About the Peter Munk Cardiac Centre

Thank you for your interest in supporting the Peter Munk Cardiac Centre, a global leader in
cardiovascular care, research and education.

The Peter Munk Cardiac Centre (PMCC) was established in1993 through the generous support
of Peter and Melanie Munk. Today, the PMCC has grown to over 162,000 out-patient visits a
year. Multidisciplinary surgical teams perform more than 2,000 cardiac and vascular surgeries,
over 7,800 interventional cardiology procedures, 35 heart transplants and 34 left ventricular
assist device or mechanical heart support implants a year, making the PMCC a high-volume
centre that cares for patients with the most complex cardiovascular disease in North America.

The PMCC is a technological state-of-the-art facility, deploying the latest diagnostic and
interventional techniques, bringing medical imaging, cardiology, cardiac surgery, and vascular
surgery together to lead in innovative multidisciplinary practice. Multi-slice CT scanners,
the latest MRI technology, and other imaging tools now produce sharper and more precise
images, diagnosing cardiac problems and supporting the development of less intrusive
treatments for our patients.

On the Canadian and international stage, the PMCC’s impact has been transformative —
much of it fueled by the generous contributions of grateful donors. These investments allow
the PMCC to continue the tradition of cardiovascular innovation that includes discoveries
that led to the development of the world’s first pacemaker, and the development of the
first coronary intensive care unit. The talent pool at the PMCC is enviable by any and all
international standards; many of the clinicians and scientists in the PMCC are counted among
the best in their field in the world.

Everything we do at the Peter Munk Cardiac Centre is based around how to make things
better for patients and for the healthcare system. Following are opportunities for support that
will ensure the Peter Munk Cardiac Centre remains at the forefront of revolutionizing heart and
vascular care for all of our patients and their families – here, and around the world.

I look forward to having the opportunity to further discuss your participation.

Yours truly,

Dr. Barry Rubin
Chair and Program Medical Director
Peter Munk Cardiac Centre

Areas of Care at the Peter Munk Cardiac Centre

The Peter Munk Cardiac Centre has six departments, each focusing on a distinct element of
cardiovascular care.

1. Adult Congenital Heart Disease

The PMCC’s Adult Congenital Heart Disease Program (ACHD) was the first program of its kind
to offer comprehensive care for adult survivors of congenital heart diseases and, after more
than 50 years, it is the largest program of its kind in the world, with more than 8,500 active
patients, more than 17,000 in its registry and about 750 referrals each year from across the
country. The ACHD outpatient clinic alone assessed 3,575 patients in 2016 and had more
than 4,600 outpatient visits. The number of outpatient visits is projected to approach 5,000 in
2020. Educational excellence is a hallmark of the PMCC, and the ACHD Fellowship Program
plays a critical role in the provision of ACHD education locally, nationally and internationally.
The Fellowship Program is one of the first of its kind and is the largest in the world. No other
program has trained more ACHD physicians over the last 20 years.

2. Coronary Artery Disease

Coronary artery disease is the most common form of heart disease and occurs when the
coronary arteries become narrowed by a build-up of fatty layers (also known as plaque) in
the artery walls. This process is called atherosclerosis, resulting in less blood flow reaching
the heart muscle, which causes pain and can lead to heart damage. The PMCC takes a multi-
faceted approach to address this global epidemic. Clinical specialists are performing and
developing new heart therapies, researchers are finding new ways to diagnosis heart disease,
and medical students and other healthcare professionals receive the latest education. With 17
different labs and clinics that touch directly or indirectly on coronary artery disease, the PMCC
looks at the condition from all angles.

3. Vascular Disease

The PMCC’s Division of Vascular Surgery is a world-class clinical care, research and academic
unit renowned for delivering care to patients with highly complex conditions through
advanced diagnostic and surgical interventions, including the use of minimally invasive,
percutaneous (through the skin) methods. One of the most significant developments in
recent years in the treatment of vascular diseases — conditions affecting the veins, arteries
and capillaries that carry oxygen-rich blood throughout the body such as aneurysms, blood
clots and ulcers — is the reduction in open surgery whereby large incisions are made in
the body. More than 90 per cent of the vascular-related procedures at the PMCC involving
balloons, stents and wires are now done with no incision at all.

