SUBJECT: CADET MEMBERSHIP CONTRACT

1. I WANT TO BE A CADET!! I understand that the AFJROTC Program is an elective subject and that
my enrollment is entirely voluntary and that regular class attendance and attention to academics are
essential to successful completion of the AFJROTC Program. AFJROTC is a one (1) credit course and
that I must pass the full year to receive credit for the course. Corps activities are voluntary and my
participation in them will enhance my enjoyment of the program. However, violations of any AFJROTC
rules and regulations may result in reprimand, reduction in rank or ELIMINATION from AFJROTC.

2. I understand that my Activity Fees ($40) must be paid and that my hair must conform to USAF
appearance standards within THE FIRST 10 DAYS of school or they will be grounds for my removal from
AFJROTC. The Activity Fee includes end of year uniform cleaning. This money will not be refunded.

3. I do understand that SPECIAL emphasis is placed on the following:

a. The Uniform items are loaned to me and they are U.S. Government property. I am TOTALLY
RESPONSIBLE FOR THEM. If I lose Uniform items, allow them to be stolen or destroyed, I MUST PAY
FOR THEM. I must keep them clean, wear them properly and turn them in promptly when instructed to do
so. I fully understand that my Uniform must be kept clean at all times. The pants and coats must be dry
cleaned. Failure to do so will require return of the Uniform and course failure.

b. I must wear the Uniform on Military Training Days and other special occasions, as directed, and I will
present the best personal appearance that I possibly can. When assigned to Flag Detail, Flag
Presentations, Officer of the Day, Parades or any other event, I will perform in the best possible manner
so as to reflect honor on myself, PHS and the United States Air Force. Failure to do so could result in
disciplinary action.

c. I agree to abide by the Dress Code at ALL times, which includes hair, sideburns, mustaches, jewelry
and other cosmetics. Male cadets may not have beards or goatees and will not wear earrings at any time
in uniform. Female cadets may wear one gold, silver, or pearl stud earring on each ear when in uniform.
Pants/slacks and skirts will be worn at the length directed by Air Force regulations and will not be pegged
or flared.

d. Failure to wear the uniform on Inspection Day will result in an "F" for that week's Uniform grade.
Failure to wear the uniform four (4) times during a grading period automatically FAILS ME for the entire
grading period.

e. I understand that my conduct must reflect HONOR and CREDIT to myself, the US Air Force, and
Pensacola High School at all times. This means at school and off campus. I will always respect the flag.
When giving the PLEDGE OF ALLEGIANCE, I will do it properly. (Other teachers notice and report
AFJROTC Cadets who fail to show proper respect to the flag or showing other inappropriate conduct.)

4. My signature below indicates that I have read and understand this contract. I agree to do MY BEST to
follow the rules and regulations of AFJROTC and the US Air Force. The signature of my Parent or
Guardian indicates that they have read and understand this contract, and are aware of my responsibilities
while I am an AFJROTC Cadet.

1. A military style letter is a “memorandum” or often just a “memo.” The headings for,
from, subject and date are included in each letter. The heading AFJROTC FL-12 and
PHS are optional unless the memo is leaving FL-12.

2. As in any formal writing, the author is expected to clearly state the purpose in the
first paragraph. If additional paragraphs are required to clarify or discuss the purpose,
they are to be numbered and have a space between them.

3. The closing or salutation is as indicated. The signature block is right of center, four
lines down from the last paragraph. The author should sign above the signature block.
The name in the signature block is to be typed/written in all capital letters.
