Low-THC Cannabis and Medical Marijuana
Medical marijuana is available in the state of Florida pursuant to Section 381.986, Florida Statues and Article X, Section 29, Florida Constitution and can be purchased in a variety of formulations and routes of administration at state-registered dispensaries. Low-THC cannabis and medical marijuana are forms of plant-based medicine that can be used to effectively manage symptoms for many medical conditions while enhancing overall wellness and quality of life. This Physician Reference Booklet is intended to be a resource for medical personnel to guide you in determining the right formulation, cannabinoid ratio, and route of administration to best meet your patient’s individualized needs.
Low-THC Cannabis:
Low-THC Cannabis contains very low amounts of the psychoactive component tetrahydrocannabinol (THC). Low-THC Cannabis is specifically characterized as having less than 0.8% THC and greater than 10% cannabidiol (CBD). With low-THC cannabis, the medication is intended to deliver the benefits of medical cannabinoids while minimizing the intoxicating effects of THC.
Medical Marijuana:
Medical Marijuana contains larger amounts of THC, specifically, more than 0.8% of THC, in addition to CBD, terpenes, and other cannabinoids. Medical marijuana is intended to deliver the benefits of medical cannabinoids in combination with THC, leveraging the effects of THC to assist patients in managing pain, discomfort, and mood.

The Endocannabinoid System
The Endocannabinoid System (ECS) is a complex network of molecules and receptors that consists of two primary phytocannabinoids receptors: CB1 and CB2 receptors. CB1 receptors are most abundant in the central nervous system and the CB2 receptors are most abundant in the peripheral nervous system, the circulatory system and the digestive system. Endocannabinoids and phytocannabinoids interact with the CB1 and CB2 receptors to regulate many physiological processes including:

Cannabis
Cannabis is a complex plant that has been used medicinally world-wide for over a thousand years. Many clinical studies have been completed in the United States and in international markets, yet, there is much more to learn about the medicinal properties of cannabis and the quest for modern research on cannabis continues to grow.
Cannabinoids:
Cannabis contains over 400 chemical compounds, including cannabinoids, which give the cannabis plant its medical and recreational properties. The marijuana plant produces over 100 different cannabinoids, including THC and CBD, which are the most well-known and most investigated cannabinoids. CBD is known for its relaxing and restorative effects while THC is known for pain relief and promoting mental uplift.
Terpenes:
Terpenes are naturally occurring oils found in cannabis and commonly used herbs, spices, and fruits. Terpenes contribute to the effects of cannabinoids by enhancing and balancing out the overall effects of the medication. When it comes to cannabis, terpenes are the real drivers behind the differences between strains and cannabis varieties. For example, Indica strains which are known for their relaxing effects contain terpenes with calming properties, while Sativa strains contain terpenes that contribute to an uplifting and energizing effect. Clinical Pharmacology
(The following pharmaceutical profile relates to all products.)
The Mechanism of Action of THC is complex and not yet completely understood but is believed to interact with the CB1 and CB2 cannabinoid receptors and may also target opioid and benzodiazepine receptors. CBD is believed to affect the CB1 and CB2 receptors more indirectly through calcium channels, potassium channels, and possibly other electrolyte channels.
THC is rapidly absorbed through inhalation and the extent of which depends on depth of inhalation, lung volume, and exposure time. Through inhalation, peak plasma concentrations may be attained within a few minutes of inhalation. Oral ingestion is much slower compared to inhalation and peak plasma concentrations may be attained within 2 hours. The complexity of metabolism of THC is further complicated because the chemical changes that occur after smoking the plant are different than the changes that occur after ingesting the plant.
THC is metabolized by the liver through interaction with the CYP3A4 and CYP2C9 enzymes. CBD is metabolized through the CYP450 system, specifically interacting with the CYP3A4 and CYP2C19 enzymes. Special consideration should be taken when recommending cannabis products in treatment of patients who are also taking medications that are metabolized through the CYP450 system or who have disease states affecting the normal function of the liver and kidney.3
THC is eliminated through the urine and feces and, depending on the frequency of use, may be detectable in the body up to 25 days in chronic users and as little as 3 days among infrequent users. Half-life can vary from 1.3 days in infrequent users and up to 13 days in frequent users.4

Special considerations:
• Pregnancy: Marijuana has not been proven safe during pregnancy.
• Breast-feeding: Marijuana is likely unsafe during breast feeding. THC is found 6 days post consumption in breast milk.5
• Using cannabis with alcohol may increase intoxicating effects.