6

4. Valvular Disease
The PMCC has received international recognition for its scientific contributions on the
diagnosis and treatment of patients with aortic and mitral valve diseases. Diseases of the
aortic and mitral valves are a leading cause of heart failure, yet the underlying causes and
molecular mechanisms of these diseases are not well understood. Known worldwide for its
pioneering valve-sparing root replacement procedure named for Dr. Tirone David, the valvular
disease team at PMCC continues to innovate. In August 2016, a multidisciplinary team at
the PMCC performed two successful tricuspid valve replacement procedures. The tricuspid
valve replacement procedure is very rare, with only about a dozen having been performed
worldwide since the method was established in early 2015.
5. Heart Failure/Heart Function
The Ted Rogers Centre of Excellence in Heart Function at the PMCC is the largest advanced
heart failure and heart transplant program in Canada. The Centre receives about 400 new
patient referrals and performs 2,600 follow-up visits each year for advanced therapies for
heart failure, including: transplantation, mechanical circulatory support, high-risk surgical
interventions, cardiac resynchronization therapy and optimization of medical therapy. The
team actively manages more than 2,000 patients with advanced heart failure, 320 heart
transplant recipients and 25 outpatient long-term mechanical circulatory supported patients.
The Centre has assessed about 4,000 new patients since 2000.
6. Heart Rhythm
The PMCC is an international leader in the investigation and treatment of heart rhythm
disorders. Electrical disorders of the heart or heart rhythm disorders are a major and
increasing cause of sudden cardiac death and disability in both young and aging populations.
Electrophysiology research staff is involved in ground breaking research aimed at improving
our understanding of and treatment of heart arrhythmias. Its specialists have already been
able to contribute to breakthroughs that have impacted treatments, diagnosis and care of
patients with electric disorders of the heart across Canada and around the world.

Opportunity for Impact:
In memory and honour of Sheldon Friedland

1. Peter Munk Cardiac Centre Fellowship Program
The Fellowship Program at the Peter Munk Cardiac Centre provides trainees from around the
world with the best clinical and research training in cardiology. This year alone the PMCC has
Fellows from Israel, Australia, Brazil, Argentina, England, the United States and Canada. Fellows
train as physician specialists through merging core competencies with specialized treatments,
including: cardiac resynchronization, mechanical circulatory support, electrophysiology,
transplantation, imaging and human physiology. Internationally recognized for its outstanding
physicians and its excellence in training, our goal is to develop Fellows into competent
practitioners.

Doctors in the Peter Munk Cardiac Centre share their insights with visiting Fellows from around
the world.
Opportunity for support
Our goal is to recruit the top talent in the world, which is why securing funding for Fellowships
is so crucial. There is no government funding to support these needs, and Fellowship
candidates are not eligible for student support. Generous donations from our patients and
their families have helped to fund these Fellowships.
There is an opportunity for the Friedland Family to honour Sheldon Friedland through the
naming of a Clinical Fellow MD for a period of two years at $85,000/yr. or a PhD Fellow in the
amount of $65,000/yr. for three years. Your support will help in training and establishing future
generations of specialists who will work tirelessly to fight heart disease.

The Upper Canada Medal is presented to our donors at each of the levels of giving ($25,000+)
in gratitude for their generous support.

The Upper Canada Medal was originally created by the Loyal and Patriotic Society of Upper
Canada to provide recognition of distinguished service and bravery to volunteer soldiers in
the War of 1812.

Recognizing an even greater need, the Loyal and Patriotic Society decided to reduce the med-
als to gold bullion for the establishment in 1829, of the original Toronto General Hospital.

Toronto General & Western Hospital Foundation continues to honour both the original intent
of the medals, to recognize distinguished service and theFirrolantter role to support the greater
good.
