Business Policy

DEPOSITS

A credit card is required to reserve your appointment. A $25 non-refundable fee is required upon
booking. Deposits will go towards service. A processing fee may apply if you chose to pay remaining
balance with card. Must be 18yrs or older to book.

APPOINTMENTS

Please arrive early: If it is your first visit, please arrive at least 10 to 15 minutes before your appointment
start time in order to complete all necessary paperwork without cutting into your session time. For all
returning guests, please arrive at least 5 to 10 minutes before your appointment time to check in, get
situated, and be ready to go for your full session time. All clients are to arrive without make up.

Late arrivals: Arriving late will cut into your session time and may result in additional charges if your
appointment must be cancelled. If you arrive more than 15 minutes after your appointment time, your
appointment will be canceled, and you will be charged 50% of the service fee, per our late cancellation
policy. Out of respect for other guests, we cannot cut into another guest’s appointment time to make up
for your lost appointment minutes if you are late.

CANCELLATIONS

Your appointment is important, it is time reserved especially for you. If any changes need to be made to
your appointment, please notify at least 24 hours in advance in order to avoid additional charges. Same
day cancellations will forfeit your deposit and be charged half of the service total. No call/No shows will
be charged FULL AMOUNT of service. No exceptions.

GUESTS:

Unfortunately, due to lack of space, please, NO additional guest at the time of your appointment.

REFLLS

40% of lashes must be remaining to be considered a refill. Anything less, will be considered a full set and
charged accordingly. Unfortunately, we do not work over any other artist’s work. With that in mind, we
do offer removals that are packaged with a full set.

eyelash extensions to my natural eyelashes and/or remove and retouch. By signing this agreement, I
consent to the placement and/or removal of the eyelash extensions by the certified eyelash extension
professional.

_______ I understand that in rare occasions, there are risks associated with having artificial eyelashes
and eyelash extensions applied to or removed from my natural eyelashes. I further understand that in
rare cases as part of the procedure, eye irritation and discomfort could occur. I agree that if I experience
any of these conditions with my lashes, that I will contact the lash extensionist who performed this
procedure and it may be beneficial to have the eyelashes removed.

_______ I understand and consent to having my eyes closed and covered for the duration of
approximately 60-120 minute procedure. Times may vary depending on the type and number of
eyelashes applied.

_______ I understand and agree to the after-care instructions provided for the use and care of my
eyelash extensions. I realize and accept the consequences of failure to adhere to these instructions may
cause the eyelash extensions to fall out and/or decrease the time the lashes will last.

_______ I understand that a touch-up appointment will be necessary a few weeks after my first initial
full-set application and agree that I may be asked to pay an additional fee for the touch-up procedure.

 Unusual sensitivity or skin reactions to Cyanoacrylate- based adhesives
 Moderate to severe allergies in combination with abnormal eye discharge
 Any eye disease or medical condition, such as Conjunctivitis (“pink eye”)
 Recent history of Chemotherapy
 Other medical conditions which would compromise placement and retention of eyelash

extensions

_______ I understand and agree to the maintenance instructions provided and realize that failure to
adhere to these instructions may cause the eyelash extensions to fall out and/or decrease the time the
lashes will last.

 No waterproof mascara
 No oil-based products around the eye area
 No water can come in contact with the eye area for 24hrs after application
 No tinting or perming of the eyelash extensions
 No pulling or rubbing of the eyelash extensions

This agreement will remain in effect for this procedure and all future follow-ups conducted by the
eyelash extension professional. I represent that I am over 18 years of age and that I have the right to
enter this agreement, or if I am under 18 years of age, I have had my parent or legal guardian consent to
