 และ
วนั ที่ 10 หลังพบนักเรยี นตดิ เชื้อ

กรณีผลการตรวจเป็นลบ นักเรียนท่ีเปน็ ผู้สัมผัสผู้ติดเชื้อใกล้ชิด หยุดเรยี น 10 วัน และส่งผล
ATK กอ่ นกลบั มาเรยี นตามปกติ

กรณีผลเป็นบวก ปฏิบัติตามมาตรการดำเนินการเมอ่ื พบการตดิ เชอื้ ในโรงเรียน
5.7 นักเรียนร่วมห้องแต่ไม่ได้ที่เป็นผู้สัมผัสผู้ติดเชื้อใกล้ชิด หยุดเรียนและกักตัวเองสังเกตุอาการ
ตวั อยา่ งน้อย 3 วนั และส่งผล ATK ก่อนกลับมาเรยี นตามปกติ
5.8 ปิดห้องเรียนตามคำสั่งของศูนย์บริหารสถานการณ์แพร่ระบาดของโรคติดต่อเชื้อไวรัสโคโรนา
2019 เชียงราย
กรณีพบนกั เรยี นตดิ เชื้อมากกวา่ 1 หอ้ งเรยี น
1) ปฏบิ ตั ติ ามมาตรการดำเนินการเมอื่ พบการตดิ เชื้อในโรงเรียน
2) ปิดหอ้ งเรยี นระดับเดียวกนั 3 วนั
กรณพี บนักเรียนติดเชอื้ มากกว่า 1 ระดบั
1) ปฏิบตั ติ ามมาตรการดำเนินการเม่อื พบการติดเชื้อในโรงเรียน
2) ปิดเรียนทงั้ โรงเรยี น 3 วัน
The Response Action to the scenario of there is an infected case in School
The scenario of one or more detected case in the class
5.1 Isolate the case and screen the close contacted case.
5.2 Coordinate with CR Hospital.
5.3 School inform the parents for the closure of the classroom and disinfected the
materials for at least 3 days.
5.4 Students and teachers of the index class test for COVID-19 with the antigen rapid
test kit.
5 .5 The student who has COVID-1 9 positive tested, student follow MOPH Medical
Treatment at least 10 days. Submit the ATK result again before returning to school.

5.6 School coordinate with CR Hospital for further investigation for the close contacts
students on day 5 and day 10 after the day that an index attended school.

In case of Negative result The contacted student isolated at home for 10 days
and submit the ATK result before returning to school.

In case of positive result Follow the Response Action to the scenario of there
is an infected case.
5.7 The classmate students who have low risk (not a close contacted students ) home
isolate and observation for 3 days. Submit the ATK result again before returning to school.
5.8 Close the index class as recommended by the government.
The scenario of more than one class in the same level has detected case.
1) Follow the Response Action to the scenario of there is an infected case in School.
2) School close the contacted level for 3 days.
The scenario of more than one level has detected case.
1) Follow the Response Action to the scenario of there is an infected case in School.
2) A whole school close for 3 days.
