Guide
WELCOME
Our employees are our most valuable asset. Creating a culture of wellness that rewards those employees for taking
care of their health and improving their overall well-being is an important part of Green Business ’s Employee Care values. The
Wellness Program has been revamped for 2017 and now provides you with the opportunity to reduce your monthly medical pre-
miums by competing the necessary requirements.
This guide provides a brief overview of the Wellness Program offered through Wellworks. For more information, contact your
Human Resources department.
Getting Started.
If you have not yet registered on the Wellworks portal, you’ll want to do that now. Visit www.wellworksforyoulogin.com and
create a user account. You can also download the mobile app by searching
“Wellworks For You” in the App Store or Google Play.
Initial Sign On
Your username will be (HSC_(first four letters of your last name)(six-digit date of birth).
Example: HSC_Smit080159
Your default password will be your date of birth (MMDDYY). You will be prompted to
change your password after your initial login.

How Can I Participate?
1. Employees enrolled in the Green Business medical plan must each complete an onsite biometric screening OR an annual
physical with a physician between December 1, 2016 and September 30, 2017. If you complete the physician form, it must
be submitted to Wellworks by September 30, 2017.
Biometric screenings and physical exams are an essential way to determine your current health status. These screenings
provide valuable information, including your blood pressure, total cholesterol, and glucose numbers, as well your height,
weight, and BMI.
Check with your local Human Resource Representative to see if a Biometric Event will be held at your site. If you work at a
site that is no offering a Biometric Event, you can still have a free biometric screening. Schedule a routine preventive
appointment with your Primary Care physician. When you go to your appointment, be sure to take the physician form with
you. The doctor will submit it to Wellworks on your behalf.
If you have already completed your annual exam, please have your doctor fax your lab results to Wellworks to complete the
requirement.
2. Employees enrolled in the Green Business medical plan must each complete the General Health Assessment on the
Wellworks website by September 30, 2017. You will need to know your biometric information to complete the assessment.
3. If you enrolled in the 2017 medical plan as a non-tobacco user, no further action is required. If you are a tobacco user and
would like to earn the incentive on your medical premiums, you need to complete the Tobacco Cessation program on the
Wellworks website AND complete 3 tobacco cessation coaching calls. The tobacco cessation program online takes 6
weeks to complete. It is important you plan your timing accordingly.
To access the online Tobacco Cessation
program, you will need to complete the
Pre-Module Survey to access Module 1.
After each video, there is a short quiz you must pass
in order to move on to the next module.
Get Rewarded.
For completing these requirements, you will receive a discount on your 2018 medical premiums. Though the specific amount
has not yet been determined, it will be between XXX and XXX per month. You could save between XXX and XXX per year!
Premium incentives will take effect on January 1, 2018.
EXTRA INCENTIVE! Once you have completed an onsite biometric screening OR an annual physical, and once you
have completed the General Health Assessment, you will receive a $25 gift card in the mail. Incentives will be mailed
monthly.
Note: Any employees who enroll in the medical plan on or after July 1, 2017 will be given the wellness premium incentive for
the 2018 plan year and do not need to compete the initiatives in their first calendar year of employment.