Dosing
Patient Titration and Tolerance:
• In general, there are no absolutes when recommending medical cannabis. Practitioners and patients have the freedom to titrate the medication dosing and route of administration to meet individual needs. Finding the right dose will require some trial and error as dosing is dependent on individual body composition and metabolism. New users should start low and go slow and keep a medication journal to assist in finding the right product, dosage and route of administration that works best.
• When choosing the right product for your patient, take into consideration: - Qualifying condition
- Prior cannabis experience
- Product total cannabinoid content, CBD and THC ratio
- Product terpene profile
- Product route of administration, onset and duration
• For patients wishing to avoid a mood-altering experience, it is recommended that they start with a high CBD product like
Calm or Serene.
• For patients with prior cannabis experience, their tolerance for THC may be higher and starting with a stronger dose of
THC may be needed to achieve the desired effect.

Recommended Use
Topical Lotion: Apply lotion liberally to the affected area. Lotion is best used to address localized pain and inflammation in muscles or joints.Lotions that contain THC provide local pain relief without the intoxicating effects. Avoid contact with eyes or mucus membranes.
Vape Pen Inhalation: Use a vape pen for inhalation when you want immediate results. The vape pen offers rapid onset of systemic effects and the effects of the medication begin to fade after a couple of hours. Place the pen to your lips, inhale for 2 seconds and then exhale the vaporized medication. Some coughing may occur. Use caution in patients with respiratory conditions.
Oral Tincture Oils: Use oral tincture oils to obtain
effects that are quick and long lasting. Oral tincture oils may be placed sublingually to obtain a quick onset and then swallow the remaining dose for a longer lasting effect. The initial absorption through the mucus membranes will offer an initial onset that begins within 30 minutes while the digested portion will begin taking effect in about 60 minutes, providing a longer lasting effect that may last up to 6 hours.
TheraGels: TheraGels are easy to swallow soft gels, are digested quickly and begin taking effect in 30 minutes. TheraGels are recommended for patients seeking a longer lasting medication through a more traditional route of administration.
Pure Reserve Oil: The Pure Reserve Oil is a concentrated medication in a viscous oil and is intended to deliver the strongest dose of medication. Before taking a product in the form of the Pure Reserve Oil, it is advised to test results with smaller doses in a vape or tincture oil to best understand how the patient will respond to the medication. Warm the syringe in a cup of warm water for 5 minutes before removing the
cap from the syringe, place the desired dose along the gum line and let it dissolve in the mouth. Pure Reserve Oil begins to take effect within an hour and may last up to 8 hours.
Transdermal Patch: The transdermal patch will provide a low level of medication over a period of up to 12 hours. Place the patch on skin cleansed with alcohol and away from areas that may interfere with clothing such as waist bands or straps. Patches that contain THC are reported to provide pain relief without the intoxicating effects.
Medication routes of administration may be paired together to obtain the desired effect. For example, TheraGels may be paired with a vape pen. The vape pen provides immediate relief and tapers off in a couple hours, while the TheraGels begins taking effect in the first hour and may provide long lasting medication effects up to 6 hours.

Suggested Product Use
To determine which product is best for your patients, consider two things:
1.
Patient Tolerance
For patients with little to no cannabis experience, start with the lowest strength within the recommended product line(s).
For cannabis-experienced patients, start with the lowest strength in the most psychoactive product line recommended for their condition.
2. Desired Psychoactivity
For patients who do not desire a psychoactive efect, it is recommended to start with a CBD-weighted product. For patients who desire a psychoactive e fect, it is recommended to use a balanced or THC-weighted product line.
